Abstract

Embodiments of the present invention provide systems and methods for the treatment of pain through activation of select neural fibers. The neural fibers may comprise one or more afferent neural fibers and/or one or more efferent neural fibers. If afferent fibers are stimulated, alone or in combination with efferent fibers, a therapeutically effective amount of electrical stimulation is applied to activate afferent pathways in a manner approximating natural afferent activity. The afferent fibers may be associated with primary receptors of muscle spindles, golgi tendon organs, secondary receptors of muscle spindles, joint receptors, touch receptors, and other types of mechanoreceptors and/or proprioceptors. If efferent fibers are stimulated, alone or in combination with afferent fibers, a therapeutically effective amount of electrical stimulation is applied to activate intrafusal and/or extrafusal muscle fibers, which results in an indirect activation of afferent fibers associated therewith.

Claims (30)

1. A method to reduce a perception of pain comprising:

electrically stimulating through a coiled lead having an electrode inserted in-vivo at least one target afferent sensory nerve fiber in a random or pseudo-random manner generating a stochastic response of action potentials in the at least one target afferent sensory nerve fiber while avoiding generation of action potentials in small sensory nerve fibers.

2. The method of claim 1 , wherein the electrical stimulation is generated through an electrical pulse generator coupled to a patient.

3. The method of claim 1 , wherein the electrical stimulation is a modulated high frequency.

4. The method of claim 1 , further comprising activating efferent nerve fibers.

5. The method of claim 1 further comprising percutaneously inserted the electrode in-vivo through an introducer needle.

6. The method of claim 1 , further comprising:

stimulating efferent nerve fibers to cause a contraction of a muscle to cause a plurality of responsive action potentials to be generated by the large sensory target afferent nerve fiber; and

sensing a characteristic of the responsive action potentials.

7. The method of claim 1 , wherein while generating stochastic response of action potentials in the at least one target afferent sensory nerve fiber action potentials are also generated in motor nerve fibers.

8. A method comprising:

percutaneously inserting an electrode in-vivo;

electrically stimulating through the electrode a large sensory target afferent nerve fiber in a random or pseudo-random manner generating a stochastic response of action potentials in the large sensory target afferent nerve fiber while avoiding generation of action potentials in non-target small sensory nerve fibers reducing a perception of pain.

9. The method of claim 8 , wherein the large sensory target afferent nerve fiber includes at least one of Type Ia and Ib target afferent nerve fibers.

10. The method of claim 8 , further comprising activating efferent nerve fibers.

11. The method of claim 8 , further comprising:

stimulating efferent nerve fibers to cause a contraction of a muscle to cause a plurality of responsive action potentials to be generated by the large sensory target afferent nerve fiber; and

sensing a characteristic of the responsive action potentials.

12. The method of claim 8 , wherein the electrical stimulation is generated by an external electronic pulse generator.

13. The method of claim 8 , wherein the electrode includes a fine-coiled wired lead coupled thereto.

14. The method of claim 8 , wherein the electrical stimulation is at a modulated high frequency.

15. A system to reduce a perception of pain comprising:

an electrode percutaneously inserted in-vivo;

an electrical pulse generator applying electrical stimulation to at least a portion of a nervous system through the electrode in a random or pseudo-random manner generating a stochastic response of an action potential in at least one target afferent sensory nerve fiber while avoiding action potentials in non-target small sensory afferent nerve fibers causing a reduction of perception of pain.

16. The system of claim 15 , wherein the electrical stimulation includes a modulated high frequency.

17. The system of claim 15 , wherein the modulated high frequency is about 300 Hz to about 20 kHz.

18. The system of claim 15 , wherein the modulated high frequency is between about 300 Hz and 20kHz.

19. The system of claim 15 , wherein the large sensory target afferent nerve fibers includes at least one of Type Ia and Ib target afferent nerve fibers.

20. The system of claim 15 , wherein the electrical stimulation actives efferent nerve fibers.

21. The system of claim 15 , wherein the electrical stimulation stimulates efferent nerve fibers to cause a contraction of a muscle to cause a plurality of responsive action potentials to be generated by the large sensory target afferent nerve fiber.
