Abstract

Systems and methods for crossing stenosis, partial occlusions, or complete occlusions within a body lumen. The systems generally include an elongate member such as a hollow guidewire that houses a rotatable and translatable drive shaft. The drive shaft typically has a distal portion that is advanced to create a path in the occlusive material that is large enough to allow the hollow guidewire to cross the occlusive material.

Claims (27)

1. A method for crossing through an anatomical structure, the method comprising:

advancing an elongate member intravascularly to a location proximate an anatomical structure, the elongate member having a proximal end removably coupled to a housing and a drive shaft rotatably disposed within the elongate member extending from a motor positioned in the housing to a distal tip rotatably disposed at a distal end of the elongate member;

rotating the drive shaft within the elongate member with the motor to thereby rotate the distal tip;

forming an opening through the anatomical structure with the distal tip while rotating the drive shaft within the elongate member with the motor;

decoupling the housing from the elongate member while retaining a distal portion of the elongate member proximate the opening; and

advancing an access device over the elongate member to the anatomical structure after decoupling the housing from the elongate member.

2. The method of claim 1 , wherein the elongate member serves as a guidewire to guide the access device through the opening without the use of a separate guidewire.

3. The method of claim 1 , wherein decoupling the housing from the elongate member includes unlocking a luer assembly so as to release the elongate member from the housing.

4. The method of claim 1 , further comprising:

detaching the motor from the drive shaft.

5. The method of claim 1 , wherein the motor and drive shaft are coupled together with a detachable linkage assembly.

6. The method of claim 1 , wherein the elongate member has an outer diameter of between approximately 0.009 inches and approximately 0.040 inches.

7. The method of claim 1 , wherein the distal tip creates an opening having a radius at least as large as a radius of the distal end of the elongate member.

8. The method of claim 1 , wherein the elongate member includes a coil proximate the distal end of the elongate member.

9. The method of claim 1 , wherein the access device is an intravascular catheter.

10. A method for crossing through an anatomical structure, the method comprising:

advancing a hollow guidewire intravascularly to a location proximate an anatomical structure, the guidewire having a proximal end removably coupled to a housing and a drive shaft extending through the hollow guidewire, a proximal end of the drive shaft being coupled to a motor shaft of a motor positioned in the housing and a distal end of the drive shaft being coupled to a distal tip rotatably disposed at a distal end of the guidewire;

rotating the drive shaft within the guidewire with the motor to form an opening through the anatomical structure with the distal tip;

decoupling the guidewire and the drive shaft from the housing and the motor shaft, respectively, while retaining the guidewire proximate opening through the anatomical structure; and

advancing an access device over the guidewire to the opening of the anatomical structure after decoupling the guidewire and the drive shaft from the housing and the motor shaft.

11. The method of claim 10 , wherein the guidewire serves to guide the access device to the opening through the anatomical structure without the use of a separate guidewire.

12. The method of claim 10 , wherein the guidewire has an outer diameter of between approximately 0.009 inches and approximately 0.040 inches.

13. The method of claim 10 , wherein decoupling the housing from the guidewire includes unlocking a luer assembly so as to release the guidewire from the housing.

14. The method of claim 10 , wherein the motor shaft and drive shaft are coupled together with a detachable linkage assembly.

15. The method of claim 10 , wherein the distal tip creates an opening having a radius at least as large as a radius of the distal end of the guidewire.

16. The method of claim 10 , wherein the guidewire includes a coil proximate the distal end of the guidewire.

17. The method of claim 10 , wherein the access device is an intravascular catheter.
