Abstract

A system includes an implantable neurostimulator device capable of modulating cerebral blood flow to treat epilepsy and other neurological disorders. In one embodiment, the system is capable of modulating cerebral blood flow (also referred to as cerebral perfusion) in response to measurements and other observed conditions. Perfusion may be increased or decreased by systems and methods according to the invention as clinically required.

Claims (28)

1. A method for responding to a neurological disorder in a human patient comprising:

obtaining a first perfusion measurement of tissue in an area associated with a neurological event;

obtaining a second perfusion measurement of tissue in an area not associated with the neurological event;

detecting for one of hyperperfusion and hypoperfusion based on a metric derived from the first perfusion measurement and the second perfusion measurement;

applying at least one of stimulation to a first target region when hyperperfusion is detected, and stimulation to a second target region when hypoperfusion is detected.

2. The method of claim 1 , wherein the tissue in an area associated with a neurological event comprises tissue associated with a focus of epileptiform activity.

3. The method of claim 2 , wherein the epileptiform activity corresponds to a seizure.

4. The method of claim 1 , wherein the tissue in an area not associated with a neurological event comprises tissue outside of a focus of epileptiform activity.

The method of claim 1 , wherein detecting comprises comparing the metric to an upper threshold and a lower threshold, wherein hyperperfusion is detected based at least in part on when the metric is above the upper threshold, and hypoperfusion is detected based at least in part on when the metric is below the lower threshold.

6. The method of claim 1 , wherein the metric is a ratio of the first perfusion measurement and the second perfusion measurement.

7. The method of claim 1 , wherein the tissue in an area associated with a neurological event comprises a seizure focus located in a hemisphere of the brain, and the first target region comprises a contralateral hemisphere.

8. The method of claim 1 , wherein the tissue in an area associated with a neurological event comprises a seizure focus located in a hemisphere of the brain, and the first target region comprises the a region of the hemisphere near, but not at, the seizure focus.

9. The method of claim 1 , wherein the second target region comprises a focus of epileptiform activity.

10. The method of claim 1 , wherein the first measurement and the second measurement correspond to one of the following: an optical oximetry measurement, an electromagnetic field measurement, a thermographic measurement, an ultrasonic transducer, and an electrochemical oxygen concentration measurement.

11. The method of claim 1 , wherein the stimulation comprises one of electrical stimulation, thermal stimulation, optical stimulation, electromagnetic stimulation, ultrasonic stimulation.

12. An implantable medical device for responding to a neurological disorder in a human patient comprising:

a detection subsystem configured to:

obtain a first perfusion measurement of tissue in an area associated with a neurological event,

obtain a second perfusion measurement of tissue in an area not associated with the neurological event, and

detect for one of hyperperfusion and hypoperfusion based on a metric derived from the first perfusion measurement and the second perfusion measurement; and

a therapy subsystem coupled to the detection subsystem and configured to apply at least one of a stimulation to a first target region when hyperperfusion is detected, and a stimulation to a second target region when hypoperfusion is detected.

13. The device of claim 12 , wherein the detection subsystem is configured to:

compare the metric to an upper threshold and a lower threshold;

detect hyperperfusion based at least in part on when the metric is above the upper threshold; and

detect hypoperfusion based at least in part on when the metric is below the lower threshold.

14. The device of claim 12 , wherein the metric is a ratio of the first perfusion measurement and the second perfusion measurement.

15. The device of claim 12 , further comprising a first probe implanted at the first target region, and a second probe implanted at the second target region, wherein the first probe and the second probe are coupled to one or more of the detection system and the therapy subsystem.

16. The device of claim 15 , wherein at least one of first probe and the second probe comprise a thermal probe, a sonic probe, an electromagnetic probe, an electrochemical oxygen probe, and an electrical probe.
