Abstract

An apparatus to treat a sphincter has a support member. A sphincter electropotential mapping device includes a mapping electrode. The sphincter electropotential mapping device is coupled to the support member and configured to detect aberrant myoelectric activity of the sphincter.

Claims (23)

1. An apparatus for treating a dysfunction in tissue at or near a sphincter comprising:

a catheter capable of being deployed at or near the sphincter, the catheter including a plurality of elongated arms forming an expandable basket assembly,

a mapping assembly including at least one mapping electrode carried by the arms for detecting electrical events in the tissue to detect abnormal relaxation of the sphincter,

an energy delivery device including a plurality of electrodes with tissue penetrating tips coupled to the arms to deliver energy to produce lesions, and

a controller coupled to the at least one mapping electrode and to the energy delivery device, said controller producing an electropotential map of the electrical events in the tissue; and

a feedback mechanism for selective on-off switching of different electrodes of the plurality of electrodes in a desired pattern.

2. The apparatus of claim 1 further comprising:

an output device connected to said controller, said output device capable of displaying said electropotential map.

3. The apparatus of claim 1 wherein at least a portion of the apparatus is radiopaque.

4. The apparatus of claim 1 further comprising a sheath for introducing the mapping assembly into the sphincter.

5. The apparatus of claim 1 , wherein a plurality of mapping electrodes are spaced apart on each elongated arm.

6. The apparatus of claim 5 , further comprising a balloon expandable to expand the basket.

7. The apparatus of claim 1 , wherein the arms have a lumen for fluid delivery.

8. An apparatus for treating a dysfunction in tissue at or near a sphincter comprising:

a catheter capable of being deployed at or near the sphincter, the catheter having an expandable member,

a mapping assembly comprising a plurality of mapping electrodes carried by the catheter for detecting electrical events in the tissue,

a plurality of treatment electrodes slidable with respect to the catheter and spaced from the mapping electrodes to deliver energy to tissue at or near the sphincter to disrupt generation of sufficient aberrant electrical signals to cause relaxation of a wall of the sphincter,

a feedback mechanism for selective on-off switching of different energy delivery electrodes in a desired pattern, and

a controller coupled to the mapping electrodes, said controller producing an electropotential map of the electrical events in the tissue, the mapping electrodes being positioned on a circumferential surface of the expandable member.

9. The apparatus of claim 8 , wherein the expandable member creates a three dimensional array of mapping electrodes.

10. The apparatus of claim 8 , further comprising a plurality of treatment electrodes on the expandable member.

11. The apparatus of claim 8 , wherein the mapping electrodes are positioned in a radial pattern.

12. The apparatus of claim 8 , wherein the mapping electrodes are positioned in a longitudinal pattern.
