Abstract

Devices and methods for tissue transfer are described where a cannula may be inserted into the breast of a subject at one of several points of entry. Insertion of the cannula into the breast may be accomplished by using a guidance system to distinguish between tissue types. Once desirably positioned, the cannula may be withdrawn from the breast while automatically (or manually) injecting the fat in multiple deposits of adipose tissue or fat such that the deposited fat remains within the tract formed by the withdrawn cannula. Multiple tracts of the deposited fat may be injected within the breast until the breast has been desirably remodeled and/or augmented.

Claims (37)

1. A tissue transfer system, comprising:

a handle assembly, the handle assembly including (i) a handle extending in-line with a tissue injection cannula, (ii) an actuation mechanism, and (iii) an angled receiving section configured to removably receive at least one reservoir, the angled receiving section extending from the handle at an acute angle;

the at least one reservoir removably attachable from the angled receiving section such that the at least one reservoir extends at an angle relative to the handle to facilitate manipulation of the handle, wherein an intersection between the handle and the at least one reservoir provides a grip section to a user;

the tissue injection cannula extending in-line with the handle such that the at least one reservoir is angled relative to the tissue injection cannula, wherein the tissue injection cannula defines at least one opening near or at a distal end of the tissue injection cannula and a proximal end of the tissue injection cannula is removably attachable to the handle; and

wherein the at least one reservoir and injection cannula are in fluid communication with one another through the handle.

2. The system of claim 1 , wherein the actuation mechanism comprises a pumping mechanism.

3. The system of claim 2 , wherein the pumping mechanism comprises a reversible piston configured to provide pressure through the pump.

4. The system of claim 3 , wherein the pumping mechanism includes a one-way valve.

5. The system of claim 3 , further comprising a controller in electrical communication with the pumping mechanism, wherein the controller is configured to receive pumping feedback.

6. The system of claim 1 , wherein the tissue injection cannula is laterally offset relative to the handle.

7. The system of claim 1 , further comprising a harvesting reservoir assembly couplable to the handle via a tubing.

8. The system of claim 1 , wherein the actuation mechanism comprises a lead screw and a plunger coupled to the lead screw.

9. The system of claim 1 , further comprising a tissue detection sensor positioned through or along the tissue injection cannula and configured to distinguish a tissue type.

10. The system of claim 9 , wherein the tissue detection sensor comprises one or more optical fibers positioned near or at a distal end of the tissue injection cannula.

11. The system of claim 9 , further comprising a photo detector in optical communication with the one or more optical fibers.

12. The system of claim 11 , further comprising a processor in electrical communication with the photo detector, wherein the processor is programmed to distinguish tissue types.

13. The system of claim 1 , further comprising a tissue harvesting cannula defining at least one opening near or at a distal end of the harvesting cannula, wherein a proximal end of the tissue harvesting cannula is removably attachable to the handle and is couplable to the actuation mechanism.

14. The system of claim 13 , wherein the tissue harvesting cannula further comprises a rotatable cutting member within the at least one opening.

15. A tissue injection system, comprising:

a handle having an actuation mechanism;

an injection cannula removably attachable to the handle and extending in-line with the handle;

a reservoir fluidly coupled to a proximal end of the injection cannula through the handle, wherein the reservoir is removably attachable to the handle such that the reservoir extends in-line relative to the handle and to the injection cannula, and the reservoir comprises a harvesting reservoir assembly attachable to the handle;

a controller in electrical communication with the actuation mechanism; and

a tissue harvesting cannula defining at least one opening near or at a distal end of the tissue harvesting cannula, wherein a proximal end of the tissue harvesting cannula is removably attachable to the handle such that the tissue harvesting cannula is in fluid communication with the harvesting reservoir assembly and is couplable to the actuation mechanism;

wherein the controller is configured to receive pumping feedback, and

wherein an entire structure of the handle extends in-line with the injection cannula.

16. The system of claim 15 , wherein the tissue harvesting cannula further comprises a rotatable cutting member within the at least one opening.

17. The system of claim 15 , wherein the reservoir is attachable to the handle via a tubing.

18. The system of claim 15 , wherein the actuation mechanism comprises a pumping mechanism.

19. The system of claim 18 , wherein the pumping mechanism comprises a reversible piston configured to provide pressure through the pump.

20. The system of claim 19 , wherein the pumping mechanism includes a one-way valve.

21. The system of claim 15 , further comprising a harvesting reservoir assembly couplable to the handle via a tubing.

22. The system of claim 15 , wherein the pumping feedback comprises a distance traveled by a piston within the actuation mechanism.

23. The system of claim 15 , wherein the actuation mechanism comprises a lead screw and a plunger coupled to the lead screw.

24. The system of claim 15 , further comprising a tissue detection sensor positioned through or along the injection cannula and configured to distinguish a tissue type.

25. The system of claim 24 , wherein the tissue detection sensor comprises a photo detector in optical communication with one or more optical fibers positioned near or at a distal end of the injection cannula.

26. The system of claim 25 , further comprising a processor in electrical communication with the photo detection, wherein the processor is programmed to distinguish tissue types.
