Abstract

Devices and methods for altering the spacing and motion at the facet joints of the vertebral column are provided. One embodiment of the invention comprises a prosthesis with surfaces configured to articulate with the facets of the facet joint. A retaining member for anchoring the prosthesis within the facet joint is optionally included. Methods for surgically and less invasively implanting the prosthesis and securing the prosthesis to the articular processes or surrounding soft tissue are also provided.

Claims (28)

1. A method of treating facet joint dysfunction, comprising:

accessing a facet joint comprising a first articular process and a second articular process;

preparing an articular surface of at least one of the first and second articular processes;

drilling a hole through both the first articular process and second articular process;

inserting an elongate retaining member comprising a first end and a second end through the hole in the first articular process and the hole in the second articular process;

securing the elongate retaining member to the articular processes,

wherein securing the elongate retaining member comprises placing a first retaining ring on the first end of the elongate retaining member.

2. The method of claim 1 , wherein the elongate retaining member is a solid, braided, or multi-filamented wire or cable.

3. The method of claim 2 , wherein the wire or cable comprises a cable of braided polymer, including but not limited to a braided polymer such as PEEK or PEKK, or a braided metal, such as braided cobalt chromium or titanium.

4. The method of claim 2 , wherein the wire or cable has a diameter of about 0.5 mm to about 2 mm.

5. The method of claim 2 , wherein the wire or cable has a length of about 10 mm to about 40 mm.

6. The method of claim 1 , wherein the elongate retaining member is a single strand or multistrand polymeric string or weave, polymeric meshes or fabric.

The method of claim 1 , wherein securing the elongate retaining member comprises placing the first end of the elongate retaining member through the first retaining ring and creating a friction fit or mechanical interfit to resist movement between the first end of the elongate retaining member and the first retaining ring.

8. The method of claim 1 , wherein preparing an articular surface comprises roughening the surface of the articular surface.

9. The method of claim 1 , wherein securing the elongate retaining member comprises placing a second retaining ring on the second end of the elongate retaining member.

10. The method of claim 1 , wherein securing the elongate retaining member comprises creating a friction fit between the first retaining ring and the first end of the elongate retaining member.

11. The method of claim 10 , wherein securing the elongate retaining member comprises creating a friction fit between the second retaining ring and the second end of the elongate retaining member.

12. A method of treating facet joint dysfunction, comprising:

accessing a facet joint comprising a first articular process and a second articular process;

preparing an articular surface of at least one of the first and second articular processes;

drilling a hole through both the first articular process and second articular process;

inserting an elongate retaining member comprising a first end and a second end through the hole in the first articular process and the hole in the second articular process;

securing the elongate retaining member to the articular processes; and

wherein securing the elongate retaining member comprises placing at least one retaining ring with radially inward biased projections defining a central lumen over the elongate retaining member creating a friction fit.

The method of claim 1 , wherein the elongate retaining member is secured to the articular processes with sufficient laxity or length between the first end and the second end so that the articular processes are not fixed in position relative to each other and remain capable of performing movements.

14. The method of claim 13 , wherein securing the elongate retaining member comprises placing the second end of the elongate retaining member through a second retaining ring.

15. The method of claim 14 , wherein securing the elongate retaining member comprises creating a friction fit between the first retaining ring and the first end of the elongate retaining member.

16. The method of claim 15 , wherein securing the elongate retaining member comprises creating a friction fit between the second retaining ring and the second end of the elongate retaining member.
