Abstract

A method for extinguishing a cardiac arrhythmia utilizes destructive interference of the passing of the reentry wave tip of an anatomical reentry through a depolarized region created by a relatively low voltage electric field in such a way as to effectively unpin the anatomical reentry. Preferably, the relatively low voltage electric field is defined by at least one unpinning shock(s) that are lower than an expected lower limit of vulnerability as established, for example, by a defibrillation threshold test. By understanding the physics of the electric field distribution between cardiac cells, the method permits the delivery of an electric field sufficient to unpin the core of the anatomical reentry, whether the precise or estimated location of the reentry is known or unknown and without the risk of inducting ventricular fibrillation. A number of embodiments for performing the method are disclosed.

Claims (35)

1. A method for delivering electrical stimulation to a heart of a patient, the method comprising:

sensing at least one biological signal corresponding to electrical activity of the heart;

analyzing the at least one biological signal to identify irregular electrical activity in a ventricle of the heart; and

in response to identifying irregular electrical activity in the ventricle of the heart, delivering a plurality of stages of electrical stimulation to the heart, wherein each stage of the plurality of stages of electrical stimulation is separated from a subsequent stage or a prior stage by a pre-determined inter-stage delay, the plurality of stages of electrical stimulation includes at least a first stage and a second stage, the first stage includes a first plurality of pulses and the second stage includes a second plurality of pulses, wherein the first plurality of pulses generates a voltage gradient within the heart of the patient of less than 4.5V/cm and more than 0.25 V/cm.

2. The method of claim 1 , wherein the identified irregular electrical activity is ventricular tachyarrhythmia.

3. The method of claim 1 , wherein the identified irregular electrical activity is ventricular tachycardia.

4. The method of claim 1 , wherein, during the delivery of the plurality of stages of electrical stimulation, electrical activity of the ventricle is not evaluated.

5. The method of claim 1 , wherein at least one of the plurality of stages of electrical stimulation is delivered to the heart via a far-field electrode configuration.

The method of claim 1 , wherein the irregular electrical activity is a tachyarrhythmia, and wherein analyzing the at least one biological parameter signal further includes determining a termination window for delivery of the first stage of electrical stimulation based on a cycle length of the tachyarrhythmia.

7. The method of claim 1 , wherein the first stage includes a plurality of pulses delivered as a function of a cycle length of the arrhythmia.

8. The method of claim 1 , wherein the first stage includes a plurality of pulses delivered over a period of about 100-150 ms.

9. The method of claim 8 , wherein a duration of the plurality of pulses of the first stages is about 10 ms and a number of the plurality of pulses of the first stage is 3-4.

10. A method for delivering electrical stimulation to a heart of a patient, the method comprising:

sensing at least one biological signal corresponding to electrical activity of the heart;

analyzing the at least one biological signal to identify irregular electrical activity in a ventricle of the heart; and

in response to identifying irregular electrical activity in the ventricle of the heart, delivering a plurality of stages of electrical stimulation to the heart, wherein each stage of the plurality of stages of electrical stimulation is separated from a subsequent stage or a prior stage by a pre-determined inter-stage delay, the plurality of stages of electrical stimulation includes at least a first stage and a second stage, the first stage includes a first plurality of pulses and the second stage includes a second plurality of pulses, and wherein the plurality of stages of electrical stimulation are delivered without confirmation of conversion of the detected ventricular arrhythmia during delivery of the plurality of stages of electrical stimulation.

11. The method of claim 10 , wherein the identified irregular electrical activity is ventricular tachyarrhythmia.

12. The method of claim 10 , wherein the identified irregular electrical activity is ventricular tachycardia.

13. The method of claim 10 , wherein at least one of the plurality of stages of electrical stimulation is delivered to the heart via a far-field electrode configuration.

14. The method of claim 10 , wherein the irregular electrical activity is a tachyarrhythmia, and wherein analyzing the at least one biological parameter signal further includes determining a termination window based on a cycle length of the tachyarrhythmia.

15. The method of claim 8 , wherein the first stage includes a plurality of pulses delivered as a function of the cycle length of the arrhythmia.

16. The method of claim 8 , wherein the first stage includes a plurality of pulses delivered over a period of about 100-150 ms.

17. The method of claim 16 , wherein a duration of the plurality of pulses of the first stages is about 10 ms and a number of the plurality of pulses of the first stage is 3-4.

18. A method for delivering electrical stimulation to a heart of a patient, the method comprising:

implanting an electrical stimulator within the patient, wherein the electrical stimulator is configured to be coupled to a plurality of electrodes positioned proximate the heart, the implantable electrical stimulator comprising:

detection circuitry configured to evaluate at least one biological signal to detect a ventricular arrhythmia, wherein the at least one biological signal corresponds to electrical activity of the heart; and

control circuitry configured to deliver a plurality of stages of electrical stimulation to the heart in response to the detected ventricular arrhythmia, wherein each stage of the plurality of stages of electrical stimulation is separated from a subsequent stage or a prior stage by a pre-determined inter-stage delay, the plurality of stages of electrical stimulation includes at least a first stage and a second stage, the first stage includes a first plurality of pulses and the second stage includes a second plurality of pulses, wherein the first plurality of pulses generates a voltage gradient within the heart of the patient of less than 4.5V/cm and more than 0.25 V/cm.

19. A method for delivering electrical stimulation to a heart of a patient, the method comprising:

implanting an electrical stimulator within the patient, wherein the electrical stimulator is configured to be coupled to a plurality of electrodes positioned proximate the heart, the implantable electrical stimulator comprising:

detection circuitry configured to evaluate at least one biological signal to detect a ventricular arrhythmia, wherein the at least one biological signal corresponds to electrical activity of the heart; and

control circuitry configured to deliver a plurality of stages of electrical stimulation to the heart in response to the detected ventricular arrhythmia, wherein each stage of the plurality of stages of electrical stimulation is separated from a subsequent stage or a prior stage by a pre-determined inter-stage delay, the plurality of stages of electrical stimulation includes at least a first stage and a second stage, the first stage includes a first plurality of pulses and the second stage includes a second plurality of pulses, and wherein the plurality of stages of electrical stimulation are delivered without confirmation of conversion of the detected ventricular arrhythmia during delivery of the plurality of stages of electrical stimulation.

20. A method for delivering electrical stimulation to a heart of a patient, the method comprising:

implanting an electrical stimulator within the patient, wherein the electrical stimulator is configured to be coupled to a plurality of electrodes positioned proximate the heart, the implantable electrical stimulator comprising:

detection circuitry configured to evaluate at least one biological signal to detect a ventricular tachyarrhythmia, wherein the at least one biological signal corresponds to electrical activity of the heart; and

control circuitry configured to deliver a plurality of stages of electrical stimulation to the heart in response to the detected ventricular arrhythmia, wherein each stage of the plurality of stages of electrical stimulation is separated from a subsequent stage or a prior stage by a pre-determined inter-stage delay, the plurality of stages of electrical stimulation includes at least a first stage and a second stage, the first stage includes 3-4 pulses delivered over a period of about 100-150 ms, each pulse having a duration of about 10 ms, and the second stage includes a second plurality of pulses, and wherein the plurality of stages of electrical stimulation are delivered without confirmation of conversion of the detected ventricular tachyarrhythmia during delivery of the plurality of stages of electrical stimulation.
