Abstract

Described here are systems and methods for accessing Schlemm's canal and for delivering an ocular device or fluid composition therein. The ocular devices may maintain the patency of Schlemm's canal without substantially interfering with transmural fluid flow across the canal. The fluid composition may be a viscoelastic fluid that is delivered into the canal to facilitate drainage of aqueous humor by disrupting the canal and surrounding trabeculocanalicular tissues. Tools for disrupting these tissues and minimally invasive methods for treating medical conditions associated with elevated intraocular pressure, including glaucoma, are also described.

Claims (26)

1. An ocular delivery device comprising:

a) a universal handle;

b) a cannula coupled to the universal handle and comprising an opening at a distal end;

c) a flexible polymer slidably disposed within and extendable from the distal opening, the flexible polymer having a length configured to advance 180 degrees around Schlemm's canal, wherein the length of the portion of the flexible polymer extended from the distal opening is configured to cut trabeculocanalicular tissues during withdrawal of the cannula;

d) a housing; and

e) a wheeled drive assembly,

where the ocular delivery device is implant-free.

2. The ocular delivery device of claim 1 , wherein the wheeled drive assembly comprises a single rotatable component.

3. The ocular delivery device of claim 1 , wherein the wheeled drive assembly comprises a plurality of rotatable components.

4. The ocular delivery device of claim 1 , wherein the flexible polymer has an outer diameter of between about 50 microns and about 500 microns.

5. The ocular delivery device of claim 4 , wherein the flexible polymer has an outer diameter of about 240 microns.

6. A single-handed, single operator controlled device for lowering intraocular pressure comprising:

a) a cannula;

b) a flexible polymer slidably disposed within, and extendable distally from, the cannula, and the flexible polymer having a length configured to advance 180 degrees around Schlemm's canal, wherein the length of the portion of the flexible polymer extended from the cannula is configured to cut trabeculocanalicular tissues during withdrawal of the cannula; and

c) a handle coupled to the cannula,

wherein the handle is configured to extend and retract the flexible polymer with a single hand, and wherein the device is implant-free.

7. The single-handed, single-operator controlled device of claim 6 , wherein the flexible polymer has an outer diameter between about 50 microns and about 500 microns.

8. The single-handed, single-operator controlled device of claim 7 , wherein the flexible polymer has an outer diameter of about 240 microns.

9. The single-handed, single-operator controlled device of claim 6 , further comprising a wheeled drive assembly.

10. The single-handed, single-operator controlled device of claim 9 , wherein the wheeled drive assembly comprises a single rotatable component.

11. The single-handed, single-operator controlled device of claim 9 , wherein the wheeled drive assembly comprises a plurality of rotatable components.

12. The ocular delivery device of claim 1 , wherein the flexible polymer is configured to extend about 20 mm from the distal opening.

13. The ocular delivery device of claim 12 , wherein the flexible polymer is configured to extend about 36 to 40 mm from the distal opening.

14. The single-handed, single-operator controlled device of claim 6 , further comprising a grasping element.

15. The single-handed, single-operator controlled device of claim 6 , wherein the flexible polymer is configured to advance about a 180 degree arc into Schlemm's canal.

16. The single-handed, single-operator controlled device of claim 6 , wherein the flexible polymer is configured to advance about 360 degrees into Schlemm's canal.
