Abstract

Surgical access systems and obturators for or used in surgical access systems are provided. The obturator is configured to operate or accommodate the use of at least two different sized trocars. The outer diameter of the adaptable obturator when inserted into a trocar compresses from an initial condition to a compressed condition to match the inner diameter of the trocar.

Claims (34)

1. An obturator insertable into at least two different sized trocars, the obturator comprising:

an elongate shaft having a proximal end and a distal end;

a handle at the proximal end of the elongate shaft;

a tip at the distal end of the elongate shaft; and

at least one compressible portion disposed between the handle and the tip and not extending past the tip, the at least one compressible portion having an initial state delimiting a first diameter and the at least one compressible portion movable to a compressed state delimiting a second diameter smaller than the first diameter;

wherein the at least one compressible portion is at least one compressible rib, the at least one compressible rib extends up to a gap portion separating a proximal end of the at least one compressible rib with a prong.

The obturator of claim 1 further comprising a gap between the at least one compressible rib and the elongate shaft, the gap between the at least one compressible rib and the elongate shaft diminishing in size as the at least one compressible rib moves from the initial state to the compressed state.

3. The obturator of claim 1 further comprising a core seal between the handle and the at least one compressible rib and the core seal being as rigid as the elongate shaft.

4. The obturator of claim 1 wherein the at least one compressible rib comprises a plurality of compressible ribs spaced circumferentially around the elongate shaft.

5. The obturator of claim 1 wherein the at least one compressible rib moves towards the elongate shaft as the least one compressible rib moves from the initial state to the compressed state.

6. The obturator of claim 1 wherein the at least one compressible rib is curvilinear.

7. The obturator of claim 1 wherein the at least one compressible rib is not twistable or inflatable.

The obturator of claim 1 further comprising a compressible sleeve covering the at least one compressible rib, the compressible sleeve and the at least one compressible rib having an initial state delimiting a first diameter and the compressible sleeve and the at least one compressible rib compressible to a compressed state delimiting a diameter smaller than the first diameter.

9. The obturator of claim 1 wherein the obturator further comprises a core seal between the handle and the tip, the core seal being made of a non-elastomeric material.

10. The obturator of claim 1 wherein the obturator further comprises a core seal between the handle and the tip, the core seal being non-compressible.

11. The obturator of claim 1 wherein the obturator further comprises a core seal between the handle and the tip, the core seal and the elongate shaft being made from the same material.

12. A trocar system comprising:

a first trocar having a seal assembly and a cannula with an inner diameter and a length;

an obturator insertable into the first trocar, the obturator comprising:

an elongate shaft having a proximal end and a distal end;

a handle at the proximal end of the elongate shaft;

a tip at the distal end of the elongate shaft, the tip being as rigid as the elongate shaft and the tip and the elongate shaft being insertable through the seal assembly of the first trocar; and

a compressible portion disposed between the handle and the tip and not extending past the tip, the at least one compressible portion delimiting a first diameter greater than the inner diameter of the cannula and compressible to delimit a second diameter smaller than the inner diameter of the cannula.

13. The trocar system of claim 12 wherein the obturator further comprises a depth limiter disposed near the handle; wherein a length between the depth limiter to the tip is longer than the length of the cannula.

14. The trocar system of claim 12 wherein the obturator has an insufflation pathway.

15. The trocar system of claim 12 further comprising a surgical access device with the obturator and first trocar adapted to extend through the access device.

16. The trocar system of claim 15 wherein the surgical access device further comprises a gel material.

17. The trocar system of claim 12 wherein the compressible portion, elongate shaft and tip form a monolithic structure.

18. The trocar system of claim 17 wherein the compressible portion, elongate shaft and tip are made from the same material.

19. The trocar system of claim 12 wherein the obturator further comprises a core seal disposed between the handle and the at least one compressible portion; wherein the seal assembly of the first trocar has an instrument seal and the core seal has a sealing relationship with the instrument seal when the obturator is inserted fully through the first trocar and further comprising a second trocar having a seal assembly with an instrument seal and the core seal has a sealing relationship with the instrument seal when the obturator is inserted fully through the second trocar.

20. The obturator of claim 12 wherein the compressible portion comprises at least one compressible rib, the at least one compressible rib extends up to a gap portion separating a proximal end of the at least one compressible rib with a prong.

21. The obturator of claim 12 wherein the obturator further comprises a core seal between the handle and the compressible portion, the core seal being made of a non-elastomeric material.

22. The obturator of claim 12 wherein the obturator further comprises a core seal between the handle and the compressible portion, the core seal being non-compressible.

23. The obturator of claim 12 wherein the obturator further comprises a core seal between the handle and the compressible portion, the core seal and the elongate shaft being made from the same material.
