Abstract

Described here are systems and methods for accessing Schlemm's canal and for delivering an ocular device or fluid composition therein. The ocular devices may maintain the patency of Schlemm's canal without substantially interfering with transmural fluid flow across the canal. The fluid composition may be a viscoelastic fluid that is delivered into the canal to facilitate drainage of aqueous humor by disrupting the canal and surrounding trabeculocanalicular tissues. Tools for disrupting these tissues and minimally invasive methods for treating medical conditions associated with elevated intraocular pressure, including glaucoma, are also described.

Claims (17)

1. An implant-free, ab-interno method for treating conditions of the eye comprising:

advancing a cannula at least partially through the anterior chamber of the eye and into Schlemm's canal; advancing a slidable cutting tool comprising a flexible polymer from the cannula into and along a length of the canal, the slidable cutting tool having an outer diameter between about 100 and about 500 microns; and disrupting the trabeculocanalicular tissues using the slidable cutting tool, and not an implant, to reduce intraocular pressure.

2. The implant-free method of claim 1 , wherein the slidable cutting tool is advanced in both clockwise and counterclockwise directions along a length of Schlemm's canal.

3. The implant-free method of claim 1 , wherein the slidable cutting tool is advanced about a 360 degree arc of Schlemm's canal.

4. The implant-free method of claim 1 , wherein the slidable cutting tool is advanced about a 180 degree arc of Schlemm's canal.

5. The implant-free method of claim 1 , wherein the slidable cutting tool is advanced about a 90 degree arc of Schlemm's canal.

6. The implant-free method of claim 1 , wherein the slidable cutting tool is advanced from a single access point in Schlemm's canal.

7. The implant-free method of claim 2 , wherein the slidable cutting tool is advanced from a single access point in Schlemm's canal.

8. The implant-free method of claim 1 , wherein the slidable cutting tool is advanced from a plurality of access points in Schlemm's canal.

9. The implant-free method of claim 2 , wherein the slidable cutting tool is advanced from a plurality of access points in Schlemm's canal.

10. The implant-free method of claim 1 , wherein advancement of the cutting tool is accomplished by a wheeled drive assembly.

11. The implant-free method of claim 10 , wherein the wheeled drive assembly comprises a single rotatable component.

12. The implant-free method of claim 10 , wherein the wheeled drive assembly comprises a plurality of rotatable components.

13. The implant-free method of claim 1 , wherein the slidable tool has an outer diameter of about 200 microns.

14. The implant-free method of claim 1 , wherein the trabeculocanalicular tissues comprise at least a portion of the trabecular meshwork or juxtacanalicular tissue.

15. The implant-free method of claim 1 , wherein the trabeculocanalicular tissues comprise collector channels.

16. The implant-free method of claim 1 , wherein the eye conditions are glaucoma, pre-glaucoma, or ocular hypertension.
