Abstract

A needle and suture assembly is provided for use with an endoscopic suturing device with a needle holder arm. The needle assembly includes a needle tip and a needle body. The needle tip has a sharp end, a capture groove, a tab groove and a plug portion positioned between the capture groove and the tab groove. The needle body has first and second ends, tip tabs, retainers for removably retaining the needle body relative to the needle holder arm, and a suture opening. The needle tip is fixed relative to the needle body by plastic deformation of the tip tabs into the tab groove. A suture extends into the suture opening of the needle body and is fixed therein.

Claims (41)

1. A needle and suture assembly for use in association with a suturing device having a movable needle holder arm, comprising:

a) a needle assembly including,

a needle tip including a sharp end, a capture groove, a tab groove and a plug portion positioned between the capture groove and the tab groove,

a needle body having first and second ends, tip tabs provided adjacent said first end, flexible tab retainers adjacent said second end for releasably retaining said needle body relative to the needle holder arm, said tab retainers elastically deforming to allow the needle body to be released from the needle holder arm upon application of force therebetween, and a suture opening located between said tip tabs and said retainers,

said needle tip being located relative to said first end of said needle body such that said plug portion seats against said first end of said needle body and said tip tabs are longitudinally aligned with said tab groove, and

said needle tip being fixed relative to said needle body by plastic deformation of said tip tabs into said tab groove; and

b) a suture extending into said suture opening of said needle body and fixed therein.

2. A needle and suture assembly according to claim 1 , wherein:

said needle body is a tubular member.

3. A needle and suture assembly according to claim 2 , wherein:

said tubular member includes a circumferential wall, and

said tip tabs and said retainers are defined in said circumferential wall of said tubular member.

4. A needle and suture assembly according to claim 2 , wherein:

said needle body defines a suture hole and a suture retention tab, said suture extending through said suture hole and said suture retention tab plastically deformed into an interior of said tubular body to fix said suture within said needle body.

5. A needle and suture assembly according to claim 1 , wherein: said needle body is straight.

6. An endoscopic suturing system for use within a mammalian body, comprising:

a) an endoscope having a proximal end and a distal end, and an internal instrument channel for passage of an endoscopic instrument from said proximal end to said distal end;

b) an endoscopic suturing device including,

i) an end effector assembly attachable at said distal end of said endoscope, said end effector assembly including,

a base couplable relative to said distal end of said endoscope,

a needle holder arm rotatable relative to said base, said needle holder arm having a straight needle coupling tip defining an axis, and

a link mechanism coupled to said base and said needle holder arm, and

ii) a flexible transmission member having a distal end portion which is capable of being inserted into the mammalian body through or adjacent the endoscope, said distal end portion of said flexible transmission member coupled to said link mechanism, said flexible transmission member being actuated by an operation outside the mammalian body,

wherein said link mechanism causes rotation of said needle holder arm upon actuation of said flexible transmission member;

c) a straight needle having a sharp first end and a circumferential capture recess adjacent said sharp end, and an opposite second end provided with a retaining structure into which the needle holder arm is coaxially received and retained relative to said needle, said needle holder arm being removed from retention within said retaining structure by application of a first longitudinal force in a direction away from said needle coupling tip of said needle holder arm and coaxial with said axis of said needle coupling tip;

d) a suture having first and second ends, said first end fixed to said needle; and

e) a needle capture device having a flexible shaft insertable through said instrument channel of the endoscope, a capture device provided at a distal end of said shaft, and a handle provided a proximal end of said shaft and operable from outside the mammalian body, operation of said handle moving said capture device between an disengaged configuration and an engaged configuration,

wherein in said disengaged configuration said capture device is readied for receiving said sharp first end of said needle and said capture device does not apply a longitudinal force to said needle at least as great as said first longitudinal force, and

wherein in said engaged configuration said capture device engages said capture recess of said needle so that the force of retention of said needle within said capture device is at least as great as said first longitudinal force when said needle holder arm is rotated away from said capture device, such that said needle is retained within said capture device.

7. An endoscopic suture system according to claim 6 , wherein:

said needle holder arm has a reduced diameter portion defining a groove and a tapered end that is received within the retaining structure.

8. An endoscopic suture system according to claim 7 , wherein:

said needle includes a tubular body portion, and said retaining structure includes at least one tab formed in the tubular body portion and plastically deformed into an interior of said tubular body portion to engage within said groove.

9. An endoscopic suture system according to claim 6 , wherein:

said capture recess is a circumferential groove.

10. An endoscopic suture system according to claim 6 , wherein:

said needle comprises a cylindrically tubular body portion and a separate needle tip joined to said tubular body portion.

11. An endoscopic suture system according to claim 10 , wherein:

said needle tip is joined to said tubular body portion using a mechanical interlock.

12. An endoscopic suture system according to claim 10 , wherein:

said tubular body portion defines a suture hole and a suture retention tab, said suture extending through said suture hole and said suture retention tab plastically deformed into an interior of said tubular body to fix said suture within said needle body.
