Abstract

Methods and devices that displace bone or other hard tissue to create a cavity in the tissue. Where such methods and devices rely on a driving mechanism for providing moving of the device to form a profile that improves displacement of the tissue. These methods and devices also allow for creating a path or cavity in bone for insertion of bone cement or other filler to treat a fracture or other condition in the bone. The features relating to the methods and devices described herein can be applied in any region of bone or hard tissue where the tissue or bone is displaced to define a bore or cavity instead of being extracted from the body such as during a drilling or ablation procedure.

Claims (22)

1. A medical device for ablation of tissue within a bone of a patient, the medical device comprising:

a proximal hub that is configured for electrically coupling the medical device to a radiofrequency generator;

a sleeve extending distally from the proximal hub, wherein the sleeve comprises a first electrode that is electrically coupled to a first pole of the radiofrequency generator when the radiofrequency generator is electrically coupled to the medical device;

an actuator that, upon actuation, is configured to cause articulation of the sleeve such that the sleeve forms a curved shape, wherein the sleeve is configured to remain in the curved shape upon application of a driving force or a rotational force on the proximal hub such that the curved shape creates or expands a cavity in the bone of the patient; and

an elongate shaft that is at least partially disposed within a lumen of the sleeve, wherein the elongate shaft is configured to transition from a retracted configuration to an extended configuration;

wherein a distal portion of the elongate shaft comprises:

a second electrode that is electrically coupled to a second pole of the radiofrequency generator when the radiofrequency generator is electrically coupled to the medical device; and

an axially extending insulator that is disposed alongside the second electrode.

The medical device of claim 1 , further comprising a second actuator, wherein actuation of the second actuator when a distal portion of the medical device is positioned within bone of the patient causes current to flow between the second electrode and the first electrode via tissue in the bone of the patient.

3. The medical device of claim 1 , wherein, when the elongate shaft is in the retracted configuration and the sleeve is in the curved shape, the axially extending insulator is disposed adjacent a concave side of the curved shape.

4. The medical device of claim 1 , wherein the insulator is positioned such that, when the medical device is activated for ablation, current is preferentially directed towards a side of the device that is opposite the insulator.

5. The medical device of claim 1 , wherein the sleeve comprises a plurality of notches, wherein the plurality of notches are disposed on a concave side of the sleeve when the sleeve is in the curved shape.

6. The medical device of claim 1 , wherein the sleeve comprises an axially extending insulator disposed adjacent a distal end of the sleeve, wherein the insulator does not extend around a circumference of the sleeve.

7. The medical device of claim 1 , wherein the lumen of the sleeve extends through a port at a distal end of the sleeve.

8. The medical device of claim 1 , wherein the first electrode is separated from the second electrode by an insulator collar.

9. The medical device of claim 1 , further comprising a pointed tip at a distal end of the sleeve, wherein the pointed tip is configured for the penetration of bone.

10. A medical device for ablation of tissue within a bone of patient, the medical device comprising:

a hub that is configured for electrically coupling the medical device to a radiofrequency generator;

an elongate shaft comprising a first electrode and a second electrode that is spatially separated from the first electrode, wherein, when the medical device is coupled to the radiofrequency generator, the first electrode is electrically coupled to a first pole of the radiofrequency generator and the second electrode is electrically coupled to a second pole of the radiofrequency generator;

an actuator that, upon actuation, is configured to cause articulation of the elongate shaft such that the elongate shaft forms a curved shape, wherein the elongate shaft is configured to remain in the curved shape upon application of a driving force or a rotational force on the hub such that the curved shape creates or expands a cavity in the bone of the patient; and

an elongate lumen that extends longitudinally through the elongate shaft, wherein the lumen is in fluid communication with one or more distal side openings that facilitate preferential delivery of flowable media to one side of the elongate shaft.

11. The medical device of claim 10 , wherein the first electrode is axially displaceable relative to the second electrode.
