Methods and apparatus for performing spine surgery
View Patent ↗Systems and methods are described for correcting sagittal imbalance in a spine including instruments for performing the controlled release of the anterior longitudinal ligament through a lateral access corridor and hyper-lordotic lateral implants.
1. A spinal implant positionable within an interbody space between a first spinal vertebra and a second spinal vertebra from a lateral access pathway after cutting the Anterior Longitudinal Ligament (ALL), said implant comprising:
an upper surface including anti-migration elements to contact the first vertebra when the implant is positioned within the interbody space, a lower surface including anti-migration elements to contact the second vertebra when the implant is positioned within the interbody space, a distal wall, a proximal wall, an anterior sidewall that faces anteriorly when the implant is positioned within the interbody space, and a posterior sidewall that faces posteriorly when the implant is positioned within the interbody space;
wherein said implant has a maximum longitudinal length extending parallel to a straight longitudinal axis from a proximal end of the proximal wall to a distal end of the distal wall, a width extending from the anterior end of the anterior sidewall to the posterior end of the posterior sidewall, the maximum longitudinal length being at least 40 mm, an anterior height extending from the upper surface to the lower surface at the anterior sidewall and a posterior height extending from the upper surface to the lower surface at the posterior sidewall, the anterior height being greater than the posterior height such that the upper and lower surfaces increase in slope from the posterior sidewall to the anterior sidewall forming an angle ranging from 20 degrees to 40 degrees;
at least a first engagement feature configured to releasably couple an insertion instrument to the implant adjacent the proximal wall; and
at least a first fusion aperture extending through the upper surface and lower surface and configured to permit bone growth between the first vertebra and the second vertebra when the implant is positioned within the interbody space, the first fusion aperture having: a longitudinal aperture length extending generally parallel to the longitudinal length of the implant, and an aperture width extending from the anterior sidewall to the posterior sidewall, wherein the longitudinal aperture length is greater than the aperture width.
2. The spinal implant of claim 1 , wherein said distal wall, proximal wall, anterior sidewall, and posterior sidewall comprise radiolucent material.
3. The spinal implant of claim 2 , wherein the radiolucent material is PEEK.
4. The spinal implant of claim 1 , further including a second fusion aperture extending through said upper surface and lower surface and configured to permit bone growth between the first vertebra and the second vertebra when the implant is positioned within the interbody space.

The spinal implant of claim 4 , wherein the second fusion aperture has a longitudinal aperture length extending generally parallel to the maximum longitudinal length of the implant, and an aperture width extending from the anterior sidewall to the posterior sidewall, wherein the longitudinal aperture length is greater than the aperture width.
6. The spinal implant of claim 1 , wherein said anti-migration elements on said upper surface comprise a plurality of ridges.
7. The spinal implant of claim 6 , wherein said plurality of ridges extend perpendicularly to said longitudinal length.
8. The spinal implant of claim 1 , wherein said anti-migration elements on said upper surface comprise spike elements.
9. The spinal implant of claim 1 , further comprising an osteoinductive material positioned within said first aperture.
10. The spinal implant of claim 1 , further including at least one visualization aperture extending through the anterior sidewall.
11. The spinal implant of claim 1 , wherein the upper surface and lower surface are generally planar.
12. The spinal implant of claim 1 , wherein the upper and lower surface are convex.
13. The spinal implant of claim 1 , wherein the first engagement feature is a threaded receiving aperture situated in the proximal wall.
14. The spinal implant of claim 13 , wherein the first engagement feature further includes a pair of grooves situated in the proximal end and extending from the threaded receiving aperture.
15. The spinal implant of claim 1 , wherein the proximal wall includes a fixation aperture extending therethrough and configured to receive a fixation anchor to fix the implant to one of the first vertebra and second vertebra when the implant is positioned within the interbody space.
16. The spinal implant of claim 15 , wherein the fixation aperture includes a locking mechanism.
17. The spinal implant of claim 1 , wherein the proximal wall includes a tab extending above the upper surface and configured to engage an exterior surface of the first vertebra when the implant is positioned within the interbody space, the tab including a fixation aperture extending therethrough and configured to receive a fixation anchor to fix the implant to the first vertebra when the implant is positioned within the interbody space.
18. The spinal implant of claim 17 , wherein the tab also extends below the lower surface and is configured to engage an exterior surface of the second vertebra when the implant is positioned within the interbody space.
19. The spinal implant of claim 18 , wherein the tab includes two fixation apertures.
