Abstract

A surgical instrument system includes an end effector, a generator, and a controller. The end effector includes an ultrasonic blade and at least one electrode surface. The generator provides power to the end effector. The end effector applies ultrasonic energy to tissue via the blade or RF energy to tissue via the at least one electrode surface. The controller is configured to select between one or both of ultrasonic energy or RF energy, and thereby control the generator to provide the selected one or both of ultrasonic energy or RF energy at the end effector, based on a sensed operating condition of the end effector. The controller may select between ultrasonic energy and RF energy based on whether a clamp arm is in an open position relative to the blade, based on which button is being activated, and based on whether tissue is sensed at the end effector.

Claims (47)

1. A surgical instrument system comprising:

(a) an end effector operable to cut and seal tissue, wherein the end effector comprises:

(i) an ultrasonic blade, wherein the end effector is operable to apply ultrasonic energy at the blade for cutting tissue,

(ii) a clamp arm, wherein the clamp arm is pivotable relative to the ultrasonic blade,

(iii) a first electrode provided by the ultrasonic blade, and

(iv) a second electrode provided by the clamp arm, wherein the first and second electrodes are configured to cooperate to apply bipolar RF energy for sealing tissue cut by the ultrasonic blade;

(b) a generator configured to provide power to the end effector having the ultrasonic blade and the first and second electrodes, wherein the generator is configured to drive the ultrasonic blade to apply ultrasonic energy for cutting tissue, wherein the generator is further configured to deliver RF energy sufficient to seal tissue to the first and second electrodes for sealing tissue cut by the ultrasonic blade; and

(c) a controller in communication with the generator, wherein the controller is configured to control the generator to drive the ultrasonic blade and to energize the first and second electrodes of the end effector, wherein the controller is further configured to select one or both of ultrasonic energy or RF energy sufficient to seal tissue, and thereby control the generator to provide the selected one or both of ultrasonic energy or RF energy sufficient to seal tissue at the end effector, based on at least one sensed operating condition of the end effector.

2. The system of claim 1 , wherein the first electrode is provided at a distal portion of the ultrasonic blade.

3. The system of claim 1 , wherein the controller is configured to sense the presence of tissue at the end effector.

4. The system of claim 3 , wherein the controller is configured to sense the presence of tissue at the end effector based on a measured impedance level associated with the first and second electrodes.

5. The system of claim 3 , wherein the controller is configured to sense an amount of force applied to the clamp arm to detect the presence of tissue at the end effector.

6. The system of claim 3 , wherein the controller is configured to sense the position of the clamp arm relative to the ultrasonic blade.

7. The system of claim 3 , further comprising:

(a) a first actuator operable to actuate the system to transect tissue with the end effector; and

(b) a second actuator operable to actuate the system to seal tissue with the end effector.

8. The system of claim 7 , wherein the controller is configured to control the generator to provide RF energy at the end effector when the second actuator is actuated and no tissue is sensed at the end effector.

9. The system of claim 7 , wherein the controller is configured to control the generator to provide a combination of RF energy and ultrasonic energy at the end effector when the second actuator is actuated and tissue is sensed at the end effector.

10. The system of claim 7 , wherein the controller is configured to control the generator to provide ultrasonic energy when the first actuator is actuated.

11. The system of claim 1 , wherein the controller is configured to control the generator to provide a continuous amount of RF energy.

12. The system of claim 1 , wherein the controller is configured to control the generator to provide a continuous amount of ultrasonic energy.

13. The system of claim 1 , wherein the controller is configured to control the generator to provide alternating pulses of RF energy and ultrasonic energy.

14. The system of claim 1 , wherein the controller is configured to control the generator to automatically provide an increase or decrease in the amount of RF energy.

15. The system of claim 1 , wherein the controller is configured to control the generator to automatically provide an increase or decrease in the amount of ultrasonic energy.

16. A surgical instrument system comprising:

(a) an end effector operable to cut and seal tissue, the end effector comprising:

(i) an ultrasonic blade, wherein the end effector is operable to apply ultrasonic energy at the blade for cutting tissue,

(ii) a clamp arm pivotable relative to the blade,

(iii) a first electrode provided by one of the ultrasonic blade or the clamp arm, and

(iv) a second electrode provided by one of the ultrasonic blade or the clamp arm, wherein the first and second electrodes are configured to cooperate to apply bipolar RF energy for sealing tissue cut by the ultrasonic blade;

(b) a generator configured to provide power to the end effector having the ultrasonic blade and the first and second electrodes;

(c) a first actuator; and

(d) a second actuator;

wherein the generator is configured to selectively provide ultrasonic energy to the ultrasonic blade of the end effector for cutting tissue and RF energy to the first and second electrodes of the end effector for sealing tissue based on at least two of:

(i) the position of the clamp arm relative to the blade,

(ii) the presence of tissue at the end effector, or

(iii) the actuation of at least one of the first actuator or the second actuator.

17. The system of claim 16 , wherein the first electrode is provided by the ultrasonic blade.

18. The system of claim 17 , wherein the second electrode is provided by the clamp arm.

19. A surgical instrument system comprising:

(a) an end effector operable to cut and seal tissue, wherein the end effector comprises:

(i) an ultrasonic blade, wherein the end effector is operable to apply ultrasonic energy at the blade for cutting tissue,

(ii) a first electrode provided by the ultrasonic blade, and

(iii) a second electrode provided separately from the first electrode, wherein the first and second electrodes are configured to cooperate to apply bipolar RF energy for sealing tissue cut by the ultrasonic blade;

(b) a generator configured to provide power to the end effector having the ultrasonic blade and the first and second electrodes, wherein the generator is configured to deliver RF energy sufficient to seal tissue to the first and second electrodes; and

(c) a controller in communication with the generator, wherein the controller is configured to control the generator to provide a combination of ultrasonic energy to the ultrasonic blade of the end effector and RF energy sufficient to seal tissue to the first and second electrodes of the end effector, based on at least one sensed operating condition of the end effector.

20. The system of claim 19 , wherein the second electrode is provided by the clamp arm.
