Abstract

Described are methods and devices useful for supporting posterior vaginal tissue for various purposes such as treating or preventing vaginal prolapse or enterocele, especially in a patient not having a uterus, the devices including implants designed to contact the vaginal cuff and connect to sacral anatomy such as the sacrum or the uterosacral ligaments.

Claims (35)

1. A surgical implant comprising a central support portion having a length, and two extension portions extending longitudinally in opposite directions from the central support portion and aligned with the length, the central support portion comprising:

an anterior lobe extending in an anterior lateral direction on an anterior side of the length, and

a posterior lobe extending in a posterior lateral direction on a posterior side of the length that is on a side of the central support portion opposite of the anterior lobe, the posterior lobe comprising:

a main posterior lobe body comprising:

a proximal portion aligned with the length of the central support portion, wherein the proximal portion extends from a line extending along a length of the extension portions on a posterior edge of the extension portions; and

a distal portion spaced from the proximal portion in the posterior lateral direction, and

two lobe extensions made of planar implant material and extending from the distal portion in the posterior lateral direction,

a suture extending along at least a portion of a length of one of the extension portions, the suture extending between a proximal location along the length of the one extension portion to a distal location along the length of the one extension portion, wherein the suture does not extend into the central support portion,

wherein the central support portion is sized and shaped to be attached to vaginal or cervical tissue after removal of a uterus, a cervix, or both, by attaching the anterior lobe to pubocervical fascia, attaching the posterior lobe to rectovaginal fascia, and attaching the lobe extensions to anatomy selected from a uterosacral ligament and a sacrum, and

wherein each of the two extension portions extend from a proximal end located at the central support portion to a distal end, and are fixedly attached to the central support portion.

2. A kit comprising the implant of claim 1 , further comprising a tunneler or needle useful for placing one of the extension portions through a tissue path extending between tissue of a vaginal region and an external perirectal incision.

3. A kit comprising

the implant of claim 1 , comprising a connecter at an end of each extension portion, and

a needle useful for placing the extension portions through tissue paths, each connector capable of connecting to a tip of the needle.

4. The implant of claim 1 comprising one or more sheaths extending along the length of one or more of the extension portions.

5. The implant of claim 1 wherein the lobe extensions are about 1 cm to about 5 cm long.

6. The implant of claim 1 wherein the lobe extensions are about 0.5 cm to about 1.5 cm wide.

7. The implant of claim 1 comprising a dilator at an end of each extension portion.

8. The implant of claim 1 wherein the lobe extensions are fixedly attached to the main posterior lobe body.

9. A kit comprising

the implant of claim 8 , comprising a connecter at an end of each extension portion, and

a needle useful for placing the extension portions through tissue paths, each connector capable of connecting to a tip of the needle.

10. The kit of claim 3 , wherein the tip of the needle is capable of engaging the connector to pull the connector through the tissue paths.

11. The implant of claim 1 , wherein the lobe extensions are sufficiently long to reach the sacrum while the central support portion is attached to vaginal or cervical tissue.

12. The implant of claim 1 , wherein the suture includes a suture free end extending from the extension portion at a distal location.

13. The implant of claim 1 wherein the central support portion includes the two extension portions but does not include any additional extension portion extending from the support portion.

14. The implant of claim 1 wherein the planar implant material is a biologic material or a mesh material.

15. The implant of claim 1 wherein the posterior lobe body and the two lobe extensions are integrally made of the planar implant material.

16. The implant of claim 1 wherein the two lobe extensions are not sutures.

17. The implant of claim 1 wherein the two extension portions extend in opposed directions from the support portion, and the two lobe extensions extend in directions from the support portion that are not parallel to the directions of the two extension portions.

18. The implant of claim 16 wherein the lobe extensions extend from the central support portion in directions that are not collinear.

19. The implant of claim 16 wherein the lobe extensions extend from the central support portion in directions that are parallel but not collinear.

20. The implant of claim 1 comprising only two extension portions.

21. The implant of claim 1 wherein the posterior lateral direction is perpendicular to the length of the central support portion.

22. The implant of claim 1 wherein the main posterior lobe body comprises a distal edge of the planar implant material extending in a lengthwise direction between the two lobe extensions, and the distal portion is aligned with the distal edge.
