Abstract

Methods and devices are disclosed for treating the vertebral column. An integrated fixation plate and spacer having a retaining structure within the screw holes of the fixation plate to resist backout of screws attaching the fixation plate to the bone is provided. A movable joint may be provided between the fixation plate and spacer. In some embodiments, a screw hole insert is also provided to resist shear forces acting between the screw and fixation plate. In some embodiments, an integrated fixation plate and spacer system is provided, comprising two or more integrated fixation plate and spacer implants, wherein the fixation plates of each implant has a complementary configuration to allow attachment of the implants at adjacent intervertebral spaces. Alternative fixation systems are also contemplated.

Claims (35)

1. A method for treating a spine, comprising:

providing a first stabilization plate, wherein the first stabilization plate comprises an upper portion, a lower portion, an access surface, a bone facing surface, and the upper portion comprises a first hole between the access surface and the bone facing surface of the first stabilization plate, wherein the first hole comprises a retaining channel,

providing a retaining structure, wherein the retaining structure comprises a retaining ring at least partially located within the retaining channel,

providing an attachment insert, and wherein the attachment insert comprises a hole extending through the attachment insert, the attachment insert having an inner insert diameter and an outer perimeter having an outer insert diameter, wherein at least a part of the outer insert diameter is adjacent to the first hole, and wherein the retaining ring is integral with the attachment insert;

attaching the upper portion of the first stabilization plate to a first vertebra; and

attaching the lower portion of the first stabilization plate to a second vertebra.

2. The method for treating a spine as in claim 1 , wherein the attachment insert comprises a concave surface.

The method for treating a spine as in claim 1 , wherein attaching the upper portion of the first stabilization plate to the first vertebra comprises inserting a bone fastener through the attachment insert located in the upper portion of the first stabilization plate and engaging the first vertebra with the bone fastener.

4. The method for treating a spine as in claim 1 , further comprising attaching an upper portion of a second stabilization plate to the second vertebra, wherein the second stabilization plate comprises the upper portion and a lower portion.

5. The method for treating a spine as in claim 4 , wherein the lower portion of the first stabilization plate has a complementary configuration to the upper portion of the second stabilization plate.

6. The method for treating a spine as in claim 1 , further comprising inserting a first spacer into a vertebral space between the first vertebra and the second vertebra.

7. The method for treating a spine as in claim 6 , further comprising inserting a second spacer into a vertebral space between the second vertebra and a third vertebra.

8. The method for treating a spine as in claim 3 , wherein inserting the bone fastener comprises inserting the bone fastener through the retaining ring toward the bone facing surface of the first stabilization plate.

9. The method for treating a spine as in claim 8 , wherein the bone fastener is adjacent to the attachment insert.

10. A method for treating a spine, comprising:

providing a stabilization plate comprising an access surface and a bone facing surface;

providing a first attachment lumen between the access surface and the bone facing surface of the stabilization plate, the first attachment lumen having a first attachment diameter adjacent to the access surface of the stabilization plate, a lumen surface, and a second attachment diameter adjacent to the bone facing surface of the stabilization plate, a retaining channel along the lumen surface, and a third attachment diameter about the retaining channel, wherein the third attachment diameter is greater than the first attachment diameter and the second attachment diameter;

providing a retaining ring at least partially located within the retaining channel; and

providing a first attachment lumen insert coupled to the lumen surface, the first attachment lumen insert comprising a hole extending through the first attachment lumen insert, the first attachment lumen insert having an inner insert diameter and an outer perimeter having an outer insert diameter, wherein a proximal surface of the first attachment lumen insert is located distal to a distal edge of the retaining channel toward the bone facing surface side of the stabilization plate; and

attaching the stabilization plate to a first vertebra.

11. The method for treating a spine as in claim 10 , wherein the retaining ring is integral with the first attachment lumen insert.

12. The method for treating a spine as in claim 11 , wherein the first attachment lumen insert is located within the first attachment lumen.

13. The method for treating a spine as in claim 10 , further comprising providing a bone screw, the bone screw having a screw head and a screw body.

14. The method for treating a spine as in claim 13 , wherein attaching the stabilization plate to a first vertebra comprises inserting the bone screw into the retaining ring and into the first vertebra.

15. The method for treating a spine as in claim 13 , wherein the first attachment lumen insert is located within the first attachment lumen.

16. The method for treating a spine as in in claim 15 , wherein attaching the stabilization plate to a first vertebra comprises inserting the bone screw into the first attachment lumen insert and into the first vertebra.

17. The method for treating a spine as in claim 10 , further comprising a second attachment lumen between the access surface and the bone facing surface of the stabilization plate, the second attachment lumen having a fourth attachment diameter adjacent to the access surface of the stabilization plate, a lumen surface, and a fifth attachment diameter adjacent to the bone facing surface of the stabilization plate, a retaining channel along the lumen surface, and a sixth attachment diameter about the retaining channel, wherein the sixth attachment diameter is greater than the fourth attachment diameter and the fifth attachment diameter.

18. The method for treating a spine as in claim 10 , wherein the stabilization plate and spacer comprise a polyaryl polymer, PEK, PEEK, PEKK, PEKEKK, or blend thereof and the first attachment lumen insert comprises a metal or ceramic.

19. A method for treating a spine, comprising:

providing a stabilization plate comprising an access surface and a bone facing surface;

providing a first attachment lumen between the access surface and the bone facing surface of the stabilization plate, the first attachment lumen having a first attachment diameter adjacent to the access surface of the stabilization plate, a lumen surface, and a second attachment diameter adjacent to the bone facing surface of the stabilization plate, a retaining channel along the lumen surface, and a third attachment diameter about the retaining channel, wherein the third attachment diameter is greater than the first attachment diameter and the second attachment diameter;

providing a retaining ring at least partially located within the retaining channel; and

providing a first attachment lumen insert located at least partially in the first attachment lumen, the first attachment lumen insert comprising a hole extending through the first attachment lumen insert, the first attachment lumen insert having an inner insert diameter and an outer perimeter having an outer insert diameter, wherein a proximal surface of the first attachment lumen insert is located distal to a distal edge of the retaining channel toward the bone facing surface side of the stabilization plate; and

attaching the stabilization plate to a first vertebra.

20. The method for treating a spine as in claim 19 , wherein the retaining ring is integral with the first attachment lumen insert.
