Abstract

A dermatological skin treatment device is provided. The device comprises a handpiece and a cutting tool, wherein the tool is inserted through the conduit and percutaneously inserted into a tissue disposed within a recessed area of the handpiece. The device and method cut the fibrous structures under the skin that cause cellulite at an angle substantially parallel to the surface of the skin and replace these structures with a non-cellulite forming structure by deploying a highly fibrous mesh through a single needle hole to create a highly fibrous layer directly or through wound healing processes.

Claims (42)

1. A method of dissecting tissue below a surface of a dermis of a patient, comprising:

engaging a first portion of the dermis with a handpiece;

applying a force to the dermis to move a target portion of tissue into a treatment region of the handpiece so that the dermis and the target portion of tissue is elevated relative to surrounding tissue;

inserting a tool through an aperture of a wall of the handpiece, through the dermis and into the treatment region so that a distal end of the tool penetrates the target portion of tissue;

coupling the tool to a guidance track;

coupling the guidance track to the handpiece; and

moving the tool along the guidance track in forward, backward and side-to-side motions so that the tool pivots about the aperture and the distal end of the tool is moved within a region of predetermined shape within the target portion of tissue;

wherein the tool creates a first dissection in the predetermined shape at a first depth below the surface of the dermis.

2. The method of claim 1 , wherein the first dissection releases dermal tissue from subdermal tissue to reduce surface irregularities of the dermis.

3. The method of claim 1 , wherein the distal end of the tool cuts tissue in the target portion of tissue at the first depth.

4. The method of claim 3 , wherein the tool cuts the tissue with a blade.

5. The method of claim 4 , further comprising reciprocating the blade to cut the tissue.

6. The method of claim 1 , further comprising, after the first dissection is created, adjusting the handpiece such that the tool creates a second dissection at a second death below the surface of the dermis.

7. The method of claim 6 , wherein the handpiece has an apposition surface facing the treatment region for engaging the dermis, and adjusting the handpiece comprises changing the height of the apposition surface relative to the surrounding tissue.

8. The method of claim 1 , wherein the distal end of the tool moves along a first surface in the target region having the predetermined shape.

9. The method of claim 8 , wherein the first surface is substantially planar.

10. The method of claim 1 , wherein the guidance track comprises a plurality of channels.

11. The method of claim 10 , wherein the channels include a first channel extending toward the treatment region and a plurality of transverse channels extending transverse to the first channel.

12. The method of claim 11 , wherein at least a portion of the transverse channels are arc-shaped.

13. The method of claim 1 , further comprising coupling the tool to a second guidance track, and moving the tool along the second guidance track to move the distal end within a second region of a second predetermined shape in the target portion of tissue.

14. The method of claim 1 , further comprising coupling an injection device to a second guidance track, wherein the injection device is constrained to move within a predetermined area such that a needle on the injection device moves in a predetermined trajectory in the target portion of tissue.

15. The method of claim 1 , wherein applying a force comprises applying a vacuum within the treatment region to elevate the target portion of tissue.

16. A method of dissecting tissue below a surface of a dermis of a patient, comprising:

engaging dermal tissue with a handpiece;

elevating a target portion of dermal tissue within a treatment region of the handpiece;

penetrating an aperture of a wall of the handpiece and the dermal tissue to access the target portion of dermal tissue with a distal end of a tool;

coupling the tool to a guidance track which constrains the tool to move within a predetermined area relative to the handpiece; and

manipulating the tool within the predetermined area such that the tool moves forward, backward and side-to side while pivoting about the aperture and the distal end of the tool dissects the tissue in a predetermined shape at a predetermined depth below the surface of the dermis.

17. The method of claim 16 , wherein dissecting the tissue releases dermal tissue from subdermal tissue to reduce surface irregularities of the dermis.

18. The method of claim 17 , wherein reducing the surface irregularities improves the appearance of gynoid lypodystrophy.

19. The method of claim 17 , wherein the surface irregularities are on the legs or buttocks.

20. The method of claim 17 , wherein the surface irregularities are on the face.

21. The method of claim 17 , wherein the surface irregularities are formed by acne scars.

22. The method of claim 16 , wherein dissecting the tissue creates a subdermal pocket for receiving an implant.

23. The method of claim 16 , wherein dissecting the tissue damages eccrine glands to treat hyperhidrosis.

24. The method of claim 16 , wherein the distal end of the tool comprises a blade.

25. The method of claim 24 , further comprising reciprocating the blade to dissect the tissue.

26. The method of claim 16 , wherein elevating the target portion of dermal tissue comprises applying a vacuum within the treatment region of the handpiece.

27. The method of claim 16 , wherein the handpiece has an apposition surface and the target portion of dermal tissue is elevated into engagement with the apposition surface, the apposition surface being at a predetermined height relative to the surrounding tissue.

28. The method of claim 16 , wherein the guidance track comprises one or more channels defining a pattern such that as the tool moves through the pattern a lesion of predetermined shape is created within the target portion of dermal tissue.

29. The method of claim 16 , wherein the tool is inserted into the treatment region through a conduit in the handpiece.

30. The method of claim 16 , wherein the target portion of dermal tissue is held in traction while the tool dissects the tissue.
