Abstract

Unitary endoscopic vessel harvesting devices are disclosed. In some embodiments, such devices may comprise an elongated body having a proximal end and a distal end. A conical tip may be disposed at the distal end of the elongated body. In addition, the surgical instrument may include one or more surgical instruments moveable in a longitudinal direction along an axis substantially parallel to a central longitudinal axis of the cannula from a retracted position proximally of a distal end of the tip to an advanced position toward the distal end of the tip to seal and cut a blood vessel.

Claims (33)

1. A surgical device comprising:

an elongated body having a proximal end and a distal end;

a conical tip disposed at the distal end of the elongated body; and

a cutting unit having a first cutting portion and a second cutting portion, the cutting unit being movable in a longitudinal direction, relative to the elongated body, from a retracted position substantially proximal of the conical tip to an extended position at least partially distal of the conical tip, wherein in the extended position, the first and second cutting portions being circumferentially spaced apart to capture a blood vessel in a circumferential space therebetween with at least one of the first and second cutting portions being rotatable circumferentially around the conical tip toward another of the first and second cutting portions to cut the captured blood vessel.

2. The surgical device of claim 1 wherein the first and second cutting portions are moveable beyond a distal end of the conical tip in the extended position.

3. The surgical device of claim 1 , wherein the first and second cutting portions each comprise an elliptical protrusion extending distally from the cutting unit and terminating in a rounded distal tip.

4. The surgical device of claim 1 , wherein the another of the first and second cutting portions is stationary.

5. The surgical device of claim 1 , wherein the first cutting portion is circumferentially rotatable about the conical tip toward the second cutting portion and the second cutting portion is circumferentially rotatable about the conical tip toward the first cutting portion to cut the captured blood vessel.

6. The surgical device of claim 1 , wherein the first cutting portion has a sharpened edge and an edge of the second cutting portion facing the sharpened edge of the first cutting portion is flat.

7. The surgical device of claim 1 , wherein the first cutting portion and the second cutting are configured to be energized for sealing, cutting or both of the captured blood vessel.

The surgical device of claim 1 , wherein the elongated body comprises a first cutting member and a second cutting member concentrically disposed within the first cutting member, and the first cutting portion and the second cutting portion are formed at respective distal ends of the first cutting member and the second cutting member.

9. The surgical device of claim 8 , wherein the first cutting member and second cutting member are configured to conduct radiofrequency (RF) energy to the first cutting portion and the second cutting portion, respectively.

10. The surgical device of claim 9 , wherein an insulating coating is disposed between the first cutting member and the second cutting member.

The surgical device of claim 1 , further comprising a sleeve having a proximal end attached to an outer surface of the elongated body and a distal tip engaging the conical tip, wherein the distal tip is unattached to the conical tip to enable the first cutting portion and the second cutting portion to advance through the sleeve.

12. The surgical device of claim 1 , the conical tip defining an inner cavity for receiving an endoscope.

The surgical device of claim 12 , wherein the inner cavity terminates at an internal apex, the inner cavity being in communication with one or more lumens of the elongated body and an indented external apex at a distal end of the conical tip, wherein the internal apex and the indented external apex are co-linear with a central axis of the elongated body.

14. A surgical device comprising:

an elongated body having a proximal end and a distal end;

a conical tip disposed at the distal end of the elongated body, the conical tip having one or more openings in a wall of the conical tip; and

a cutting unit having a first cutting portion and a second cutting portion, the cutting unit being moveable in a longitudinal direction, relative to the elongated body, from a retracted position substantially proximal of the conical tip to an extended position at least partially distal of the conical tip, wherein in the extended position, the first and second cutting portions being circumferentially spaced apart to capture a blood vessel in a circumferential space therebetween with at least one of the first and second cutting portions being rotatable circumferentially around the conical tip toward another of the first and second cutting portions to cut the captured blood vessel.

15. The surgical device of claim 14 wherein the one or more openings comprise one or more optical windows.

16. The surgical device of claim 15 further comprising a cover moveable from a first position over the one or more optical windows to cover the one or more optical windows to a second position away from the one or more optical windows to expose the one or more optical windows.

17. The surgical device of claim 14 wherein the one or more openings are located at a distal end of the conical tip for advancing one or more surgical instruments therethrough.

18. A surgical device comprising:

an elongated body having a proximal end and a distal end;

a conical tip disposed at the distal end of the elongated body; and

a cutting unit having a first cutting portion and a second cutting portion, the cutting unit being movable in a longitudinal direction, relative to the elongated body, from a retracted position substantially proximal of the conical tip to an extended position over the conical tip, wherein in the extended position, the first and second cutting portions being circumferentially spaced apart to capture a blood vessel in a circumferential space therebetween with at least one of the first and second cutting portions being rotatable circumferentially about the conical tip toward another of the first and second cutting portions to cut the captured blood vessel,

wherein the first and second cutting portions each extend in the longitudinal direction and terminate in a rounded distal tip.

19. A surgical device comprising:

an elongated body having a proximal end and a distal end;

a conical tip disposed at the distal end of the elongated body; and

a cutting unit having a first cutting portion and a second cutting portion, the cutting unit being movable in a longitudinal direction, relative to the elongated body, from a retracted position substantially proximal of the conical tip to an extended position over the conical tip, wherein in the extended position, the first and second cutting portions being circumferentially spaced apart to capture a blood vessel in a circumferential space therebetween with at least one of the first and second cutting portions being rotatable circumferentially about the conical tip toward another of the first and second cutting portions to cut the captured blood vessel,

wherein the first cutting portion has a sharpened edge and an edge of the second cutting portion facing the sharpened edge of the first cutting portion is flat.
