Abstract

A method for performing a total joint arthroplasty procedure on a patient's damaged bone region. A CT image or other suitable image is formed of the damaged bone surfaces, and location coordinate values (x n ,y n ,z n ) are determined for a selected sequence of bone surface locations using the CT image data. A mathematical model z=f(x,y) of a surface that accurately matches the bone surface coordinates at the selected bone spice locations, or matches surface normal vector components at selected bone surface locations, is determined. The model provides a production file from which a cutting jig and an implant device (optional), each patient-specific and having controllable alignment, are fabricated for the damaged bone by automated processing.

Claims (44)

1. A method of manufacturing a custom arthroplasty jig for performing an arthroplasty procedure on a bone of a patient, wherein an implant is implanted on the bone of the patient as part of the arthroplasty procedure, the method comprising:

a) receive medical images of the bone including a damaged region, the medical images having been generated by a medical imaging machine;

b) generate a computer model of the bone from the medical images, the computer model of the bone comprising a virtual surface profile of the damaged region of the bone;

c) generate a preoperative plan using the computer model of the bone generated from the medical images and a computer model of the implant,

wherein the preoperative plan is a result of using the computer model of the implant with the computer model of the bone to preoperatively plan proposed alignment conditions, which includes superimposing the computer model of the implant and the computer model of the bone, the proposed alignment conditions reflecting an alignment of the implant relative to the bone of the patient at completion of the arthroplasty procedure,

wherein generating the preoperative plan includes defining a region of a virtual jig to be recessed according to a shape of the virtual surface profile of the damaged region of the bone of the computer model;

d) output the proposed alignment conditions to a surgeon for assessment;

e) generate a production file based on assessed alignment conditions and the virtual jig, the assessed alignment conditions reflecting surgeon assessment of the proposed alignment conditions; and

f) produce the custom arthroplasty jig via a rapid production machine according to the production file, the custom arthroplasty jig comprising a custom mating surface corresponding to the region of the virtual jig, the custom mating surface including a curvilinear surface profile configured to matingly receive the damaged region of bone of the patient prior to resection of the bone in a single relative position.

2. The method of claim 1 , wherein the assessed alignment conditions reflecting surgeon assessment of the proposed alignment conditions includes adjustment to the proposed alignment conditions.

3. The method of claim 1 , wherein the computer model of the bone includes at least one of a three dimensional computer model of the bone or a two dimensional computer model of the bone.

4. The method of claim 1 , wherein the rapid production machine includes at least one of stereo-lithograph machine or a computer numerical control machine.

5. The method of claim 1 , wherein the custom arthroplasty jig further includes at least one of a drilling guide, a reaming guide or a sawing guide referenced to the curvilinear surface profile of the custom arthroplasty jig.

6. The method of claim 1 , wherein the computer model of the bone includes at least one of a femoral mechanical axis, femoral mechanical center, femoral anatomical axis, or tibial mechanical axis.

7. The method of claim 1 , wherein the computer model of the implant is selected from a computer library of computer models of candidate implants.

8. The method of claim 1 , wherein the proposed alignment conditions include a transverse resection of the bone of the patient, the bone being a femur of the patient and the transverse resection being perpendicular to a femoral mechanical axis of the patient.

9. The method of claim 1 , wherein the bone of the patient includes at least one of a femur or tibia.

10. The method of claim 1 , wherein the surgeon assessment is based on available orthopedic theories, techniques or case studies.

11. The method of claim 1 , wherein the surgeon assessment includes a definition of a femoral mechanical axis including a center of the hip and a center of the femoral head that are surgeon-identified via the computer model of the bone.

12. The method of claim 1 , wherein the proposed alignment conditions are displayed on a computer display.

13. The method of claim 12 , wherein displaying the proposed alignment conditions includes displaying the computer models of the bone and the implant.

14. The method of claim 1 , wherein the medical images are at least one of magnetic resonance images or computed tomography images.

15. A method of designing a custom arthroplasty jig for performing an arthroplasty procedure on a joint of a patient, wherein an implant is implanted on a bone of the joint as part of the arthroplasty procedure, the method comprising:

a) receiving medical images of the bone including a damaged region, the medical images having been generated by a medical imaging machine;

b) generating a computer model of the bone from the medical images, the computer model of the bone comprising a virtual surface profile of the damaged region of the bone;

c) determining an alignment axis associated with the computer model of the bone of the joint;

d) generating a preoperative plan using the computer model of the bone generated from the medical images and a computer model of the implant,

wherein the preoperative plan is a result of using the computer model of the implant with the computer model of the bone to preoperatively plan proposed alignment conditions, which includes superimposing the computer model of the implant and the computer model of the bone, the proposed alignment conditions including a position of a resection plane relative to the alignment axis defined during the preoperative planning;

wherein generating the preoperative plan includes defining a region of a virtual jig to be recessed according to a shape of the virtual surface profile of the damaged region of the bone of the computer model;

e) outputting the proposed alignment conditions to a surgeon for assessment;

f) generating a production file based on assessed alignment conditions and the virtual jig the assessed alignment conditions reflecting surgeon assessment of the proposed alignment conditions; and

g) producing the custom arthroplasty jig via a rapid production machine according to the production file, the custom arthroplasty jig comprising a custom mating surface corresponding to the region of the virtual jig, the custom mating surface including a curvilinear surface profile configured to matingly receive the damaged region of the bone of the patient prior to resection of the bone in a single relative position.

16. The method of claim 15 , wherein the assessed alignment conditions reflecting surgeon assessment of the proposed alignment conditions includes adjustment to the proposed alignment conditions.

17. The method of claim 15 , wherein the computer model of the bone includes at least one of a three dimensional computer model of the bone or a two dimensional computer model of the bone.

18. The method of claim 15 , wherein the rapid production machine includes a stereo-lithograph machine or a computer numerical control machine.

19. The method of claim 15 , wherein the custom arthroplasty jig further includes at least one of a drilling guide, a reaming guide or a sawing guide referenced to the curvilinear surface profile of the custom arthroplasty jig.

20. The method of claim 15 , wherein the alignment axis includes at least one of a femoral mechanical axis, femoral anatomical axis, or tibial mechanical axis.

21. The method of claim 15 , wherein the computer model of the implant is selected from a computer library of computer models of candidate implants.

22. The method of claim 15 , wherein for the proposed alignment conditions, the position of the resection plane is perpendicular to the alignment axis.

23. The method of claim 15 , wherein the bone of the patient includes at least one of a femur or tibia.

24. The method of claim 15 , wherein the surgeon assessment is based on available orthopedic theories, techniques or case studies.

25. The method of claim 15 , wherein the surgeon assessment includes a definition of a femoral mechanical axis including a center of the hip and a center of the femoral head that are surgeon-identified via the computer model of the bone.

26. The method of claim 15 , wherein the proposed alignment conditions are displayed on a computer display.

27. The method of claim 26 , wherein displaying the proposed alignment conditions includes displaying the computer models of the bone and the implant.
