Abstract

A spinal stabilization system may be formed in a patient. In some embodiments, a minimally invasive procedure may be used to form a spinal stabilization system in a patient. Bone fastener assemblies may be coupled to vertebrae. Each bone fastener assembly may include a bone fastener and a collar. Extenders may be coupled to the collar to allow for formation of the spinal stabilization system through a small skin incision. The extenders may allow for alignment of the collars to facilitate insertion of an elongated member in the collars. An elongated member may be positioned in the collars and a closure member may be used to secure the elongated member to the collars. An adjuster may be used in conjunction with the extenders to change a separation distance between the bone fastener assemblies.

Claims (36)

1. A system for adjusting a distance between vertebral bodies of a human spine, comprising:

a first extender configured to detachably couple to a first bone fastener assembly that is securable to a first vertebral body;

a second extender configured to detachably couple to a second bone fastener assembly that is securable to a second vertebral body, wherein one of the first and second bone fastener assemblies secures to a rod so as to inhibit movement relative to the rod, and the other of the first and second bone fastener assemblies is movably coupled to the rod to allow movement along the rod relative to the secured bone fastener assembly; and

an adjuster having:

a first handle coupled to a shaft, wherein the shaft couples the adjuster to the first extender; and

a second handle having a driver engager, wherein the driver engager receives a driver and thereby couples the adjuster to the second extender;

wherein actuation of the first handle relative to the second handle at a pivot point forces the extender that is coupled to the bone fastener assembly movably coupled to the rod to move towards the extender that is coupled to the bone fastener assembly secured to the rod, whereby the bone fastener assembly movably coupled to the rod is forced to move towards the bone fastener assembly secured to the rod, causing compression of the first and second vertebral bodies.

2. The system of claim 1 , wherein the pivot point is offset from a shaft of the driver, the driver engager, or both.

3. The system of claim 1 , wherein a shaft of the driver is longer than the driver engager.

4. The system of claim 1 , wherein the driver engager includes a shaft that fits over a portion of the second extender.

The system of claim 1 , wherein the adjuster further comprises an engagement mechanism having a movable member and an opening, wherein the movable member is movable within the opening between a first position and a second position to adjust the position of the pivot point along a length of the engagement mechanism.

6. The system of claim 5 , wherein the opening is in the second handle.

7. The system of claim 1 further comprising a locking mechanism coupled to the first handle and the second handle operable to engage so as to prevent relative movement between the first handle and the second handle.

8. An apparatus for adjusting a separation distance between vertebral bodies of a human spine, comprising:

an adjuster, wherein the adjuster comprises:

a first handle coupled to a shaft for detachably engaging the adjuster to a first extender that is coupled to a first bone fastener assembly securable to a first vertebral body; and

a second handle having a driver engager, wherein the driver engager receives a driver and couples the adjuster to a second extender that is coupled to a second bone fastener assembly securable to a second vertebral body, wherein actuation of the first handle relative to the second-handle at a pivot point forces the first and second extenders to move towards each other, thereby forcing the first and second bone fastener assemblies to move towards each other, thereby causing compression of the first vertebral body and the second vertebral body.

9. The apparatus of claim 8 , wherein the driver engager includes a shaft that fits over the second extender.

The apparatus of claim 8 , wherein the adjuster further comprises an engagement mechanism having a movable member and an opening, wherein the pivot point is adjustable via the engagement mechanism, and wherein the movable member is movable within the opening between a first position and a second position.

11. The apparatus of claim 10 , wherein the engagement mechanism allows a separation distance to be established between the first and second handles at the pivot point.

12. The apparatus of claim 8 , further comprising a locking mechanism coupled to the first handle and the second handle operable to engage so as to prevent relative movement between the first handle and the second handle.

13. The apparatus of claim 8 , wherein the pivot point is offset from a shaft of the driver, the driver engager, or both.

14. The apparatus of claim 8 , wherein a shaft of the driver is longer than the driver engager.

15. A method of adjusting a distance between vertebral bodies of a human spine, comprising:

securing first and second bone fastener assemblies to first and second vertebra, respectively;

coupling first and second extenders to the first and second bone fastener assemblies, respectively;

securing the second bone fastener assembly to a rod to inhibit movement of the second bone fastener assembly relative to the rod;

coupling the rod to the first bone fastener assembly, wherein the first bone fastener assembly is movable along the rod;

coupling an adjuster to the first extender via a shaft of the adjuster, wherein the shaft is attached to a first handle of the adjuster;

inserting a driver through a second handle of the adjuster into the second extender, wherein the first handle is coupled to the second handle at a pivot point;

actuating the first handle relative to the second handle at the pivot point to move the first extender relative to the second extender, thereby causing the first bone fastener assembly secured to the first vertebra to move relative to the second bone fastener assembly secured to the second vertebra, thereby adjusting a distance between the first vertebra and the second vertebra.

The method of claim 15 , further comprising loosening an engagement mechanism of the adjuster, adjusting a separation distance between the first and second handles of the adjuster, and tightening the engagement mechanism to inhibit translation of the first and second handles relative to each other.

17. The method of claim 16 , wherein the engagement mechanism includes a movable member and an opening, wherein the movable member defines the pivot point, wherein adjusting the separation distance includes moving the movable member along the opening, thereby adjusting the position of the pivot point.

18. The method of claim 15 , wherein inserting the driver through the second handle includes inserting the driver through a driver engager, wherein the driver engager is disposed over a portion of the second extender.

19. The method of claim 15 , wherein the adjuster comprises a compressor configured to reduce the distance between the first vertebra and the second vertebra.

20. The method of claim 15 , wherein the adjuster comprises a distractor configured to increase the distance between the first vertebra and the second vertebra.
