Abstract

Embodiments presented herein describe techniques for administering a care plan that describes an overall treatment regimen for a patient. Embodiments include receiving a care plan specifying a plurality of assigned tasks for the patient to perform, timing information specifying when each of the plurality of assigned tasks should be performed, and a plurality of observation metrics that each indicate a type of biometric data to monitor. At least one monitoring device available to collect biometric data to compare against observation metrics specified in the care plan is identified. Biometric data is collected using the at least one monitoring device. Embodiments further include, upon determining that the collected biometric data satisfies at least one threshold condition specified in the care plan, initiating at least one treatment plan specified in the care plan and corresponding to the satisfied at least one threshold value.

Claims (101)

1. A method for administering a care plan for a patient, the method comprising:

receiving, at a care plan management system, a care plan specifying a plurality of assigned tasks for the patient to perform, timing information specifying when each of the plurality of assigned tasks should be performed, and a plurality of observation metrics that each indicate a type of biometric data to monitor;

identifying, using the care plan management system, one or more monitoring devices available to collect biometric data related to the patient to compare against observation metrics specified in the care plan;

selecting a first monitoring device from among the one or more monitoring devices, and associating the first monitoring device with the patient using an inventory management component of the care plan management system;

associating a mobile device with the care plan using the inventory management component, wherein the mobile device facilitates communication between the a first monitoring device and the care plan management system;

transmitting the care plan from the care plan management system to the mobile device;

configuring, using the mobile device and based on the care plan, the first monitoring device for use in collecting biometric data, the configuration comprising:

transmitting, from the mobile device to the first monitoring device, information related to at least one threshold condition specified in the care plan;

collecting biometric data using the first monitoring device;

upon determining, using the first monitoring device, that the collected biometric data satisfies the at least one threshold condition specified in the care plan, treating the patient by initiating at least one treatment plan specified in the care plan and corresponding to the satisfied at least one threshold condition, comprising:

altering a graphic user interface of the mobile device to ask the patient for information related to possible relevant symptoms according to the satisfied threshold condition, based on the care plan;

collecting, using the mobile device, symptom information describing one or more symptoms being experienced by the patient;

determining a first treatment action to perform in administering the treatment plan, based on the symptom information, the collected biometric data, and information relating to the patient's history of compliance with actions relating to administering the treatment plan; and

performing the first treatment action at the mobile device;

receiving the biometric data at the mobile device from the first monitoring device;

aggregating the biometric data at the mobile device, and transmitting the aggregated data from the mobile device to the care plan management system, wherein transmitting the aggregated data to the care plan management system consumes less bandwidth than transmitting the biometric data; and

modifying, at the care plan management system, the care plan, and in response automatically transmitting the modified care plan to the mobile device.

2. The method of claim 1 , wherein the determined first action comprises at least one of requesting the patient perform one or more tasks, transmitting educational content to the mobile device for review by the patient, and generating an alert message describing a current state of the patient.

The method of claim 1 , wherein the first action is further determined based on historical patient information for the patient, wherein the historical patient information specifies previous medical conditions the patient has been diagnosed with and medical treatments the patient has undergone, and further comprising:

determining that the treatment plan specifies that the first action be performed when the at least one threshold condition is satisfied by the collected biometric data; and

upon determining that the historical patient information specifies that the patient has previously tried the first action, specified by the treatment plan and responsive to a first set of symptoms, and that the first action did not alleviate the first set of symptoms when tried by the patient previously, selecting a second action to perform instead of the first action.

4. The method of claim 1 , wherein collecting biometric data corresponding the plurality of observation metrics using the at least one monitoring device further comprises:

generating the biometric data using one or more sensors of the at least one monitoring device; and

receiving the biometric data from the at least one monitoring device, at the care plan management system, wherein the biometric data is transmitted from the at least one monitoring device to the care plan management system through the mobile device.

5. The method of claim 4 , further comprising:

providing an interface through which a healthcare provider can view the received biometric data for the patient.

6. The method of claim 1 , wherein the care plan further specifies a plurality of phases, wherein only one of the plurality of phases is active at a time, wherein each of the plurality of phases comprises:

a respective two or more of the plurality of observational metrics;

a respective two or more of the plurality of assigned tasks; and

conditional logic that, when satisfied while the phase is active, indicates that another one of the plurality of phases should become active.

7. The method of claim 1 , further comprising:

providing an interface through which the patient can view the plurality of assigned tasks for the patient to perform;

determining whether the patient is performed the plurality of assigned tasks according to the timing information specified in the care plan; and

upon determining that the patient has not performed one of the plurality of assigned tasks within a window of time specified by the timing information, generating a reminder for the patient to perform the assigned task.

The method of claim 1 , wherein the care plan further specifies a plurality of threshold values each corresponding to a respective one of the plurality of observation metrics, and a respective treatment plan for each of the plurality of threshold values specifying one or more actions to take responsive to detecting monitored biometric data that satisfies the respective threshold value.

9. A non-transitory computer-readable medium containing computer program code that, when executed by a processor, performs an operation for administering a care plan for a patient, the operation comprising:

receiving, at a care plan management system, a care plan specifying a plurality of assigned tasks for the patient to perform, timing information specifying when each of the plurality of assigned tasks should be performed, and a plurality of observation metrics that each indicate a type of biometric data to monitor;

identifying, using the care plan management system, one or more monitoring devices available to collect biometric data related to the patient to compare against observation metrics specified in the care plan;

selecting a first monitoring device from among the one or more monitoring devices, and associating the first monitoring device with the patient using an inventory management component of the care plan management system;

associating a mobile device with the care plan using the inventory management component, wherein the mobile device facilitates communication between the a first monitoring device and the care plan management system;

transmitting the care plan from the care plan management system to the mobile device;

configuring, using the mobile device and based on the care plan, the first monitoring device for use in collecting biometric data, the configuration comprising:

transmitting, from the mobile device to the first monitoring device, information related to at least one threshold condition specified in the care plan;

collecting biometric data using the first monitoring device;

upon determining, using the first monitoring device, that the collected biometric data satisfies the at least one threshold condition specified in the care plan, treating the patient by initiating at least one treatment plan specified in the care plan and corresponding to the satisfied at least one threshold condition, comprising:

altering a graphic user interface of the mobile device to ask the patient for information related to possible relevant symptoms according to the satisfied threshold condition, based on the care plan;

collecting, using the mobile device, symptom information describing one or more symptoms being experienced by the patient;

determining a first treatment action to perform in administering the treatment plan, based on the symptom information, the collected biometric data, and information relating to the patient's history of compliance with actions relating to administering the treatment plan; and

performing the first treatment action at the mobile device;

receiving the biometric data at the mobile device from the first monitoring device;

aggregating the biometric data at the mobile device, and transmitting the aggregated data from the mobile device to the care plan management system, wherein transmitting the aggregated data to the care plan management system consumes less bandwidth than transmitting the biometric data; and

modifying, at the care plan management system, the care plan, and in response automatically transmitting the modified care plan to the mobile device.

The non-transitory computer-readable medium of claim 9 , wherein the determined first action comprises at least one of requesting the patient perform one or more tasks, transmitting educational content to the mobile device for review by the patient, and generating an alert message describing a current state of the patient.

11. The non-transitory computer-readable medium of claim 9 , wherein the first action is further determined based on historical patient information for the patient, wherein the historical patient information specifies previous medical conditions the patient has been diagnosed with and medical treatments the patient has undergone, and the operation further comprising:

determining that the treatment plan specifies that the first action be performed when the at least one threshold condition is satisfied by the collected biometric data; and

upon determining that the historical patient information specifies that the patient has previously tried the first action, specified by the treatment plan and responsive to a first set of symptoms, and that the first action did not alleviate the first set of symptoms when tried by the patient previously, selecting a second action to perform instead of the first action.

12. The non-transitory computer-readable medium of claim 9 , wherein collecting biometric data corresponding the plurality of observation metrics using the at least one monitoring device further comprises:

generating the biometric data using one or more sensors of the at least one monitoring device; and

receiving the biometric data from the at least one monitoring device, at the care plan management system, wherein the biometric data is transmitted from the at least one monitoring device to the care plan management system through the mobile device.

13. The non-transitory computer-readable medium of claim 9 , wherein the care plan further specifies a plurality of phases, wherein only one of the plurality of phases is active at a time, wherein each of the plurality of phases comprises:

a respective two or more of the plurality of observational metrics;

a respective two or more of the plurality of assigned tasks; and

conditional logic that, when satisfied while the phase is active, indicates that another one of the plurality of phases should become active.

14. A system for administering a care plan for a patient, comprising:

a first monitoring device to collect biometric data to compare against observation metrics specified in the care plan;

a care plan management system; and

a mobile device that facilitates communication between the first monitoring device and the care plan management system,

wherein the care plan management system is configured to:

receive a care plan specifying a plurality of assigned tasks for the patient to perform, timing information specifying when each of the plurality of assigned tasks should be performed, and a plurality of observation metrics that each indicate a type of biometric data to monitor;

identify one or more monitoring devices available to collect biometric data related to the patient to compare against observation metrics specified in the care plan, and select the first monitoring device from among the one or more monitoring devices;

associate the first monitoring device with the patient using an inventory management component of the care plan management system;

associate the mobile device with the care plan using the inventory management component of the care plan management system;

transmit the care plan from the care plan management system to the mobile device;

modify the care plan, and in response automatically transmit the modified care plan to the mobile device; and

configure, using the mobile device and based on the care plan, the first monitoring device for use in collecting biometric data, the configuration comprising:

transmitting, from the mobile device to the first monitoring device, information related to at least one threshold condition specified in the care plan;

wherein the mobile device is configured to:

receive biometric data collected using the first monitoring device;

upon determining, using the first monitoring device, that the collected biometric data satisfies the at least one threshold condition specified in the care plan, treat the patient by initiating at least one treatment plan specified in the care plan and corresponding to the satisfied at least one threshold condition, comprising:

altering a graphic user interface of the mobile device to ask the patient for information related to possible relevant symptoms according to the satisfied threshold condition, based on the care plan;

collecting, using the mobile device, symptom information describing one or more symptoms being experienced by the patient;

determining a first treatment action to perform in administering the treatment plan, based on the symptom information, the collected biometric data, and information relating to the patient's history of compliance with actions relating to administering the treatment plan; and

performing the first treatment action at the mobile device; and

aggregate the received biometric data, and transmit the aggregated data from the mobile device to the care plan management system, wherein transmitting the aggregated data to the care plan management system consumes less bandwidth than transmitting the biometric data.

15. The method of claim 1 , further comprising:

receiving the modified care plan at the mobile device, and in response transmitting an electronic message from the mobile device to the first monitoring device updating the configuration of the first monitoring device.

16. The method of claim 1 , further comprising:

performing an initial classification of the biometric data, at the first monitoring device, to identify a patient event, wherein the initiating the at least one treatment plan is based on the initial classification of the patient event; and

re-classifying the patient event, at the care plan management system, based on the aggregated biometric data,

wherein the care plan management system comprises more computational resources than the first monitoring device,

wherein the re-classifying is more computationally expensive than the initial classification, and

wherein the modifying the care plan is based on the re-classified patient event.

17. The method of claim 16 , wherein the re-classified event is a sub-classification of the initially classified event.

18. The system of claim 14 , wherein the mobile device is further configured to

receive the modified care plan at the mobile device, and in response transmit an electronic message from the mobile device to the first monitoring device updating the configuration of the first monitoring device.

19. The system of claim 14 , wherein the first monitoring device is configured to perform an initial classification of the biometric data to identify a patient event, wherein the initiating the at least one treatment plan is based on the initial classification of the patient event; and

wherein the care plan management system is further configured to re-classify the patient event based on the aggregated biometric data,

wherein the care plan management system comprises more computational resources than the first monitoring device,

wherein the re-classifying is more computationally expensive than the initial classification, and

wherein the modifying the care plan is based on the re-classified patient event.

20. The system of claim 19 , wherein the re-classified event is a sub-classification of the initially classified event.
