Abstract

An apparatus comprises a sleeve, a seal, and an anchor assembly. The sleeve is flexible and non-permeable and is configured to fit in a duodenum. The seal is positioned at the proximal end of the sleeve and is configured to seal the proximal end of the sleeve against the stomach mucosa, the pylorus, or the duodenum mucosa. The anchor assembly comprises a pair of atraumatic anchor ends and a resilient member positioned between the anchor ends. At least a portion of the resilient member extends along at least a portion of the length of the sleeve. The resilient member is configured to bias the anchor ends generally toward each other, to thereby substantially anchor the sleeve in the duodenum. The sleeve is configured to prevent chyme from contacting the mucosa of the duodenum; and to prevent enzymes excreted in the duodenum from mixing with chyme in the duodenum.

Claims (35)

1. An apparatus comprising:

(a) a flexible, non-permeable sleeve configured to fit in a duodenum, wherein the sleeve includes a distal end and a proximal end, wherein the sleeve is configured to define a first lumen;

(b) a seal positioned at the proximal end of the sleeve, wherein the seal is configured to maintain patency of the first lumen at the proximal end of the sleeve, wherein the seal is further configured to seal the proximal end of the sleeve against an anatomical structure associated with the duodenum;

(c) an anchor assembly, wherein the anchor assembly comprises:

(i) a distal anchor end,

(ii) a proximal anchor end, and

(iii) a resilient member positioned between the distal anchor end and the proximal anchor end, wherein at least a portion of the resilient member extends along at least a portion of the length of the sleeve, wherein the resilient member is configured to bias the anchor ends generally toward each other.

2. The apparatus of claim 1 , wherein the first anchor end and the second anchor end each have atraumatic shapes.

3. The apparatus of claim 2 , wherein the first anchor end and the second anchor end each define a loop shape.

4. The apparatus of claim 1 , wherein the resilient member is biased to assume a bent configuration.

5. The apparatus of claim 1 , wherein the resilient member is secured to the seal.

6. The apparatus of claim 5 , wherein the resilient member is not directly secured to a portion of the sleeve distal to the seal.

7. The apparatus of claim 1 , wherein at least one of the anchor ends includes an integral bladder.

8. The apparatus of claim 1 , wherein the seal comprises a stent.

9. The apparatus of claim 8 , wherein the stent is resiliently biased to define a circumference larger than a pylorus.

10. The apparatus of claim 1 , wherein the distal anchor end is positioned within the sleeve.

11. The apparatus of claim 1 , wherein the proximal anchor end is positioned proximal to the sleeve.

12. The apparatus of claim 1 , wherein the proximal anchor end is positioned proximal to the seal.

13. The apparatus of claim 1 , further comprising a fillable member extending along at least part of the length of the resilient member.

14. The apparatus of claim 13 , wherein the fillable member includes a foam material.

15. The apparatus of claim 13 , wherein the fillable member comprises a second lumen defined in a sidewall of the sleeve.

16. The apparatus of claim 1 , wherein the resilient member is secured to the sleeve.

17. An apparatus comprising:

(a) a resilient member comprising a wire positionable along a curvature of a duodenum, wherein the resilient member is moveable between a first configuration and a second configuration, wherein the member has a straight shape in the first configuration, wherein the resilient member has a bent shape in the second configuration, wherein the resilient member is resiliently biased to assume the second configuration;

(b) a first anchor end positioned at a distal end of the resilient member;

(c) a second anchor end positioned at a proximal end of the resilient member;

(d) a flexible sleeve extending along at least part of the resilient member, wherein the flexible sleeve and the resilient member are configured to fit within a duodenum; and

(e) a sealing member, wherein the sealing member is configured to seal the flexible sleeve against an anatomical structure associated with the duodenum.

18. A method of installing a duodenal sleeve using an insertion instrument in a duodenum and stomach of a patient, wherein the duodenal sleeve comprises a resilient member, a flexible sleeve, a first anchor end, and a second anchor end, wherein the flexible sleeve has a distal end and a proximal end, the method comprising:

(a) advancing a distal end of the insertion instrument into the duodenum;

(b) advancing the flexible sleeve into the duodenum;

(c) sealing the proximal end of the flexible sleeve against an anatomical structure associated with the duodenum, wherein the anatomical structure associated with the duodenum comprises mucosa of the duodenum, mucosa of the stomach, or the pylorus;

(d) positioning the first anchor end in the duodenum;

(e) positioning the second anchor end in the stomach; and

(f) releasing the resilient member from the insertion instrument, wherein the released resilient member urges the first and second anchor ends generally toward each other to substantially secure the sleeve in the duodenum.
