Abstract

A treatment device for use with an endoscope having a instrument channel for passage of an endoscopic instrument and a distal end is provided. The treatment device includes an end effector assembly with a base for placement at the distal end of the endoscope, an end effector rotatable relative to the base, and a mounting portion for attaching the base to the endoscope. The mounting portion is resiliently compressible and can be radially compressed for insertion into the instrument channel, whereby an expansion force of the resiliently compressed mounting portion retains the base relative to the distal end of the endoscope. The mounting portion also includes an opening longitudinally extending therethrough and communicable with the instrument channel and sized for receiving an endoscopic instrument therethrough.

Claims (73)

1. A treatment device for use with an endoscope having an instrument channel for passage of an endoscopic instrument and a distal end, the instrument channel defining an inner wall, the endoscope used to perform treatment in a body and being operable from outside the body, comprising:

a) an end effector assembly including,

i) a base for placement at the distal end of the endoscope,

ii) a mounting portion extending proximally from said base, wherein said mounting portion is structured such that when said mounting portion is positioned in the instrument channel of the endoscopic instrument, said mounting portion provides an outward force against the inner wall of the instrument channel and thereby retains the base relative to the distal end of the endoscope, said mounting portion further defining an opening longitudinally extending therethrough communicable with the instrument channel and sized for receiving the endoscopic instrument therethrough,

iii) an end effector rotatable relative to said base, and

iv) a link mechanism coupled to said base and said end effector; and

b) a flexible transmission member having a distal end portion which is capable of being inserted into the body through or adjacent the endoscope, said distal end portion of said flexible transmission member coupled to said link mechanism, said flexible transmission member being actuated by an operation outside the body,

wherein said link mechanism causes rotation of said end effector upon actuation of said flexible transmission member.

2. A treatment device according to claim 1 , wherein:

said mounting portion includes a longitudinal slot in its periphery about which said mounting portion is split, said mounting portion compressible about said slot when positioned in the instrument channel.

3. A treatment device according to claim 2 , wherein:

said longitudinal slot is non-straight.

4. A treatment device according to claim 2 , wherein:

said longitudinal slot extends in a zig-zag.

5. A treatment device according to claim 2 , wherein:

said mounting portion defines a longitudinal axis, and

a proximal portion of said mounting portion receivable within the instrument channel tapers to a smaller profile in a direction transverse to said longitudinal axis without decreasing a diameter of said opening through said proximal portion.

6. A treatment device according to claim 5 , wherein:

said mounting portion defines a longitudinal axis, and

a plane extending through an end surface of said proximal portion of said mounting portion extends at an oblique angle relative to said longitudinal axis.

7. A treatment device for use with an endoscope having an instrument channel for passage of an endoscopic instrument, and a distal end, the instrument channel defining an inner wall, the endoscope used to perform treatment in a body and being operable from outside the body, comprising:

a) an endoscopic suturing assembly including,

i) a base for placement about the distal end of the endoscope,

ii) a mounting portion extending proximally from the base, wherein said mounting portion is structured such that when said mounting portion is positioned in the instrument channel of the endoscopic instrument, said mounting portion provides an outward force against the inner wall of the instrument channel to retain said base relative to the distal end of the endoscope, said mounting portion further defining an opening longitudinally extending therethrough communicable with the instrument channel and sized for receiving the endoscopic instrument therethrough,

iii) a needle holder arm rotatable relative to said base, and

iv) a gear assembly coupled to said base and said needle holder arm, said gear assembly having a first gear element and a second gear element meshing with said first gear element; and

b) a flexible transmission member having a distal end portion which is capable of being inserted into the body through or adjacent the endoscope, said distal end portion of said flexible transmission member coupled to said gear assembly, said flexible transmission member being actuated by an operation outside the body,

wherein said gear assembly causes rotation of said needle holder arm upon actuation of said flexible transmission member.

8. A treatment device according to claim 7 , wherein:

said mounting portion includes a non-straight longitudinal slot in its periphery and said outward force is provided relative to said longitudinal slot.

9. A treatment device according to claim 8 , wherein:

said longitudinal slot extends in a zig-zag.

10. A treatment device according to claim 7 , wherein:

a proximal portion of said mounting portion receivable within the instrument channel tapers to a smaller cross-sectional profile without decreasing a diameter of said opening through said proximal portion.

11. A treatment device according to claim 10 , wherein:

said mounting portion defines a longitudinal axis, and

a plane extending through an end surface of said proximal portion of said mounting portion extends at an oblique angle relative to said longitudinal axis.

12. A treatment device according to claim 7 , further comprising:

a tissue guard extends distally from said mounting portion at a distal side of said base.

13. A treatment device according to claim 12 , wherein:

said mounting portion defines a longitudinal axis, and said tissue guard has a distal end which is obliquely angled relative to said longitudinal axis.

14. A treatment device according to claim 12 , wherein:

said tissue guard includes a longitudinal recess.

15. A treatment device according to claim 7 , further comprising:

a needle coupled to said needle holder arm; and

suture coupled to said needle.

16. A treatment system for use on a mammalian body, comprising:

a) an endoscope having a proximal end, a distal end defining a circumference, a instrument channel with an inner wall defining a first diameter and a distal end, said endoscope sized for insertion into a natural orifice of the mammalian body while being operated from outside the body; and

b) a treatment device, comprising,

i) an end effector assembly positioned on said distal end of said endoscope, said end effector assembly including,

A) a base positioned in contact with said distal end of said endoscope,

B) a mounting portion extending proximally from said base, wherein said mounting portion is structured such that when said mounting portion is positioned in said instrument channel of said endoscopic instrument, said mounting portion provides an outward force against said inner wall of said instrument channel with sufficient force to retain said end effector assembly to said distal end of said endoscope, said mounting portion further defining an opening longitudinally extending therethrough communicable with said instrument channel and sized for receiving said endoscopic instrument therethrough,

C) an end effector rotatable relative to said base, and

D) a link mechanism coupled to said base and said end effector, and

ii) a flexible transmission member having a distal end portion which is capable of being inserted into the body through or adjacent the endoscope, said distal end portion of said flexible transmission member coupled to said link mechanism, said flexible transmission member being actuated by an operation outside the body,

wherein said link mechanism causes rotation of said end effector upon actuation of said flexible transmission member.

17. A treatment system according to claim 16 , wherein:

said end effector is a needle holder arm rotatable relative to said base.

18. A treatment system according to claim 17 , further comprising:

a needle coupled to said needle holder arm; and

suture coupled to said needle.

19. A treatment system according to claim 16 , wherein:

said mounting portion includes a longitudinally extending slot about which said mounting portion is resiliently compressible.

20. A treatment system according to claim 19 , wherein:

said longitudinal slot extends in a non-straight form.

21. A treatment system according to claim 19 , wherein:

said longitudinal slot extends in a zig-zag.

22. A treatment system according to claim 16 , wherein:

said opening has a diameter,

said mounting portion has a proximal portion received within said instrument channel that tapers to a smaller profile without decreasing said diameter through said proximal portion.

23. A treatment system according to claim 22 , wherein:

said mounting portion defines a longitudinal axis, and

a plane extending through an end surface of said proximal portion of said mounting portion extends at an oblique angle relative to said longitudinal axis.
