Abstract

An implantable neurostimulator system for treating pain includes scheduled and responsive therapy capabilities including responsive stimulation applied to the brain and peripheral sections of the nervous system. Methods for treating chronic nociceptive, neuropathic, and psychogenic pain employ an inventive system to advantageously reduce multiple symptoms and components of pain and to address underlying causes of pain.

Claims (37)

1. An implantable system for treating pain in a patient, the system comprising:

a first module configured to be implanted in the patient at or near a first detection location and to identify a first pain signature based at least in part on signals sensed at or near the first detection location;

a second module configured to be implanted in the patient at or near a second detection location remote from the first detection location, and configured to at least one of:

obtain information corresponding to one or more of signals sensed at or near the second detection location, and

identify a second pain signature based on signals sensed at or near the second detection location;

wherein the first module is further configured to:

confirm a pain detection based on the first pain signature and at least one of the obtained information corresponding to one or more of signals sensed at or near the second detection location, and the second pain signature; and

initiate an application of a therapy in response to confirmation of a pain detection.

2. The system of claim 1 , further comprising a therapy subsystem configured to be implanted in the patient and to apply the therapy initiated by the first module.

3. The system of claim 2 , wherein the therapy subsystem is independent of both the first module and the second module.

4. The system of claim 2 , wherein the therapy subsystem is included in one of the first module and the second module.

5. The system of claim 2 , further comprising a therapy output configured to be implanted at or near a first therapy location in the patient and coupled to the therapy subsystem, the therapy output configured to deliver the therapy initiated by the first module.

6. The system of claim 5 , wherein the therapy output comprises an electrical stimulation electrode, a magnetic stimulator, a transcranial magnetic stimulator, an optical stimulator, a catheter coupled to a drug dispenser, or a thermal conductor coupled to a thermal stimulator.

7. The system of claim 1 , wherein the first module is configured to confirm a pain detection based on both the signals sensed at or near the first detection location and the parameters of signals sensed at or near the second detection location.

8. The system of claim 1 , wherein the first module is configured to confirm a pain detection when both of a first pain signature and a second pain signature are identified.

9. The system of claim 1 , wherein the first module is configured to confirm a pain signature when at least one of a first pain signature or a second pain signature is identified.

10. The system of claim 1 , wherein each of the first pain signature and the second pain signature comprise at least one of an electrical burst, an electrographic pattern indicative of activation, an electrographic pattern indicative of inhibition, and an oscillating signal.

11. The system of claim 1 , wherein the first module processes signals sensed at or near the first detection location to identify a first pain signature by being further configured to:

obtain a measure of spectral energy in the sensed signals; and

compare the measure to a criterion indicative of the first pain signature.

12. The system of claim 1 , wherein the second module processes signals sensed at or near the second detection location to identify a second pain signature by being further configured to:

obtain a measure of spectral energy in the sensed signals; and

compare the measure to a criterion indicative of the second pain signature.

13. The system of claim 1 , wherein the first detection location comprises the patient's brain.

14. The system of claim 13 , wherein the second location comprises one of the spinal column of the patient, a proximal end of a peripheral nerve of the patient, and a distal end of a peripheral nerve of the patient.

The system of claim 1 , wherein the second module is configured to switch between a sleep mode during which the second module does not obtain information or identify a second pain signature, and an awake mode during which the second module does obtains information or identifies a second pain signature.

16. The system of claim 15 , wherein each of the first module and the second module comprises a communications subsystem configured to provide a wireless communication link between the first module and the second module.

17. The system of claim 16 , wherein:

the first module is configured to output an activation signal to the second module over the wireless communication link when the first module identifies a first pain signature; and

the second module is configured to enter the awake mode upon receipt of the activation signal.

18. The system of claim 16 , wherein the second module is configured to periodically enter the awake mode and communicate with the first module over the wireless communication link to determine if a first pain signature has been identified.

19. A method of treating pain in a patient, said method comprising:

identifying, by a first module implanted in the patient at or near a first detection location, a first pain signature based at least in part on signals sensed at or near the first detection location;

obtaining, by a second module implanted in the patient at or near a second detection location remote from the first detection location, information corresponding to one or more of signals sensed at or near the second detection location;

identifying, by the second module, a second pain signature based on signals sensed at or near the second detection location;

confirming, by the first module, a pain detection based on the first pain signature and at least one of the obtained information corresponding to one or more of signals sensed at or near the second detection location, and the second pain signature; and

initiating, by the first module, an application of a therapy in response to confirmation of a pain detection.
