Abstract

An apparatus treats tissue at or near a sphincter. The apparatus has an elongated member having at least one lumen including an inflation lumen, and a basket assembly including a first and a second arm. An inflatable member is coupled to the inflation lumen and has a deployed and a non-deployed state. In the deployed state, the inflatable member expands the basket assembly into contact with tissue. At least one of the first and second arms of the basket assembly has a fluid lumen having an aperture for conveying a fluid from the basket assembly. A source of fluid is coupled to the fluid lumen for conveyance of fluid through the aperture. The fluid can be, e.g., an electrolytic solution, and/or an anti-infection agent, and/or an echogenic media, and/or a steroid, and/or an anesthetic, and/or a medicament, and/or a tissue cooling agent. The source can be a drug delivery device.

Claims (27)

1. An apparatus for treating gastroesophageal reflux disease by treating tissue at or near a lower esophageal sphincter, the apparatus comprising a shaft having at least one lumen and an expandable device coupled to the elongated member, the expandable device including a basket assembly having first and second arms, the first arm having a first lumen and a second lumen, the first lumen configured to convey a fluid therethrough, and a first electrode movable within the second lumen of the first arm from a retracted position to an extended position extending radially outwardly from the arm, the first electrode is adapted to apply electrosurgical energy to the tissue to tighten the sphincter to treat gastroesophageal reflux disease by reducing frequency of reflux of stomach contents into an esophagus, and a temperature sensor, the electrode couplable to an energy delivery device and the apparatus is couplable to a feedback system which couples the temperature sensor to the energy delivery device to control temperature of the tissue.

2. The apparatus of claim 1 , wherein the first and second arms have an external layer of texturized material having a sufficient coefficient of friction to at least partially immobilize an area of tissue contacted by the arms.

3. The apparatus of claim 1 , wherein the first and second lumens are formed by multiple tubes.

4. The apparatus of claim 3 , wherein the first and second arms in the expanded position have a bow shape.

5. The apparatus of claim 4 , wherein the first and second arms converge at a proximal end and at a distal end.

6. The apparatus of claim 1 , in combination with the feedback system, the feedback system responsive to impedance measurement and temperature measurement.

7. The apparatus of claim 6 , wherein the second arm has a first lumen and a second lumen and a second electrode is movable within the second lumen of the second arm of the basket assembly, and the feedback system enables selective energy delivery to the first and second electrodes.

8. The apparatus of claim 6 , wherein output power of the energy delivery device is adjusted in accordance with monitoring of temperature of the tissue.

9. The apparatus of claim 6 , wherein temperature at the sensor provides a safety to interrupt delivery of energy to the electrode when a maximum pre-set temperature is exceeded.

10. The apparatus of claim 6 , further comprising a comparator which sends a signal to a flow regulator if tissue temperature is too high to increase flow rate of cooling solution through the second lumen of the first arm.

11. The apparatus of claim 1 , wherein the electrode has an insulating material on an exterior surface.

12. The apparatus of claim 1 , wherein the second lumen has an aperture through which the electrode is advanced, and the fluid is advanced through the aperture in which the electrodes are advanced.

13. The apparatus of claim 1 , wherein the first and second arms each have separate apertures for introduction of the fluid.

14. The apparatus of claim 1 , wherein the electrode is adapted to apply radiofrequency energy.

15. The apparatus of claim 1 , wherein the shaft receives a viewing device.

16. The apparatus of claim 1 , wherein the electrode is hollow to introduce fluid therethrough.

17. The apparatus of claim 1 , wherein expansion of the first and second arms dilates tissue.

18. A method for treating gastroesophageal reflux disease by treating tissue at or near a lower esophageal sphincter to tighten the sphincter, the method comprising:

providing an apparatus comprising an elongated member having at least one lumen and an expansion device coupled to the elongated member;

inserting the apparatus with the expansion device in a non-expanded position;

expanding the expansion device to an expanded position;

advancing a first electrode through a first lumen of a first arm of the expansion device so the first electrode extends radially outwardly to apply energy to the tissue to form lesions to tighten the sphincter to reduce frequency of reflux of stomach contents into an esophagus;

conveying a fluid through a second lumen of the first arm of the expansion device to cool the tissue to regulate temperature of the tissue;

measuring tissue temperature of the tissue to regulate fluid flow to regulate temperature of the tissue; and

retracting the electrode and moving the expansion device back to the non-expanded position for removal of the apparatus.

The method of claim 18 , further comprising the steps of advancing a second electrode through a first lumen of a second arm of the expansion device so the second electrode extends radially outwardly to apply energy to the tissue to form lesions to tighten the sphincter to reduce frequency of reflux of stomach contents into an esophagus, and conveying a fluid through a second lumen of the second arm of the expansion device to cool the tissue to regulate temperature of tissue.

20. The method of claim 19 , further comprising the step of individually applying energy to the first and second electrodes to separately control energy delivery of the electrodes.
