Abstract

Devices and methods for altering gaseous flow within a lung to improve the expiration cycle of an individual, particularly individuals having chronic obstructive pulmonary disease. The methods and devices create channels in lung tissue and maintain the patency of these surgically created channels in tissue. Maintaining the patency of the channels allows air to pass directly out of the lung tissue which facilitates the exchange of oxygen ultimately into the blood and/or decompresses hyper-inflated lungs.

Claims (36)

1. A hole-making catheter for creating and dilating an opening within tissue, the catheter comprising:

an elongate shaft having a proximal portion and a distal portion, and at least one lumen extending through the proximal end, said shaft having a flexibility and diameter to slidably fit within a working channel of a bronchoscope;

a nondistensible balloon having an interior in fluid communication with the lumen, the nondistensible balloon located on the distal portion of the elongate shaft;

a piercing member located at the distal portion of the elongate shaft; the piercing member being extendable and retractable within the elongate shaft; and

a depth limiting feature wherein the depth limiting feature cooperates with the elongate shaft to halt distal movement of the distal portion of the elongate shaft beyond a fixed distance.

2. The catheter of claim 1 , wherein said piercing member and balloon are an integrated assembly and said piercing member is cooperatively extendable and retractable through said balloon.

3. The catheter of claim 2 , wherein the balloon catheter comprises an opening locating feature.

4. The catheter of claim 3 , wherein the opening locating feature is a guide member and comprises a resistance surface that increases resistance to removal of the guide member from the opening to temporarily anchor the balloon catheter in the opening.

5. The catheter of claim 4 , wherein the guide member comprises a shape selected from the shapes consisting of a tapered shape, rounded shape, partially-spherical shape, elliptical shape, prolate shape, cone-shape, and triangular shape.

6. The catheter of claim 4 , wherein the guide member includes more than one resistance surface.

7. The catheter of claim 4 , wherein the piercing member is advanceable through the guide member.

8. The catheter of claim 1 , wherein the balloon has a preformed generally uniform expanded shape.

9. The catheter of claim 1 , wherein the depth limiting feature is a circular ring on the elongate shaft.

10. A method for treating a lung, the method comprising:

inserting a bronchoscope into an airway of the lung;

selecting a site along the airway to create a hole;

advancing a hole-making catheter through a working channel of the bronchoscope and to the site, the hole-making catheter comprising a nondistensible balloon and a depth limiting member adjacent to the nondistensible balloon;

creating a hole in the airway wall by piercing the airway wall at the site;

advancing the nondistensible balloon into the hole until a depth limiting member contacts a tissue wall to prevent further advancement of the hole-making catheter which limits a penetration depth of the nondistensible balloon into the tissue;

enlarging the hole by expanding the nondistensible balloon; and

advancing a tubular member through the hole.

11. The method of claim 10 , wherein selecting the site comprises visually selecting the site using a bronchoscope.

12. The method of claim 10 , wherein selecting the site comprises determining the presence or absence of a blood vessel adjacent to the site to avoid the blood vessel.

13. The method of claim 10 , further comprising delivering a bio-active agent through the site.

14. The method of claim 10 , further comprising locating the hole.

15. The method of claim 14 , wherein the locating is performed using a guide body.

16. The method of claim 15 , wherein the guide body comprises a partially-spherical shape.

17. The method of claim 15 , wherein the piercing member is advanceable through the guide body.

18. The method of claim 10 , further comprising observing a bronchoscopic visual feature on the tubular member, and using the feature to aid in advancing the tubular member.

19. The method of claim 10 , wherein enlarging comprises dilating with a balloon.

20. A system for treating a lung, the system comprising:

a bronchoscope having, a working channel;

a hole-making catheter slidably disposed within the working channel of the bronchoscope, said hole-making catheter comprising: an elongate shaft having a proximal portion and a distal portion, and at least one lumen extending through the proximal end; a nondistensible balloon having an interior in fluid communication with the lumen, the nondistensible balloon located on the distal portion of the elongate shaft;

a piercing member located at the distal portion of the elongate shaft; the piercing member being extendable and retractable within the elongate shaft;

a depth limiting feature adapted to limit movement of the hole-making catheter a fixed distance through a tissue wall; and

an obturator insertable within the hole-making catheter.
