Method and apparatus for minimally invasive insertion of intervertebral implants
View Patent ↗A dilation introducer for orthopedic surgery is provided for minimally invasive access for insertion of an intervertebral implant. The dilation introducer may be used to provide an access position through Kambin's triangle from a posterolateral approach. A first dilator tube with a first longitudinal axis is provided. A second dilator tube may be introduced over the first, advanced along a second longitudinal axis parallel to but offset from the first. A third dilator tube may be introduced over the second, advanced along a third longitudinal axis parallel to but offset from both the first and the second. An access cannula may be introduced over the third dilator tube. With the first, second, and third dilator tubes removed, surgical instruments may pass through the access cannula to operate on an intervertebral disc and/or insert an intervertebral implant.
1. A method of inserting a spinal implant into an intervertebral disc, the method comprising: defining a trajectory through a Kambin's triangle of the patient and towards a center of the intervertebral disc by an instrument extending through the Kambin's triangle; enlarging the Kambin's triangle about said trajectory by the instrument to define a pathway positioned about said about said trajectory; and advancing the spinal implant through the instrument along said trajectory utilizing the enlarged Kambin's triangle and into the intervertebral disc.
2. The method of inserting a spinal implant into an intervertebral disc of claim 1 , further comprising expanding a height of the spinal implant within the intervertebral disc.
3. The method of inserting a spinal implant into an intervertebral disc of claim 2 , wherein enlarging the Kambin's triangle is performed at least partially using cutting surfaces on one or more dilation tubes.
4. The method of inserting a spinal implant into an intervertebral disc of claim 1 , wherein enlarging the Kambin's triangle is performed at least partially using cutting surfaces on one or more dilation tubes.
5. The method of inserting a spinal implant into an intervertebral disc of claim 1 , wherein the trajectory into the disk can be between 45 and 55 degrees from a midline of the intervertebral disc.

The method of inserting a spinal implant into an intervertebral disc of claim 1 , wherein the instrument comprises a plurality of dilators with cutting surfaces and an access catheter, wherein the plurality of dilators and the access catheter are configured to be sequentially advanced over each other into the Kambin's triangle.
7. The method of inserting a spinal implant into an intervertebral disc of claim 6 , wherein the step of enlarging the Kambin's triangle comprises forming a recess along an superior border of an inferior vertebrae.
8. The method of inserting a spinal implant into an intervertebral disc of claim 7 , further comprising placing an access catheter against the recess formed along the superior border of the inferior vertebrae.
9. The method of inserting a spinal implant into an intervertebral disc of claim 6 , further comprising advancing the spinal implant through an access catheter.
