Abstract

Methods for altering the natural history of degenerative disc disease and osteoarthritis of the spine are proposed. The methods focus on the prevention, or delayed onset or progression of, subchondral defects such as bone marrow edema or bone marrow lesion, and subchondral treatment to prevent the progression of osteoarthritis or degenerative disc disease in the spine and thereby treat pain.

Claims (19)

1. A method for treating osteoarthritis of the spine, comprising:

identifying a subchondral defect in a subchondral region of a bone of the spine based on a captured image, the subchondral defect being a bone marrow edema of a vertebral endplate;

determining, after identifying the presence of a subchondral defect, an extent of treatment based on a size or shape of said bone marrow edema;

creating, in said bone that contains the bone marrow edema in the subchondral region, a subchondral access path into an area in or adjacent to the bone marrow edema, while preserving an existing condition of the articular surface adjacent the subchondral region of said bone that contains the bone marrow edema;

treating the subchondral defect by:

implanting in the subchondral region, via the subchondral access path, an implantable device that mechanically supports the subchondral bone; or

injecting in the subchondral region, via the subchondral access path, a bone hardening material, wherein the bone hardening material stabilizes the subchondral defect;

wherein treatment of the subchondral region alleviates corresponding pain in the joint and slows the progression of osteoarthritis of the spine.

2. The method of claim 1 , wherein the captured image is a magnetic resonance image or x-ray.

3. The method of claim 1 , wherein the subchondral defect further includes sclerotic bone.

4. The method of claim 1 , wherein the subchondral defect further includes a fracture.

5. The method of claim 1 , further comprising stimulating a healing response to heal the defect.

6. The method of claim 5 , wherein stimulating a healing response comprises drilling.

7. The method of claim 5 , wherein stimulating a healing response comprises electrical or heat stimulation.

8. The method of claim 5 , wherein stimulating a healing response comprises biological or chemical stimulation.

9. The method of claim 1 , wherein the bone hardening material comprises a bone cement, bone void filler, or bone substitute material.

10. The method of claim 1 , wherein a sufficient volume of the bone hardening material is injected to envelope the subchondral defect.

11. The method of claim 1 , wherein the bone hardening material stimulates a healing response to heal the subchondral defect.

12. The method of claim 1 , further comprising compacting bone tissue to create a void in or adjacent to the subchondral defect.
