Abstract

A spinal stabilization system may be formed in a patient. In some embodiments, a minimally invasive procedure may be used to form a spinal stabilization system in a patient Bone fastener assemblies may be coupled to vertebrae Each bone fastener assembly may include a bone fastener and a collar. The collar may be rotated and/or angulated relative to the bone fastener. Detachable members may be coupled to the collar to allow for formation of the spinal stabilization system through a small skin incision. The detachable members may allow for alignment of the collars to facilitate insertion of an elongated member in the collars. An elongated member may be positioned in the collars and a closure member may be used to secure the elongated member to the collars.

Claims (24)

1. A medical device comprising:

a delivery device for delivering a spinal rod to a bone anchor comprising:

an inner tube having a proximal end, a distal end, and a lumen extending between the proximal end and the distal end, the distal end of the inner tube being adapted to releasably engage a bone anchor;

an outer tube disposed about at least a portion of the inner tube, the outer tube having a proximal end, a distal end, and a lumen extending between the proximal end and the distal end, the outer tube being sized to span from at least a skin incision in a patient to a predetermined site proximate the spine of the patient, the distal end of the outer tube being adapted to releasably engage the bone anchor;

a bone anchor comprising a bone screw having a proximal head and a distal bone engaging portion, and a collar having a proximal end having a recess for receiving a spinal rod, the collar having threading, the proximal head of the bone screw positioned in the collar and the distal bone engaging portion extending from the collar, wherein the distal ends of the inner and outer tubes are releasably engaged to the collar; and

wherein the lumen of the inner tube and the lumen of the outer tube are sized to pass a threaded closure mechanism configured to threadingly engage the bone anchor therethrough, and wherein the delivery device is configured such that after the delivery device is engaged to the bone anchor, the closure mechanism may be inserted through the inner tube and threaded onto the bone anchor collar.

2. The device of claim 1 , further comprising a connection element capable of connecting the inner tube to the outer tube, wherein when the inner and outer tubes are connected to each other and connected to the bone anchor, the distal end of the inner tube contacts the bone anchor.

3. The device of claim 2 , wherein the connection element includes external threading on the inner tube and internal threading on the outer tube.

4. The device of claim 1 , wherein the distal end of the outer tube includes a pair of opposed tabs, each tab including an angled surface to facilitate positioning of the tabs about the collar.

5. The device of claim 4 , wherein each tab includes a radially inward projection for engaging a connection feature on the collar.

6. The device of claim 1 , wherein the distal end of the inner tube is configured to inhibit rotation of the delivery device relative to the collar.

7. The device of claim 6 , wherein the distal end of the inner tube is roughened or textured to frictionally engage a proximal surface of the collar.

8. A medical device comprising:

a delivery device for a spinal rod to a bone anchor comprising

an inner tube having a proximal end, a distal end, and a lumen extending between the proximal end and the distal end, the inner tube having a first slot formed therein, the first slot having an opening at a distal end of the inner tube and extending a first slot length toward the proximal end of the inner tube;

an outer tube disposed about at least a portion of the inner tube, the outer tube having a proximal end, a distal end, and a lumen extending between the proximal end and the distal end, the outer tube being sized to span from at least a skin incision in a patient to a predetermined site proximate the spine of the patient, the distal end of the outer tube being adapted to releasably engage a bone anchor, the outer tube having a second slot formed therein aligned with the first slot, the second slot having an opening at the distal end of the outer tube and extending a second slot length toward the proximal end of the outer tube, the first slot length and the second slot length sized to span at least from a skin incision to the distal ends of the inner tube and the outer tube, respectively, such that the first slot and the second slot may be accessible from outside of the patient when the device is in use in surgery, and

a connection element capable of connecting the inner tube to the outer tube, wherein when the inner and outer tubes are connected to each other and connected to the bone anchor, the distal end of the inner tube contacts the bone anchor; and

a bone anchor assembly comprising

a bone anchor having a proximal head and a distal bone engaging portion; and

a collar coupled to the bone anchor, the collar having a proximal end, a distal end and a recess for receiving a spinal rod, the collar having threading, the proximal end of the collar having a pair of arcuate grooves formed on an exterior surface thereof, wherein the distal end of the outer tube includes a pair of opposed tabs, each tab having a radially inward projection that is sized and shaped to seat within one of the arcuate grooves formed in the collar of a bone anchor, wherein the lumen of the inner tube and the lumen of the outer tube are sized to pass a threaded closure mechanism configured to threadingly engage the bone anchor assembly therethrough, and wherein the delivery device is configured such that after the delivery device is engaged to the bone anchor, the closure mechanism may be inserted through the inner tube and threaded onto the bone anchor collar.

9. The device of claim 8 , wherein the connection element includes external threading on the inner tube and internal threading on the outer tube.

10. The device of claim 8 , wherein the inner tube is removable and replaceable from the outer tube to facilitate cleaning of the inner tube.

11. The device of claim 8 , wherein the distal end of the inner tube is configured to inhibit rotation of the device for delivery to the collar.

12. The device of claim 11 , wherein the distal end of the inner tube is roughened or textured to frictionally engage a proximal surface of the collar.
