Abstract

The invention relates generally to methods and systems for the treatment of pelvic tissues using a high pressure injection device to deliver a therapeutic composition that includes a large molecule therapeutic agent, such as a nucleic acid or a polypeptide, to a target tissue in the pelvic area. Methods of the invention can improve delivery of the therapeutic agent into the tissue, which can be beneficial for the treatment of a pelvic tissue disorder, such as bladder and prostate tumors.

Claims (13)

1. A method for treating or preventing a disease of the pelvic floor in a subject comprising a step of delivering a composition comprising a therapeutic agent selected from the group consisting of nucleic acids and polypeptides to a tissue of the male or female urogenital tract or lower gastrointestinal tract within the subject's body, wherein the tissue is selected from bladder, prostrate, urethral, kidney, vaginal, and endometrial tissues, and wherein the nucleic acid or polypeptide is present in a nanoparticle or microparticle comprising an excipient polymeric material selected from the group consisting of polylactic acid, poly(lactide-co-glycolide), polycaprolactone, polyhydroxybutyrate, polyorthoesters, polyphosphazine, polymethylidenemalonate, polyalkeneanhydrides, polypeptides, and polyanhydrides, where the composition is delivered to the tissue using a pressure in the range of 100 psi to 4000 psi, and therapeutic agent treats or prevents the disease in the tissue.

2. The method of claim 1 wherein the composition is delivered to the tissue using a pressure in the range of 140 psi to 2000 psi.

3. The method of claim 1 , wherein the composition volume is in the range of 0.1 cm 3 to 2 cm 3 .

4. The method of claim 1 , wherein the composition volume is in the range 0.25 cm 3 to about 1 cm 3 .

5. The method of claim 1 wherein the method comprises using a high-pressure injection device comprising a distal portion comprising a shaft configured for insertion into the urethra of the subject.

6. The method of claim 1 wherein the high-pressure injection device comprises a needleless distal end having one or more aperture(s) for the ejection of the composition.

7. The method of claim 1 wherein the tissue is bladder tissue.

8. The method of claim 7 wherein the composition is administered to an area of the bladder tissue following surgical resection to remove a malignant bladder tissue growth.

9. The method of claim 1 , wherein the nucleic acid promotes apoptosis of the cells of the tissue the composition is injected into.

10. The method of claim 1 , wherein the nucleic acid promotes an RNA interference pathway to modulate expression of a target gene.

11. The method of claim 1 , wherein the nucleic acid is present in a viral particle.

12. The method of claim 1 , wherein the nucleic acid is present in a nanoparticle.

13. The method of claim 12 , wherein the therapeutic agent is present within the nanoparticles.
