Abstract

A trocar fixation device can comprise a cannula, at least one flap coupled to an exterior surface of the cannula within a central region of the cannula, and an elongate tube rotatably mounted onto the cannula and over the at least one flap. The elongate tube has at least one opening extending between an interior surface and an exterior surface of the elongate tube. In a free, activated state, the flap biases radially outwardly from the cannula. In a constrained, deactivated state, the at least one flap is positioned between the cannula and the elongate tube and maintained substantially parallel to the exterior surface of the cannula. The at least one opening in the elongate tube is sized and positioned such that rotation of the elongate tube in a first direction about the cannula exposes the flap in its entirety through the opening and allows the flap to activate.

Claims (29)

1. A trocar fixation device for placement through a body wall, comprising:

a cannula having an interior surface, an exterior surface, a proximal end, a distal end, a lumen extending between the proximal end and the distal end to receive instruments therethrough, a proximal-end region, a distal-end region, and a central region that is positioned between the proximal-end region and the distal-end region;

a plurality of flaps coupled to the exterior surface of the cannula within the central region of the cannula, each flap having a substantially rhomboid shape, and each flap comprising a first edge extending from a first end to a second end, the first edge coupled to the exterior surface of the cannula, a second edge extending from the first end of the first edge, a third edge extending from the second end of the first edge, and a fourth edge extending substantially parallel to the first edge between the second edge and third edge, the second, third, and fourth edges deployable from the exterior surface of the cannula; and

a fixation mechanism external to the cannula to selectively deploy the plurality of flaps radially outward from the central region of the cannula, the fixation mechanism comprising an elongate tube rotatably mounted onto the cannula and over the plurality of flaps, the elongate tube having a proximal end, a distal end, a lumen extending between the proximal end and the distal end with the cannula extending through the lumen of the elongate tube such that the proximal end of the cannula is positioned proximal to the proximal end of the elongate tube and the distal end of the cannula is positioned distal to the distal end of the elongate tube, an interior surface, an exterior surface, a proximal-end region, a distal-end region, a central region that is positioned between the proximal-end region and the distal-end region, and at least one opening extending between the interior surface and the exterior surface;

wherein in a free, activated state, a first array of flaps of the plurality of flaps biases radially outwardly from the cannula to substantially prevent proximal movement and a second array of flaps of the plurality of flaps biases radially outwardly from the cannula to substantially prevent distal movement of the cannula relative to the body wall and in a constrained, deactivated state, the plurality of flaps is positioned between the cannula and the elongate tube and maintained substantially parallel to the exterior surface of the cannula; and

the at least one opening in the elongate tube is sized and positioned such that rotation of the elongate tube in a first direction about the cannula exposes the plurality of flaps in its entirety through the at least one opening and allows each flap of the plurality of flaps to activate.

2. The trocar fixation device of claim 1 , the plurality of flaps being aligned substantially parallel to a longitudinal axis of the cannula.

3. The trocar fixation device of claim 2 , wherein the first and fourth edges of each flap of the plurality of flaps are substantially perpendicular to the longitudinal axis of the cannula.

4. The trocar fixation device of claim 1 , wherein the elongate tube possesses sufficient stiffness to collapse the plurality of flaps during deactivation of the fixation device.

5. The trocar fixation device of claim 1 , wherein:

continued rotation of the elongate tube in the first direction causes a first edge of the at least one opening to be positioned under a portion of each activated flap of the plurality of flaps, thereby substantially supporting the plurality of flaps in the activated state;

rotating the elongate tube in a second direction, substantially opposite the first direction, about the cannula removes support for the plurality of flaps; and

continued rotation of the elongate tube in the second direction causes a second edge of the at least one opening to slide over the plurality of flaps and to collapse and deactivate the plurality of flaps.

6. The trocar fixation device of claim 1 , wherein the plurality of flaps is overmolded onto the central region of the cannula.

7. The trocar fixation device of claim 1 , wherein the plurality of flaps is formed of polyethylene.

8. The trocar fixation device of claim 1 , wherein elongate tube is formed of polyethylene.

9. The trocar fixation device of claim 1 , wherein the plurality of flaps is formed in a strip of material that is coupled to the cannula.

10. A trocar fixation device for placement through a body wall, comprising:

a cannula having an interior surface, an exterior surface, a proximal end, a distal end, a lumen extending between the proximal end and the distal end to receive instruments therethrough, a proximal-end region, a distal-end region, and a central region that is positioned between the proximal-end region and the distal-end region;

a plurality of flaps coupled to the exterior surface of the cannula within the central region of the cannula, each flap having a substantially rhomboid shape, and each flap comprising a first edge extending from a first end to a second end, the first edge coupled to the exterior surface of the cannula, a second edge extending from the first end of the first edge, a third edge extending from the second end of the first edge, and a fourth edge extending substantially parallel to the first edge between the second edge and third edge, the second, third, and fourth edges deployable from the exterior surface of the cannula; and

a fixation mechanism external to the cannula to selectively deploy the plurality of flaps radially outward from the central region of the cannula, the fixation mechanism comprising an elongate tube rotatably mounted onto the cannula and over the plurality of flaps, the elongate tube having a proximal end, a distal end, a lumen extending between the proximal end and the distal end with the cannula extending through the lumen of the elongate tube such that the proximal end of the cannula is positioned proximal to the proximal end of the elongate tube and the distal end of the cannula is positioned distal to the distal end of the elongate tube, an interior surface, an exterior surface, a proximal-end region, a distal-end region, a central region that is positioned between the proximal-end region and the distal-end region, and at least one opening extending between the interior surface and the exterior surface;

wherein in a free, activated state, each flap of the plurality of flaps biases radially outwardly from the cannula such that a first array of flaps of the plurality of flaps is positioned to substantially prevent proximal movement and a second array of flaps of the plurality of flaps is positioned to substantially prevent distal movement of the cannula relative to the body wall and in a constrained, deactivated state, the plurality of flaps is positioned between the cannula and the elongate tube and maintained substantially parallel to the exterior surface of the cannula;

the elongate tube possesses sufficient stiffness to collapse the plurality of flaps during deactivation of the fixation device;

the at least one opening in the elongate tube is sized and positioned such that rotation of the elongate tube in a first direction about the cannula exposes the plurality of flaps in its entirety through the at least one opening and allows the plurality of flaps to activate;

continued rotation of the elongate tube in the first direction causes a first edge of the at least one opening to be positioned under a portion of the each flap of the plurality of flaps, thereby substantially supporting the plurality of flaps in the activated state;

rotating the elongate tube in a second direction, substantially opposite the first direction, about the cannula removes support for the plurality of flaps; and

continued rotation of the elongate tube in the second direction causes a second edge of the at least one opening to slide over the plurality of flaps and to collapse and deactivate the plurality of flaps.

11. The trocar fixation device of claim 10 , the plurality of flaps being aligned substantially parallel to a longitudinal axis of the cannula.

12. The trocar fixation device of claim 10 , wherein the plurality of flaps is overmolded onto the central region of the cannula.
