Abstract

Improved devices, systems and methods for treating a tissue region provide straightforward, yet reliable ways for installing diverse functional components within the confined space of a catheter-based instrument.

Claims (15)

1. A surgical apparatus comprising a plurality of electrodes and an expandable structure including an array of spines forming a basket, the spines movable between expanded and contracted positions, the spines having a first lumen, a second lumen alongside and external of the first lumen, and a third lumen, a first side opening formed in a sidewall of the spine communicating with the first lumen, and a second side opening formed in a sidewall of the spine communicating with the second lumen, the second side opening generally axially aligned with the first side opening and radially spaced from the first side opening to be positioned side by side in a radial direction, an electrode of the plurality of electrodes movable within the first lumen of the spines, the electrode movable within the first lumen from a first retracted position to a second extended position, the distal end of the electrode including a piercing tip, and the piercing tip of the electrode extending through the first side opening in the first lumen of the spine when the electrode is moved to the second extended position, wherein the second lumen carries a temperature sensing element and the temperature sensing element is exposed through the second side opening the second side opening, generally axially aligned with the first and third side openings to rest against surface tissue where the electrode exits the first side opening, and wires extend through the second lumens for connection to a controller and the third lumen is connectable to a tube for carrying fluid therethrough, and a third side opening communicates with the third lumen, the third side opening formed in a sidewall of the spine generally axially aligned with the first side opening so cooling occurs adjacent the electrode.

2. The surgical apparatus of claim 1 , wherein the first lumen of the spine is positioned between the second and third lumen.

3. The surgical apparatus of claim 1 , wherein distal and proximal ends of the spines are held together, the distal ends held together by an elongated longitudinally extending hub.

4. The surgical apparatus of claim 1 , further comprising a balloon positioned within the basket, the balloon inflatable to move the spines to the expanded position.

5. The surgical apparatus of claim 1 , further comprising an irrigation manifold, wherein individual tubes which convey fluid to the third lumens of the spines are coupled to the manifold at one end, and a fluid conveying tube is connected to the manifold at a second end.

6. The surgical apparatus of claim 5 , wherein the irrigation manifold has multiple outlet ports and a single inlet port in communication with the fluid conveying tube, and a single outlet port is in fluid communication with one of the individual tubes.

7. The surgical apparatus of claim 6 , wherein the manifold has an indented surface such that the outlet ports are arranged in an arcuate fashion.

8. The surgical apparatus of claim 1 , further comprising a lever and a carrier, the plurality of electrodes secured to the carrier and the lever is movable to move the plurality electrodes between the retracted and extended positions.

9. The surgical apparatus of claim 1 , wherein the spines have a rectilinear cross-sectional shape.

10. The surgical apparatus of claim 1 , wherein each electrode is biased with a bend.

11. A surgical apparatus comprising a plurality of electrodes and an expandable structure including an array of spines forming a basket, the spines movable between expanded and contracted positions, the spines having an electrode receiving lumen and a fluid receiving lumen alongside and external of the electrode receiving lumen, an electrode of the plurality of electrodes movable within the electrode receiving lumen of the spines from a first retracted position to a second extended position wherein in the extended position the distal end of the electrode extends outwardly from the spine through a first side opening in a sidewall of the spine, and the fluid receiving lumens of the spines are in communication with an irrigation manifold, the manifold having a main inlet port for receiving fluid and a plurality of outlet ports fluidly communicating with the fluid receiving lumens of the spines to deliver fluid through second side openings in the spines, the second side opening positioned in a sidewall of the spine adjacent the first side opening in the spine so the first and second side openings open at a same axial region of the apparatus and the second side opening is radially spaced to be positioned side by side in a radial direction with the first side opening so cooling occurs adjacent the electrode, wherein the spines include an additional lumen for carrying a temperature sensing element, and a third side opening communicating with the additional lumen, the third side opening positioned in a sidewall of the spine adjacent the first and second side openings so the third side opening opens at the same axial region of the apparatus as where the first and second side openings to rest against surface tissue where the electrode exits the first side opening, and wires extend through the additional lumens for connection to a controller.

The surgical apparatus of claim 11 , wherein the distal end of the electrodes include a piercing tip, and the piercing tip of the electrodes extends through the first side opening in the spine when the electrode is moved to the second extended position, the first side opening communicating with the first lumen.

13. The surgical apparatus of claim 11 , wherein the outlet ports of the manifold are arranged in an arcuate fashion.

14. The surgical apparatus of claim 13 , wherein the manifold includes a longitudinally extending indentation.

15. The surgical apparatus of claim 11 , wherein the electrode receiving lumen is positioned between the fluid receiving lumen and the additional lumen of the spine.
