Abstract

A method of providing a sleep apnea nerve stimulation therapy to a subject may include detecting a respiratory waveform of the subject with a sensor. The sensor may be configured for coupling to the subject. The respiratory waveform may include a plurality of respiratory cycles each corresponding to at least one of a breath and an attempted breath of the subject. The method may also include identifying a breathing pattern within the respiratory waveform over a period of time. The breathing pattern may include a repeating pattern of a plurality of respiratory cycles followed by at least one respiratory cycle corresponding to a disordered breathing event. The method may also include generating a series of stimulation pulses with an implantable nerve stimulator configured for coupling to a hypoglossal nerve of the subject. The series of stimulation pulses may be coordinated with the breathing pattern.

Claims (14)

1. A method of providing a sleep apnea nerve stimulation therapy to a subject, the method comprising:

detecting a respiratory waveform of the subject with a sensor configured for coupling to the subject, the respiratory waveform including a plurality of respiratory cycles each corresponding to at least one of a breath or an attempted breath of the subject;

identifying a breathing pattern within the respiratory waveform over a period of time, the breathing pattern comprising a repeating pattern of a plurality of respiratory cycles followed by at least one respiratory cycle corresponding to a disordered breathing event;

comparing an inspiratory duration of an inspiratory period of the at least one respiratory cycle to a total duration of the at least one respiratory cycle;

determining, based on the comparison between the inspiratory duration and the total duration, that the inspiratory duration exceeds a threshold percentage of the total duration to support an identification of the disordered breathing event; and

upon determining that the inspiratory duration exceeds the threshold percentage of the total duration, generating a series of stimulation pulses based on the breathing pattern with an implantable nerve stimulator configured for coupling to a hypoglossal nerve of the subject, the series of stimulation pulses coordinated with the breathing pattern.

The method of claim 1 wherein the generating of the series of stimulation pulses coordinated with the breathing pattern includes the generating of a first stimulation pulse during the plurality of respiratory cycles and the generating of a second stimulation pulse during the at least one respiratory cycle corresponding to the disordered breathing event.

3. The method of claim 2 wherein the first stimulation pulse is at a first amplitude and the second stimulation pulse is at a second amplitude that is greater than the first amplitude.

4. The method of claim 1 further comprising:

receiving a command to initiate the nerve stimulation therapy and commencing an initial period when no stimulation is provided so as to allow the subject to fall asleep.

5. The method of claim 1 wherein the breathing pattern is determined from a detection of at least one of a signal peak, a signal minimum, an expiration detection, or an inspiration detection.

The method of claim 1 wherein the breathing pattern includes a trend defined by at least one of the plurality of respiratory cycles, a plurality of peak magnitudes of the plurality of respiratory cycles, a plurality of minima magnitudes of the plurality of respiratory cycles, a plurality of expiration detections of the plurality of respiratory cycles, or a plurality of inspiration detections of the plurality of respiratory cycles.

The method of claim 1 , wherein comparing the portions of the breathing pattern within the respiratory waveform to support the identification of the disordered breathing event comprises comparing a first inspiratory period to a subsequent second inspiratory period to determine that the second inspiratory period indicates the disordered breathing event.

8. The method of claim 7 , wherein the determination that the second inspiratory period indicates the disordered breathing event is based on the second inspiratory period being longer than the first inspiratory period.
