Abstract

A needle capture device includes a flexible tubular member, a flexible shaft extending through the tubular member, a proximal handle for moving the shaft and tubular member relative to each other, a housing at the distal ends of the shaft and tubular member, and a capture mechanism situated within housing. The housing has an alternating arrangement of flights and spaces radially arrayed about its exterior dimensioned to be closely received within an endoscopic instrument channel so as to coaxially orient said housing with the instrument channel. The capture mechanism has a first configuration in which it is adapted to receive and retain a needle presented into the distal end of the instrument channel, and a second configuration in which it is adapted to release the needle from the instrument channel.

Claims (74)

1. A needle capture device for use an endoscope and sized for insertion through a instrument channel of the endoscope, said needle capture device comprising:

a) a flexible tubular member having a proximal end and a distal end;

b) a flexible shaft having a proximal end and a distal end, said shaft extending through said tubular member;

c) a proximal handle to longitudinally displace said shaft and tubular members relative to each other;

d) a housing having a proximal end fixed to said distal end of said flexible tubular member and a distal end; and

e) three rigid tubular members within said housing including,

i) a rigid outer tubular member having a proximal end fixed to said distal end of said flexible tubular member, a distal end fixed to said distal end of said housing, at least one coupling tab plastically deformed into an interior of said outer tubular body, a latch tab plastically deformed into an interior of said outer tubular body, and a latch release tab proximally displaced from said latch tab and extending into an interior of said outer tubular body, wherein said latch release tab can be resiliently moved radially outward to cause radially outward displacement of said latch tab,

ii) a rigid intermediate tubular member coaxially provided within said outer tubular member, said intermediate tubular member having a proximal end fixed to said distal end of said flexible shaft, a distal end, a wall extending between the proximal and distal ends, and at least one longitudinally extending slot defined in said wall between said proximal and distal ends, and

iii) a rigid inner tubular member coaxially provided within said intermediate tubular member, said inner tubular member having a proximal end and a distal end and defining at least one coupling slot,

each of said at least one longitudinally extending slot of said intermediate tubular member being longer in length than said at least one coupling slot of said inner tubular member

said at least one coupling tab plastically deformed to extend through said at least one longitudinally extending slot in said intermediate tubular member and into said at least one coupling slot to fix the position of said inner tubular member relative to said outer tubular member,

said intermediate tubular member longitudinally displacable relative to said outer and inner tubular members in response to operation of said proximal handle, wherein operation of said proximal handle to longitudinally displace said intermediate tubular member distally within said outer tubular member causes a portion of said intermediate tubular member to resiliently move said latch release tab radially outward to cause radially outward displacement of said latch tab.

2. A needle capture device according to claim 1 , wherein:

said flexible tubular member has an exterior diameter, and said housing has an exterior diameter larger than said exterior diameter of said flexible tubular member.

3. A needle capture device according to claim 1 , wherein:

said housing includes an exterior and an alternating arrangement of flights and spaces radially arrayed about said exterior.

4. A needle capture device according to claim 3 , wherein:

said housing includes two sets of said flights and spaces, a first set provided at said distal end of said housing and a second set provided at said proximal end of said housing.

5. A needle capture device according to claim 1 , further comprising:

a surgical needle assembly including a needle having a sharp tip and a capture recess adjacent said tip and a suture having one end fixed to said needle,

wherein when said needle assembly is introduced into said housing in a tip first orientation, said needle tip is centered within said distal end of said inner tubular member.

6. A needle capture device according to claim 5 , wherein:

when said intermediate tubular member is in a relatively proximal displacement within said outer tubular member, said latch tab engages said capture recess to retain said needle relative to said housing, and

when said intermediate tubular member is in a relatively distal displacement within said outer tubular member, said latch tab is disengaged from said capture recess.

7. A needle capture device according to claim 5 , wherein:

said housing includes an exterior and an alternating arrangement of flights and spaces radially arrayed about said exterior, and said spaces are sized widthwise to receive the suture extending longitudinally therethrough.

8. An endoscopic needle exchange system for use with a surgical needle, comprising:

a) an endoscope having a proximal end, a distal end defining a circumference, a instrument channel with a first diameter and a distal end, the endoscope sized for insertion into a natural orifice of the mammalian body while being operated from outside the body; and

b) a needle capture device, comprising,

i) a flexible tubular member having a proximal end and a distal end,

ii) a flexible shaft having a proximal end and a distal end, said shaft extending through said tubular member,

iii) a proximal handle to longitudinally displace said shaft and tubular member relative to each other,

iv) a housing having a proximal end fixed to said distal end of said flexible tubular member, a distal end, an interior, an exterior, and an alternating arrangement of flights and spaces radially arrayed about said exterior, said flights dimensioned to be closely received within said distal end of said instrument channel so as to coaxially orient said housing with said instrument channel, and

v) a capture mechanism within said interior of said housing having a first element coupled to said flexible shaft and a second element coupled to said flexible tubular member, wherein in a first configuration of said first and second elements said capture mechanism is adapted to receive and retain the needle presented into said distal end of said instrument channel, and in a second configuration of said first and second elements said capture mechanism is adapted to release the needle from the instrument channel.

9. A needle exchange system according to claim 8 , wherein:

in said second configuration, said first and second element are longitudinally displaced relative to said first configuration.

10. A needle exchange system according to claim 8 , wherein:

said capture mechanism comprises,

A) a rigid outer tubular member having a proximal end fixed to said distal end of flexible tubular member, a distal end fixed at said distal end of said housing, at least one coupling tab plastically deformed into an interior of said outer tubular body, a latch tab plastically deformed into an interior of said outer tubular body for engagement with the capture recess in the surgical needle, and a latch release tab proximally displaced from said latch tab and extending into an interior of said outer tubular body, wherein said latch release tab can be resiliently moved radially outward to cause radially outward displacement of said latch tab,

B) a rigid intermediate tubular member coaxially provided within said outer tubular member, said intermediate tubular member having a proximal end fixed to said distal end of said flexible shaft, a distal end, a wall extending between the proximal and distal ends, and at least one longitudinally extending slot defined in said wall between said proximal and distal ends, and

C) a rigid inner tubular member coaxially provided within said intermediate tubular member, said inner tubular member having a proximal end and a distal end and defining at least one coupling slot,

each of said at least one longitudinally extending slot of said intermediate tubular member being longer in length than said at least one coupling slot of said inner tubular member

said at least one coupling tab plastically deformed to extend through said at least one longitudinally extending slot in said intermediate tubular member and into said at least one coupling slot to fix the position of said inner tubular member relative to said outer tubular member while allowing said intermediate tubular member to be longitudinally displaced relative to said outer and inner tubular members in response to operation of said proximal handle,

wherein operation of said proximal handle to longitudinally displace said intermediate tubular member distally within said outer tubular member causes a portion of said intermediate tubular member to resiliently move said latch release tab radially outward to cause radially outward displacement of said latch tab.

11. A needle exchange system according to claim 10 , further comprising:

said surgical needle having a sharp tip and a capture recess adjacent said tip and a suture having one end fixed to said needle,

wherein when said needle is introduced into said housing in a tip first orientation, said needle tip is centered within said distal end of said inner tubular member.

12. A needle exchange system according to claim 11 , wherein:

in said first configuration, said latch tab engages said capture recess, and

in said second configuration, said intermediate tubular member is displaced distally within said outer tubular to release said latch tab from said capture recess.

13. A needle exchange system according to claim 10 , wherein:

in said first configuration, said latch tab engages said capture recess, and

in said second configuration, said intermediate tubular member is displaced distally within said outer tubular to release said latch tab from said capture recess.

14. An endoscopic needle system for use with a surgical needle, comprising:

a) an endoscope having a proximal end with a handle, a distal end defining a circumference, a instrument channel with a first length and a first diameter, the endoscope sized for insertion into a natural orifice of the mammalian body while being operated from outside the body; and

b) a needle capture device, comprising,

i) a flexible tubular member having a proximal end and a distal end,

ii) a flexible shaft having a proximal end and a distal end, said shaft extending through said tubular member,

iii) a proximal handle to longitudinally displace said shaft and tubular member relative to each other,

iv) a housing having a proximal end fixed to said distal end of said flexible tubular member, a distal end, an interior, an exterior dimensioned to be closely received within said distal end of said instrument channel so as to coaxially orient said housing with said instrument channel, and

v) a capture mechanism within said interior of said housing having a first element coupled to said flexible shaft and a second element coupled to said flexible tubular member, wherein in a first configuration of said first and second elements said capture mechanism is adapted to receive and retain the needle presented into said distal end of said instrument channel, and in a second configuration of said first and second elements said capture mechanism is adapted to release the needle from the instrument channel,

wherein said lengths of said flexible shaft and said flexible tubular member are selected to locate said distal end of said housing within said distal end of said instrument channel without protruding therefrom when said needle capture device is fully inserted into said instrument channel of said endoscope.

15. A needle exchange system according to claim 14 , wherein:

said housing includes an exterior and an alternating arrangement of flights and spaces radially arrayed about said exterior.

16. A needle exchange system according to claim 14 , further comprising:

said surgical needle having a sharp tip and a capture recess adjacent said tip and a suture having one end fixed to said needle.

17. A needle exchange system according to claim 14 , wherein:

said capture mechanism comprises,

A) a rigid outer tubular member having a proximal end fixed to said distal end of flexible tubular member, a distal end fixed at said distal end of said housing, at least one coupling tab plastically deformed into an interior of said outer tubular body, a latch tab plastically deformed into an interior of said outer tubular body for engagement with the capture recess in the surgical needle, and a latch release tab proximally displaced from said latch tab and extending into an interior of said outer tubular body, wherein said latch release tab can be resiliently moved radially outward to cause radially outward displacement of said latch tab,

B) a rigid intermediate tubular member coaxially provided within said outer tubular member, said intermediate tubular member having a proximal end fixed to said distal end of said flexible shaft, a distal end, a wall extending between the proximal and distal ends, and at least one longitudinally extending slot defined in said wall between said proximal and distal ends, and

C) a rigid inner tubular member coaxially provided within said intermediate tubular member, said inner tubular member having a proximal end and a distal end and defining at least one coupling slot,

each of said at least one longitudinally extending slot of said intermediate tubular member being longer in length than said at least one coupling slot of said inner tubular member

said at least one coupling tab plastically deformed to extend through said at least one longitudinally extending slot in said intermediate tubular member and into said at least one coupling slot to fix the position of said inner tubular member relative to said outer tubular member while allowing said intermediate tubular member to be longitudinally displaced relative to said outer and inner tubular members in response to operation of said proximal handle,

wherein operation of said proximal handle to longitudinally displace said intermediate tubular member distally within said outer tubular member causes a portion of said intermediate tubular member to resiliently move said latch release tab radially outward to cause radially outward displacement of said latch tab.
