Abstract

A pulse generator is programmed to generate electrical stimulation to target a sacral nerve or a pudendal nerve of the patient. The electrical stimulation being delivered at least in part via a lead. The electrical stimulation is applied by ramping up a stimulation parameter over time. A first, a second, and a third physiological response are detected from the patient as a result of the electrical stimulation. A first value, a second value, and a third value of the stimulation parameter associated with the first, second, and third physiological response are measured, respectively. A placement of the lead inside the patient is evaluated based on at least one of: a chronological sequence in which the first, second, and third physiological responses occurred, a comparison of the first value with a predetermined threshold, or respective deviations of the second value or the third value from the first value.

Claims (53)

1. A portable electronic programmer, comprising:

a user interface configured to receive an input from, and display an output to, a user;

an electronic memory storage configured to store programming instructions; and

one or more processors configured to execute the programming instructions to perform, the following steps:

programming a pulse generator to generate electrical stimulation to target a sacral nerve or a pudendal nerve of a patient, the electrical stimulation being delivered via a lead, the electrical stimulation being applied by ramping up a stimulation parameter over time;

detecting, based on the input received from the user interface, that the patient has exhibited an anal sphincter contraction response, a bellows response, and a toes response from the patient as a result of the electrical stimulation;

measuring a first value of the stimulation parameter associated with the anal sphincter contraction response, a second value of the stimulation parameter associated with the bellows response, and a third value of the stimulation parameter associated with the toes response;

evaluating a placement of the lead inside the patient based on at least one of: a chronological sequence in which the anal contraction response, the bellows response, and the toes response occurred, a comparison of the first value with a predefined threshold, or respective deviations of the second value or the third value from the first value; and

displaying, via the user interface, a result of the evaluating.

2. The portable electronic: programmer of claim 1 , wherein the steps further comprise: recommending, in response to a determination that the placement of the lead has not been optimized, a repositioning of the lead.

3. The portable electronic programmer of claim 1 , wherein the displaying comprises displaying, to the user, a message indicating that the placement of the lead has not been optimized.

4. The portable electronic programmer of claim 3 , wherein the evaluating the placement of the lead further comprises: determining that the placement of the lead has not been optimized unless the first value is below the predefined threshold.

The portable electronic programmer of claim 3 , wherein the evaluating the placement of the lead comprises determining that the placement of the lead has not been optimized unless the anal sphincter contraction response occurred before the bellows response, and the bellow response occurred before the toes response.

6. The portable electronic programmer of claim 3 , wherein the evaluating the placement of the lead comprises determining that the placement of the lead has not been optimized unless:

the respective deviations of the second value or the third value from the first value are within a first set of predefined boundaries; and

the deviation of the third value from the second value is within a second set f predefined boundaries.

7. The portable electronic programmer of claim 6 , wherein the steps further comprise: displaying, via a graphical representation, the first, second, and third values and at least one of: the predefined threshold, the first set of predefined boundaries, and the second set of predefined boundaries.

8. A medical system, comprising:

a pulse generator configured to generate electrical stimulation pulses as a part of an electrical stimulation therapy for a patient;

an implantable lead configured to deliver the electrical stimulation pulses to the patient; and

a portable electronic programmer that is telecommunicatively coupled to the pulse generator through a communications link, wherein the portable electronic programmer is configured to perform the following steps:

programming the pulse generator to generate electrical stimulation to target a sacral nerve or a pudendal nerve of the patient, the electrical stimulation being delivered at least in part via the implantable lead, the electrical stimulation being applied by ramping up a stimulation parameter over time;

detecting, based on input received from a user interface, that the patient has exhibited an anal sphincter contraction response, a bellows response, and a toes response as a result of the electrical stimulation;

measuring a first value of the stimulation parameter associated with the anal sphincter contraction response, a second value of the stimulation parameter associated with the bellows response, and a third value of the stimulation parameter associated with the toes response; and

evaluating a placement of the lead inside the patient based on at least one of: a chronological sequence in which the anal contraction response, the bellows response, and the toes response occurred, a comparison of the first value with a predefined threshold, or respective deviations of the second value or the third value from the first value.

9. The medical system of claim 8 , wherein the steps further comprise: recommending, in response to a determination that the placement of the lead has not been optimized, a repositioning of the lead.

10. The medical system of claim 8 , wherein the steps further comprise: displaying a message indicating how well the lead has been placed.

11. The medical system of claim 10 , wherein the evaluating the placement of the lead further comprises: determining that the placement of the lead has not been optimized unless the first value is below the predefined threshold.

The medical system of claim 10 , wherein the evaluating the placement of the lead comprises determining that the placement of the lead has not been optimized unless the anal sphincter contraction response occurred before the bellows response, and the bellow response occurred before the toes response.

13. The medical system of claim 10 , wherein the evaluating the placement of the lead comprises determining that the placement of the lead has not been optimized unless:

the respective deviations of the second value or the third value from the first value are within a first set of predefined boundaries; and

the deviation of the third value from the second value is within a second set of predefined boundaries.

14. The medical system of claim 13 , wherein the steps further comprise: displaying, via a graphical representation, the first, second, and third values and at least one of: the predefined threshold, the first set of predefined boundaries, and the second set of predefined boundaries.

15. A medical system, comprising:

a pulse generator;

an implantable lead;

a user interface;

a on-transitory memory; and

one or more hardware processors coupled to the non-transitory memory and configured to read instructions from the non-transitory memory to cause the medical system to perform operations comprising:

programming the pulse generator to generate electrical stimulation to target a sacral nerve or a pudendal nerve of a patient, the electrical stimulation being delivered via the lead, the electrical stimulation being applied by ramping up a stimulation parameter over time;

detecting, via input received from the user interface, that the patient has exhibited an anal sphincter contraction response, a bellows response, and a toes response as a result of the electrical stimulation;

measuring a first value of the stimulation parameter associated with the anal sphincter contraction response, a second value of the stimulation parameter associated with the bellows response, and a third value of the stimulation parameter associated with the toes response; and

evaluating a placement of the lead inside the patient based on at least one of: a chronological sequence in which the anal sphincter contraction response, the bellows response, and the toes response occurred, a comparison of the first value with a predefined threshold, or respective deviations of the second value or the third value from the first value.

16. The medical system of claim 15 , wherein the operations further comprise:

determining, based on the evaluating, that the lead has not been optimally placed; and

recommending, in response to the determining, a repositioning of the lead.

17. The medical system of claim 15 , wherein the evaluating the placement of the lead further comprises: determining that the placement of the lead has not been optimized unless the first value is below the predefined threshold.

18. The medical system of claim 15 , wherein the evaluating the placement of the lead comprises determining that, the placement of the lead has not been optimized unless the anal sphincter contraction response occurred before the bellows response, and the bellow response occurred before the toes response.

19. The medical system of claim 15 , wherein the evaluating the placement of the lead comprises determining that the placement of the lead has not been optimized unless:

the respective deviations of the second value or the third value from the first value are within a first set of predefined boundaries; and

the deviation of the third value from the second value is within a second set of predefined boundaries;

and wherein the operations further comprise: displaying, via a graphical representation, the first, second, and third values and at least one of: the predefined threshold, the first set of predefined boundaries, and the second set of predefined boundaries.

20. The medical system of claim 15 , wherein the operation further comprise: displaying, via a graphical user interface, a message indicating that the placement of the lead has not been optimized.
