Abstract

An MR Spectroscopy (MRS) system and approach is provided for diagnosing painful and non-painful discs in chronic, severe low back pain patients (DDD-MRS). A DDD-MRS pulse sequence generates and acquires DDD-MRS spectra within intervertebral disc nuclei for later signal processing & diagnostic analysis. An interfacing DDD-MRS signal processor receives output signals of the DDD-MRS spectra acquired and is configured to optimize signal-to-noise ratio (SNR) by an automated system that selectively conducts optimal channel selection, phase and frequency correction, and frame editing as appropriate for a given acquisition series. A diagnostic processor calculates a diagnostic value for the disc based upon a weighted factor set of criteria that uses MRS data extracted from the acquired and processed MRS spectra along regions associated with multiple chemicals that have been correlated to painful vs. non-painful discs. A diagnostic display provides a scaled, color coded legend and indication of results for each disc analyzed as an overlay onto a mid-sagittal T2-weighted MRI image of the lumbar spine for the patient being diagnosed. Clinical application of the embodiments provides a non-invasive, objective, pain-free, reliable approach for diagnosing painful vs. non-painful discs by simply extending and enhancing the utility of otherwise standard MRI exams of the lumbar spine.

Claims (38)

1. A diagnostic system for providing diagnostic information for a medical condition associated with a region of interest comprising at least a portion of an intervertebral disc in a patient, the diagnostic system comprising:

a magnetic resonance spectroscopy (MRS) system configured to generate and acquire an MRS spectral acquisition series of data for a voxel located within the region of interest in the intervertebral disc using an MRS pulse sequence;

an MRS signal processor configured to process the MRS spectral acquisition series of data to produce a processed MRS spectrum; and

an MRS diagnostic processor configured to process the processed MRS spectrum to:

extract a first measurement from a first spectral peak region corresponding with proteoglycan (PG);

extract at least one additional measurement from at least one additional spectral peak region corresponding with each of lactate (LA) and alanine (AL); and

generate the diagnostic information for the medical condition of the intervertebral disc using the first measurement and the at least one additional measurement.

2. The diagnostic system of claim 1 , wherein the at least one additional spectral peak region comprises a second spectral peak region corresponding with lactate (LA) and a third spectral peak region corresponding with alanine (AL).

3. The diagnostic system of claim 1 , wherein the at least one additional spectral peak region comprises a combined second spectral peak region corresponding with both lactate (LA) and alanine (AL).

4. The diagnostic system of claim 3 , wherein the at least one additional measurement extracted from the combined second spectral peak region comprises a power measurement along the combined second spectral peak region.

5. The diagnostic system of claim 1 , wherein the diagnostic information is correlative for diagnosing the intervertebral disc as painful or non-painful.

6. The diagnostic system of claim 1 , wherein the MRS diagnostic processor is configured to diagnose the intervertebral disc as painful or non-painful, and to indicate the diagnosis to a user via a user interface.

7. The diagnostic system of claim 6 , wherein the MRS diagnostic processor is configured to indicate the diagnosis using a visual indicator overlay onto an MRI image of a region of the spine that includes the intervertebral disc.

8. The diagnostic system of claim 1 , wherein the MRS signal processor and the MRS diagnostic processor comprise software in computer readable non-transitory storage configured to run on one or more computer processors.

9. The diagnostic system of claim 1 , wherein the MRS diagnostic processor is configured to provide the diagnostic information based at least on a value that represents a volume of the voxel.

10. The diagnostic system of claim 1 , wherein:

the MRS system comprises an MR scanner that is configured to also perform a T1-rho measurement in the region of interest; and

the MRS diagnostic processor is further configured to derive the diagnostic information based upon both the measurements extracted from the first and at least one additional spectral peak regions, and also the T1-rho measurement.

11. A diagnostic method for providing diagnostic information for a medical condition of a region of interest comprising at least a portion of an intervertebral disc in a patient, the diagnostic method comprising;

generating and acquiring a magnetic resonance spectroscopy (MRS) spectral acquisition series of data for a voxel located within the region of interest in the intervertebral disc using an MRS system with an MRS scanner comprising a magnet and operated according to an MRS pulse sequence to generate said series of data and also with a detector assembly configured for receiving said series of data;

signal processing the MRS spectral acquisition series of data to produce a processed MRS spectrum; and

analyzing the processed MRS spectrum to:

extract a first measurement from a first spectral peak region corresponding with proteoglycan (PG);

extract at least one additional measurement from at least one additional spectral peak region corresponding with each of lactate (LA) and alanine (AL); and

generate the diagnostic information for the medical condition of the intervertebral disc using the first measurement and the at least one additional measurement.

12. The diagnostic method of claim 11 , wherein the at least one additional spectral peak region comprises a second spectral peak region corresponding with lactate (LA) and a third spectral peak region corresponding with alanine (AL).

13. The diagnostic method of claim 11 , wherein the at least one additional spectral peak region comprises a combined second spectral peak region corresponding with both lactate (LA) and alanine (AL).

14. The diagnostic method of claim 13 , wherein the at least one additional measurement extracted from the combined second spectral peak region comprises a power measurement along the combined second spectral peak region.

15. The diagnostic method of claim 11 , wherein the diagnostic information is correlative for diagnosing the intervertebral disc as painful or non-painful.

16. The diagnostic method of claim 11 , comprising:

diagnosing the intervertebral disc as painful or non-painful; and

indicating the diagnosis to a user via a user interface.

17. The diagnostic method of claim 16 , wherein indicating the diagnosis comprises displaying a visual indicator overlaid onto an MRI image of a region of the spine that includes the intervertebral disc.

The diagnostic method of claim 11 , wherein the signal processing is performed by an MRS signal processor, wherein the analyzing of the processed MRS spectrum to provide the diagnostic information is performed by an MRS diagnostic processor, and wherein the MRS signal processor and the MRS diagnostic processor comprise one or more computer processors running software in computer readable non-transitory storage.

19. The diagnostic method of claim 11 , wherein the analyzing of the processed MRS spectrum includes a calculation using a value that represents a volume of the voxel.

20. The diagnostic method of claim 11 , further comprising:

also performing a T1-rho measurement in the region of interest; and

deriving the diagnostic information using both the measurements extracted from the first and at least one additional spectral peak regions and also the T1-rho measurement.
