Abstract

Devices and methods to attach soft tissue to bone include, for example, forming a bone tunnel in a bone, passing a suture through soft tissue, and inserting an anchor in the bone tunnel, wherein a distal portion of the anchor is positioned underneath the soft tissue, a proximal portion of the anchor is positioned beyond an edge of the soft tissue, the suture first portion extends from the distal portion of the anchor to the soft tissue, and the suture second portion extends from the proximal portion of the anchor to the soft tissue.

Claims (37)

1. A method of attaching a soft tissue to a bone, the method comprising: forming a bone tunnel in the bone, the bone having a longitudinal axis parallel to a length of the bone, the bone tunnel having at least first and second spaced apart openings at the surface of the bone; passing a knotless suture through the soft tissue, the knotless suture having a suture first end on a first side of the soft tissue and a suture second end on a second side of the soft tissue and a mid-portion between the suture first and second ends; and inserting an anchor into the bone tunnel, wherein after insertion a distal end of the anchor is positioned underneath the soft tissue, such that a line parallel to the longitudinal axis intersects both the anchor and the soft tissue, a proximal end of the anchor is positioned beyond an edge of the soft tissue, the suture first end extends from the distal end of the anchor to the soft tissue by passing through the bone tunnel, and the suture second end extends from the proximal end of the anchor to the soft tissue by passing proximally out of the bone tunnel and lying along an exterior surface of the bone.

2. The method of claim 1 wherein forming the bone tunnel comprises: forming the bone tunnel through the bone adjacent to the soft tissue, the first opening being positioned underneath the soft tissue and the second opening being positioned beyond the edge of the soft tissue.

3. The method of claim 2 , wherein the suture first end extends through the first opening and the suture second end extends through the second opening.

4. The method of claim 1 further comprising:

after inserting the anchor, tensioning the knotless suture.

5. The method of claim 4 further comprising:

after tensioning the knotless suture, provisionally securing the suture second end with the anchor such that tension on the knotless suture is temporarily maintained; and

evaluating a soft tissue repair effected by provisional securement of the suture second end with the anchor.

6. The method of claim 5 further comprising:

after evaluating the soft tissue repair, adjusting the knotless suture.

The method of claim 1 wherein the anchor includes a longitudinal passage extending into the anchor from the proximal end, the anchor further comprising an insert receivable in the longitudinal passage, the insert being operable to secure the knotless suture within the longitudinal passage, the method further comprising:

advancing the insert into the longitudinal passage to secure at least one of the suture first and second ends.

8. The method of claim 1 wherein at least one of the suture first and second ends extends through a portion of the anchor and at least one of the suture first and second ends extends along a portion of an outside surface of the anchor.

9. The method of claim 1 wherein forming the bone tunnel in the bone comprises forming the bone tunnel in a proximal humeral bone adjacent to a rotator cuff.

10. The method of claim 1 wherein the bone tunnel is formed through the bone by first and second intersecting bores into the bone.

11. A method of repairing a soft tissue adjacent a shoulder joint comprising a proximal humeral bone having a longitudinal axis parallel to a length of the proximal humeral bone, and a rotator cuff, the method comprising:

forming a bone tunnel through the proximal humeral bone adjacent to the rotator cuff, the bone tunnel having at least first and second spaced apart openings at a surface of the proximal humeral bone;

passing a suture through the rotator cuff, the suture having a suture first end on a first side of the rotator cuff and a suture second end on a second side of the rotator cuff; and

inserting an anchor into the bone tunnel, wherein after insertion a distal portion of the anchor is positioned underneath the rotator cuff such that a line parallel to the longitudinal axis intersects both the anchor and the rotator cuff, a proximal portion of the anchor is positioned beyond an edge of the rotator cuff, the suture first end extends from the distal portion of the anchor, through the first opening, and to the rotator cuff, and the suture second end extends from the proximal portion of the anchor, through the second opening, and to the rotator cuff.

12. The method of claim 11 , further comprising:

after inserting the anchor, tensioning the suture.

The method of claim 11 , wherein the anchor includes a longitudinal passage extending into the anchor from the proximal portion, the anchor further comprising an insert receivable in the longitudinal passage, the insert being operable to secure the suture within the longitudinal passage, the method further comprising:

advancing the insert into the longitudinal passage to secure at least one of the suture first and second ends.

14. A method of attaching a soft tissue to a bone, the method comprising: forming a bone tunnel in the bone, the bone having a longitudinal axis parallel to a length of the bone; passing a knotless suture through the soft tissue, the knotless suture having a suture first end on a first side of the soft tissue and a suture second end on a second side of the soft tissue, wherein one of the first side and the second &de faces toward the bone, and an other of the first side and the second side faces away from the bone; and inserting an anchor into the bone tunnel, wherein after insertion a distal portion of the anchor is positioned underneath the soft tissue such that a line parallel to the longitudinal axis intersects both the anchor and the soft tissue, a proximal portion of the anchor is positioned beyond an edge of the soft tissue, the suture first end extends from the distal portion of the anchor to the soft tissue, and the suture second end extends from the proximal portion of the anchor to the soft tissue.

15. The method of claim 14 , further comprising:

after inserting the anchor, tensioning the knotless suture.

The method of claim 15 , wherein the anchor includes a longitudinal passage extending into the anchor from the proximal portion, the anchor further comprising an insert receivable in the longitudinal passage, the insert being operable to secure the knotless suture within the longitudinal passage, the method further comprising:

advancing the insert into the longitudinal passage to secure at least one of the suture first and second ends.

17. A method of repairing a soft tissue adjacent a shoulder joint comprising a proximal humeral bone having a longitudinal axis parallel to a length of the proximal humeral bone, and a rotator cuff, the method comprising:

forming a bone tunnel through the proximal humeral bone adjacent to the rotator cuff, the bone tunnel having at least first and second spaced apart openings at a surface of the proximal humeral bone;

passing a knotless suture through the rotator cuff, the knotless suture having a suture first end on a first side of the rotator cuff and a suture second end on a second side of the rotator cuff; and

inserting an anchor into the bone tunnel, wherein after insertion a distal portion of the anchor is positioned underneath the rotator cuff such that a line parallel to the longitudinal axis intersects both the anchor and the rotator cuff, a proximal portion of the anchor is positioned beyond an edge of the rotator cuff the suture first end extends from the distal portion of the anchor to the rotator cuff, and the suture second end extends from the proximal portion of the anchor to the rotator cuff.

The method of claim 17 wherein: forming the bone tunnel comprises forming the first opening underneath the soft tissue and the second opening beyond the edge of the soft tissue; and the suture first end extends through the first opening and the suture second end extends through the second opening.

19. The method of claim 17 , further comprising:

after inserting the anchor, tensioning the knotless suture.

20. The method of claim 17 , wherein the anchor includes a longitudinal passage extending into the anchor from the proximal portion, the anchor further comprising an insert receivable in the longitudinal passage, the insert being operable to secure the knotless suture within the longitudinal passage, the method further comprising:

advancing the insert into the longitudinal passage to secure at least one of the suture first and second ends.
