Abstract

The present invention provides for devices and methods for providing endovascular therapy, including facilitating establishment of vascular access, placement of endovascular sheaths, catheter tip localization, and administration of vascular occlusion. The invention includes a vessel cannulation device, an expandable sheath, an occlusion catheter, and a localizer each of which may be provided separately or used as part of a system. One embodiment of the invention is a vessel cannulation device including: a housing having a distal end with a distal tip and a proximal end; a guidewire lumen passing through the housing and at least the distal tip; a sensor coupled to the guidewire lumen; and an advancing member, which is configured for advancing at least one of a guidewire or a sheath and which is operably coupled to the sensor.

Claims (42)

1. A vessel cannulation device comprising:

a housing having a distal end with a distal tip and a proximal end, the distal tip of the housing being configured to penetrate a vessel;

a lumen passing through at least the distal tip;

a sensor coupled to the lumen, the sensor being configured to sense a physiologic parameter; and

a blunting device advancing member configured for advancing a blunting device, wherein the blunting device is operably coupled to the sensor, and

wherein the blunting device advancing member is configured to advance the blunting device in response to the sensor sensing the physiologic parameter within a predetermined range,

wherein a response time is a period of time between when the distal tip penetrates the vessel and when the blunting device advancing member finishes advancing,

wherein the device is configured to minimize the response time to be in a predetermined range of time, and

wherein the predetermined range of the physiologic parameter correlates to a particular location.

2. The vessel cannulation device of claim 1 , wherein the sensor is a membrane.

3. The vessel cannulation device of claim 2 , wherein the membrane is a pressure operable diaphragm.

4. The vessel cannulation device of claim 2 , wherein the membrane is elastic.

5. The vessel cannulation device of claim 1 , further comprising a fluid passageway coupled to the sensor.

6. The vessel cannulation device of claim 5 , wherein the fluid passageway is at least partially pre-filled with a biologically acceptable fluid.

7. The vessel cannulation device of claim 5 , wherein fluid entering the fluid passageway increases pressure in the fluid passageway thereby causing movement of the sensor which in turn engages the blunting device advancing member and thereby advances the blunting device.

8. The vessel cannulation device of claim 1 , wherein the sensor is in direct contact with fluid.

9. The vessel cannulation device of claim 1 , wherein the response time is between 0.02-1 seconds.

10. The vessel cannulation device of claim 1 , wherein the response time is between 0.05-0.5 seconds.

11. The vessel cannulation device of claim 1 , wherein the blunting device advances automatically in response to the sensor detecting the physiologic parameter.

12. The vessel cannulation device of claim 1 , wherein the predetermined range of the physiologic parameter is above 20 mm Hg.

13. The vessel cannulation device of claim 1 , wherein the predetermined range of the physiologic parameter is about 5 mm Hg to about 20 mm Hg.

14. The vessel cannulation device of claim 1 , wherein the physiologic parameter is blood pressure.

15. The vessel cannulation device of claim 1 , wherein the sensor is not dependent on creation of negative pressure within the device.

16. The vessel cannulation device of claim 1 , wherein the particular location is one of a vein and an artery.

17. The vessel cannulation device of claim 1 , wherein the predetermined range of the physiologic parameter is a controllable predetermined range.

18. The vessel cannulation device of claim 1 , wherein the sensor is one of a pressure sensor, a conductivity sensor, a flow sensor, an ultrasonic sensor, a photoelectric sensor, and a resistance sensor.

19. A method for providing a vessel cannulation, the method comprising the steps of:

placing a vessel cannulation device having a housing having a distal end with a distal tip and a proximal end in close proximity of a vessel, the distal tip of the housing being configured to penetrate the vessel and the vessel cannulation device configured to receive fluid;

advancing the vessel cannulation device until the distal tip of the housing of the vessel cannulation device pierces the vessel allowing the fluid to pass through a lumen in at least the distal tip;

sensing a physiologic parameter by a sensor coupled to the lumen; and

advancing a blunting device with a blunting device advancing member in response to the sensor sensing the physiologic parameter within a predetermined range, wherein the blunting device is operably coupled to the sensor;

wherein a response time is a period of time between when the distal tip penetrates the vessel and when the blunting device advancing member finishes advancing,

wherein the device is configured to minimize the response time to be in a predetermined range of time, and

wherein the predetermined range of the physiologic parameter correlates to a particular location.

20. The method of claim 19 , wherein the sensor is in direct contact with the fluid.

21. The method of claim 19 , wherein the sensor is coupled to a fluid passageway.

22. The method of claim 21 , wherein the fluid passageway is partially pre-filled with a biologically acceptable fluid.

23. The method of claim 19 , wherein the vessel is an artery and the physiologic parameter is pressure and wherein the sensor senses when the predetermined range is above about 20 mm Hg.

24. The method of claim 19 , wherein the vessel is a vein and the physiologic parameter is pressure and wherein the sensor senses when the predetermined range is between about 5 mm Hg to about 20 mm Hg.

25. The method of claim 19 , wherein the response time is between 0.02-1 seconds.

26. The method of claim 19 , wherein the response time is between 0.05-0.5 seconds.

27. The method of claim 19 , wherein the sensor is not dependent on creation of negative pressure within the device.
