Abstract

An undercutting system for preparing a region between an ilium and a sacrum for sacroiliac fusion. The undercutting system includes an insertion apparatus and a cutting assembly. The cutting assembly is operably mounted with respect to the insertion apparatus. The cutting assembly has a distal end and a proximal end. The cutting assembly includes a plurality of cutting elements and a cutting element attachment mechanism. The cutting element attachment mechanism engages the cutting elements to operably mount the cutting elements with respect to the insertion apparatus. The cutting element attachment mechanism permits the cutting elements to pivot with respect to each other.

Claims (35)

1. An undercutting system for preparing a region between an ilium and a sacrum for sacroiliac fusion, wherein the undercutting system comprises:

an insertion apparatus; and

a cutting assembly operably mounted with respect to the insertion apparatus, wherein the cutting assembly comprises a distal end and a proximal end and wherein the cutting assembly comprises:

a plurality of cutting elements, wherein a portion of the cutting elements proximate the distal end of the cutting assembly have a smaller height than a portion of the cutting elements proximate the proximal end of the cutting assembly; and

a cutting element attachment mechanism that engages the cutting elements to operably mount the cutting elements with respect to the insertion apparatus, wherein the cutting element attachment mechanism permits the cutting elements to pivot with respect to each other.

2. The undercutting system of claim 1 , wherein the plurality of cutting elements each comprise:

a central section; and

a pair of wing sections extending from opposite sides of the central section, wherein the central section has a height that is smaller than a height of the wing sections.

3. The undercutting system of claim 1 , wherein the cutting elements each comprise:

a distal surface that is oriented towards the distal end of the cutting assembly; and

a proximal surface that is oriented towards the proximal end of the cutting assembly, wherein at least one of the distal surface and the proximal surface has a convex shape.

The undercutting system of claim 1 , wherein each of the cutting elements has an aperture formed therein, wherein the cutting element attachment mechanism extends through the apertures to pivotally mount the cutting elements to each other and wherein the cutting element attachment mechanism is flexible.

5. The undercutting system of claim 1 , and further comprising

an advancement mechanism that causes the cutting assembly to be extended from the insertion apparatus as the insertion apparatus is rotated; and

a powered mechanism for causing the cutting assembly to rotate with respect to the insertion apparatus.

The undercutting system of claim 1 , wherein the cutting assembly is slidable with respect to the insertion apparatus between a retracted position where the cutting assembly is substantially within the insertion apparatus and an extended position where at least a portion of the cutting assembly extends from the insertion apparatus.

7. An undercutting system for preparing a region between an ilium and a sacrum for sacroiliac fusion, wherein the undercutting system comprises:

an insertion apparatus; and

a cutting assembly operably mounted with respect to the insertion apparatus, wherein the cutting assembly comprises a distal end and a proximal end and wherein the cutting assembly comprises:

an elongated base fabricated from a flexible material; and

a plurality of cutting elements that are movable with respect to the elongated base, wherein the cutting elements are configured to cut on at least one of moving towards the distal end or the proximal end of the cutting assembly, wherein the cutting elements each comprises:

a distal end and a proximal end, wherein the proximal ends of the cutting elements are each oriented towards a distal end of the cutting assembly;

a cutting surface proximate the distal end of each cutting element; and

a pivot element proximate the proximal end of each cutting element, wherein the cutting surface is located on an opposite side of the elongated base than the pivot element.

The undercutting system of claim 7 , wherein the cutting elements are pivotable between a retracted position and an extended position, wherein when in the retracted position, the cutting elements are substantially aligned with a surface of the elongated base and wherein a width of the cutting elements is no larger than a width of the elongated base.

9. The undercutting system of claim 7 , wherein the elongated base comprises two surfaces on opposite sides thereof and wherein at least one of the cutting elements is pivotally, deformably or directionally mounted on or integrated into each of the surfaces.

10. The undercutting system of claim 7 , wherein at least a portion of each of the cutting elements has an abrasive surface or a plurality of bristles extending therefrom.

11. An undercutting system for preparing a region between an ilium and a sacrum for sacroiliac fusion, wherein the undercutting system comprises:

an insertion apparatus; and

a cutting assembly that is movable with respect to the insertion apparatus between a retracted configuration and an extended configuration, wherein in the extended configuration at least a portion of the cutting assembly extends from the insertion apparatus, wherein the cutting assembly has a plurality of kerfs formed therein, wherein the kerfs are oriented at an angle with respect to an upper surface of the cutting assembly and wherein each of the kerfs has a distal end and a proximal end and has a greater width proximate the distal end than proximate the proximal end.

12. The undercutting system of claim 11 , wherein the angle is between about 20 degrees and about 70 degrees and.

The undercutting system of claim 11 , wherein the cutting assembly comprises an upper wall, a lower wall and two side walls that extend between the upper wall and the lower wall proximate opposite ends thereof to thereby define an opening therebetween and wherein the kerf extends through the upper wall and at least a portion of the side walls.

14. The undercutting system of claim 11 , and further comprising:

at least one cutting element mounted to the cutting assembly, wherein at least a portion of the cutting elements has an abrasive surface or a plurality of bristles extending therefrom; and

a probe assembly that extends through at least a portion of the cutting assembly and wherein the probe assembly is slidably mounted with respect to the cutting assembly.
