Abstract

The invention is directed towards a process for implanting a cartilage graft into a cartilage defect and sealing the implanted cartilage graft with recipient tissue. The invention is also directed towards a process for repairing a cartilage defect and implanting a cartilage graft into a human or animal. The invention is further directed toward a repaired cartilage defect.

Claims (38)

1. A process for implanting a cartilage graft into a cartilage defect having a size, contour, and location and being surrounded by a cartilage surface, and sealing the implanted cartilage graft with recipient tissue, comprising

a. creating a cylindrical bore down to a bone portion of the cartilage defect;

b. selecting a cylindrical osteochondral plug that matches the size, contour, and location of the defect site, said osteochondral plug comprising a cartilage cap having a circumferential area and a superficial surface;

c. treating the bore at said defect site with a first bonding agent;

d. treating the circumferential area of said cartilage cap on said osteochondral plug with a second bonding agent; and

e. inserting said osteochondral plug into said defect site and wherein the superficial surface of said cartilage cap is surrounded by the cartilage surface surrounding the defect site,

wherein said first and second bonding agents are activated by applying a stimulation agent that induces sealing, integration, and hydrodynamic restoration of the recipient tissue, and

wherein said first and second bonding agent are independently one or more photoactive dyes selected from the group consisting of bis-diazopyruvamide-N,N9-bis(3-diazopyruvoyl)-2,29-(ethylenedioxy)bis-(ethylamine) (DPD), diazopyruvoyl (DAP), phloxime, thionine, acridine orange, ethyl eosin, eosin Y, and combinations thereof.

2. The process according to claim 1 , wherein said stimulation agent is an energy source.

The process according to claim 1 , wherein the cartilage cap has a gap, bore, channel, or slot on a side opposite the superficial surface and a cartilage filler is inserted into the gap, bore, channel, or slot in said cartilage cap from the bottom of said osteochondral plug, wherein the superficial surface of the cartilage cap is closed.

4. The process according to claim 1 , wherein said first or second bonding agent comprises enzymes, bioactive growth supplements, natural polymers, synthetic polymers, photoactive agents, antioxidants, crosslinking agents, vital cells, or any combination thereof.

5. The process according to claim 1 , wherein said first or second bonding agent comprises a solution, gel, putty, tape, or sponge.

6. The process according to claim 1 , wherein the cartilage graft is cleaned, disinfected, devitalized, and stored.

7. The process according to claim 1 , wherein the cartilage graft is cleaned, disinfected, devitalized, recellularized in vitro, and stored.

8. The process according to claim 1 , wherein the cartilage graft is cleaned, disinfected, devitalized, stored, and recellularized, wherein said recellularization occurs in vitro, in situ, or in vivo.

9. The process according to claim 1 , wherein the cartilage graft is contoured to match the contour of a defect site.

10. The process according to claim 1 , wherein said osteochondral plug is inserted into said defect using an insertion device.

11. The process according to claim 1 , wherein the first bonding agent and the second bonding agent are crosslinked.

The process according to claim 1 , wherein the cartilage cap has a gap, bore, channel, or slot on a side opposite the superficial surface and a bone filler is inserted into the gap, bore, channel, or slot in said bone portion from the bottom of said osteochondral plug, wherein the superficial surface of the cartilage cap is closed.

13. A process for implanting a cartilage graft into a cartilage defect having a size, contour, and location and being surrounded by a cartilage surface, and sealing the implanted cartilage graft with recipient tissue, comprising

a. creating a cylindrical bore down to a bone portion of the cartilage defect;

b. selecting a cylindrical osteochondral plug that matches the size, contour, and location of the defect site, said osteochondral plug comprising a cartilage cap having a circumferential area and a superficial surface;

c. treating the bore at said defect site with a first bonding agent;

d. treating the circumferential area of said cartilage cap on said osteochondral plug with a second bonding agent; and

e. inserting said osteochondral plug into said defect site and wherein the superficial surface of said cartilage cap is surrounded by the cartilage surface surrounding the defect site,

wherein said first and second bonding agents are activated by applying a stimulation agent that induces sealing, integration, and hydrodynamic restoration of the recipient tissue, and

wherein said first and second bonding agent comprise a tethered diazopyruvate composition.

14. The process according to claim 13 , wherein said stimulation agent is an energy source.

The process according to claim 13 , wherein the cartilage cap has a gap, bore, channel, or slot on a side opposite the superficial surface and a cartilage filler is inserted into the gap, bore, channel, or slot in said cartilage cap from the bottom of said osteochondral plug, wherein the superficial surface of the cartilage cap is closed.

16. The process according to claim 13 , wherein said first or second bonding agent further comprises enzymes, bioactive growth supplements, natural polymers, synthetic polymers, photoactive agents, antioxidants, crosslinking agents, vital cells, or any combination thereof.

17. The process according to claim 13 , wherein said first or second bonding agent further comprises a solution, gel, putty, tape, or sponge.

18. The process according to claim 13 , wherein the cartilage graft is cleaned, disinfected, devitalized, and stored.

19. The process according to claim 13 , wherein the cartilage graft is cleaned, disinfected, devitalized, recellularized in vitro, and stored.

20. The process according to claim 13 , wherein the cartilage graft is cleaned, disinfected, devitalized, stored, and recellularized, wherein said recellularization occurs in vitro, in situ, or in vivo.

21. The process according to claim 13 , wherein the cartilage graft is contoured to match the contour of a defect site.

22. The process according to claim 13 , wherein said osteochondral plug is inserted into said defect using an insertion device.

23. The process according to claim 13 , wherein the first bonding agent and the second bonding agent are crosslinked.

24. The process according to claim 13 , wherein the cartilage cap has a gap, bore, channel, or slot on a side opposite the superficial surface and a bone filler is inserted into the gap, bore, channel, or slot in said bone portion from the bottom of said osteochondral plug, wherein the superficial surface of the cartilage cap is closed.
