Abstract

Method and system for treating a patient using a compressible, pressure-attenuating device. According to one embodiment, the system is used to treat urinary tract disorders and can include one or more of an access device, a delivery device, a pressure-attenuating device, and a removal device. The access device may be used to create a passageway to an anatomical structure, such as the patient's bladder. The delivery device may be inserted through the passageway created by the access device and may be used to deliver the pressure-attenuating device to the anatomical structure. The removal device may be inserted through the passageway created by the access device and may be used to view the bladder and/or to capture, to deflate and to remove the pressure-attenuating device.

Claims (37)

1. An access device comprising:

a handle comprising a fluid control system;

an elongate, tubular body extending distally from the handle and having a proximal end, a distal end, and a central lumen extending from a proximal opening at the proximal end to a distal opening at the distal end, the central lumen configured to provide access to an anatomical structure;

a first tube in communication with the lumen without obstruction of the proximal or distal openings;

a second tube in communication with the lumen without obstruction of the proximal or distal openings;

a sealable access port valve in the central lumen, on the proximal side of the first and second tubes;

wherein the fluid control system is operable to manually control fluid flow through the first and second tubes by pinching at least one of the first tube and the second tube, the fluid control system moveable between at least three positions to—1) simultaneously close the first and second tubes, 2) close only the first tube, and 3) close only the second tube—without obstruction of the central lumen.

2. The access device of claim 1 , wherein the fluid control system further comprises a cam configured to be in contact with and to open or close the first tube and/or the second tube.

3. The access device of claim 1 , wherein the fluid control system further comprises a lever on the handle for controlling the fluid control system, wherein the lever is configured to be adjustable among the three positions by a hand which is simultaneously holding the handle.

4. The access device of claim 3 , wherein the lever is configured to be adjustable through an angular range.

5. The access device of claim 4 , wherein the angular range comprises two positions and is less than or equal to about 45 30, 20, 15, 10, or 5 degrees.

6. The access device of claim 4 , wherein the angular range comprises three positions and is less than or equal to about 90, 60, 45, 40, 30, 20, or 10 degrees.

7. The access device of claim 1 , further comprising a slide ring, attached to a flexible tubular sleeve, and axially slidable with respect to the tubular body.

8. The access device of claim 7 , wherein the flexible tubular sleeve is partially contained within the central lumen when the slide ring is in a distal position, and proximal retraction of the slide ring everts the flexible tube out of the distal end of the central lumen.

9. The access device of claim 8 , further comprising a cannulated obturator positioned within the central lumen, such that at least a portion of the flexible sleeve is positioned within the obturator when the slide ring is in the distal position.

The access device of claim 9 , wherein the slide ring comprises at least one inwardly and distally extending protrusion, the at least one protrusion engaged with the tubular body and configured to restrain distal axially sliding movement of the slide ring relative to the tubular body while allowing proximal retraction.

11. An access device having fluid inflow and outflow management configured for single handed operation, comprising:

a handle comprising a fluid control system;

an elongate, tubular body extending axially from the handle along a longitudinal axis and having a proximal end, a distal end, and a central lumen extending from a proximal opening at the proximal end to a distal opening at the distal end, the central lumen configured to provide access to an anatomical structure;

a first tube in communication with the lumen without obstruction of the proximal or distal openings;

a second tube in communication with the lumen without obstruction of the proximal or distal openings; and

at least one of a sealable access port valve in the central lumen, on the proximal side of the first and second tubes; and a cannulated obturator positioned within the central lumen;

wherein the fluid control system is operable to manually control fluid flow through the first and second tubes by crushing at least one of the first tube and the second tube, the fluid control system moveable between at least three positions to—1) simultaneously close the first and second tubes, 2) close only the first tube, and 3) close only the second tube—without obstruction of the central lumen, the fluid control system comprising a lever on the handle configured for angular rotation with respect to the longitudinal axis through an angular range sufficient to reach each of the at least three positions by a digit on a hand that is holding the handle.

12. The access device of claim 11 , wherein the angular range is less than or equal to about 90, 60, 45, 40, 30, 20, or 10 degrees.

13. The access device of claim 11 , wherein a partial angular range between two positions of the at least three positions is less than or equal to about 45 30, 20, 15, 10, or 5 degrees.

14. The access device of claim 11 , wherein the fluid control system further comprises a cam configured to be in contact with and to open or close the first tube and/or the second tube.

15. The access device of claim 11 , further comprising a slide ring, attached to a flexible tubular sleeve, and axially slidable with respect to the tubular body.

The access device of claim 15 , wherein the slide ring comprises at least one inwardly and distally extending protrusion, the at least one protrusion engaged with the tubular body and configured to restrain distal axially sliding movement of the slide ring relative to the tubular body while allowing proximal axially sliding movement.

The access device of claim 16 , wherein the tubular body further comprises at least one axially-extending, exterior groove and the at least one protrusion mating with the at least one axially-extending, exterior groove in such a way as to restrain rotation of the slide ring relative to the tubular body.

18. The access device of claim 11 , further comprising a flexible tubular sleeve removably carried by the tubular body.

19. An access device comprising:

a handle comprising a fluid control system;

an elongate, tubular body extending distally from the handle and having a proximal end, a distal end, and a central lumen extending from a proximal opening at the proximal end to a distal opening at the distal end, the central lumen configured to provide access to an anatomical structure;

a flexible tubular sleeve removably carried by the tubular body;

a first tube in communication with the lumen without obstruction of the proximal or distal openings;

a second tube in communication with the lumen without obstruction of the proximal or distal openings;

wherein the fluid control system is operable to manually control fluid flow through the first and second tubes by pinching at least one of the first tube and the second tube, the fluid control system moveable between at least three positions to—1) simultaneously close the first and second tubes, 2) close only the first tube, and 3) close only the second tube—without obstruction of the central lumen.
