Abstract

A system and method for endoscopically forming an anastomosis between two naturally adjacent points in the digestive tract. The system and method utilizes elongate magnetic devices that, when connected across a tissue boundary, necrose tissue until an anastomosis forms and the devices are passed naturally. Despite the elongate shape of the devices, the resulting anastomosis is substantially round. As such, round anastomoses can be formed having increased diameters merely by increasing the lengths of the devices, obviating the need for wider endoscopes.

Claims (39)

1. A method of forming a round anastomosis between two points in a digestive tract of a patient, the method comprising the steps of:

navigating a first endoscope to a first point in the digestive tract and navigating a second endoscope to a second point in the digestive tract, each of the first and second endoscopes carrying an elongated magnetic implant having a distal end with a first polarity and a proximal end with a second polarity and an elongated flat contact surface, each of the elongated magnetic implants having a length/width ratio of between about 4:1 and about 15:1;

positioning the elongated magnetic implants such that the distal and proximal ends of the elongated magnetic implant of the first endoscope are attracted to the proximal and distal ends of the elongated magnetic implant of the second endoscope, such that the elongated flat contact surface face each other and tissue is compressed between the elongated flat contact surface of the elongated magnetic implants; and

releasing the elongated magnetic implants.

2. A method for forming a round anastomosis between two adjacent vessel walls of a digestive tract, the method comprising:

releasably engaging a first elongated magnetic implant with a distal end of a first delivery catheter insertable in a working channel of a first endoscope; and

releasably engaging a second elongated magnetic implant with a distal end of a second delivery catheter insertable in a working channel of a second endoscope;

each of the first and second elongated magnetic implants comprising:

a magnet having two opposed ends; and

a housing configured to receive the magnet therein, the housing comprising:

an elongated flat contact surface; and

a connecting member connectable to a corresponding connector extending from a lumen of a corresponding delivery catheter to be releasably engageable with the connector;

magnetically coupling the first and second elongated magnetic implants to each other through the two adjacent vessel walls of the digestive tract to compress a portion of the two adjacent vessel walls between the elongated flat contact surface of each housing, each elongated magnetic implant having a width defined between two opposed sides thereof and a length defined between two opposed ends thereof, wherein the length and the width provide a length/width ratio of between about 4:1 and about 15:1.

3. The method of claim 2 , wherein the length/width ratio of each of the first and second elongated magnetic implants is between about 5:1 and about 8:1.

The method of claim 2 , wherein the round anastomosis has a predetermined diameter determined prior to implantation of the first and second elongated magnetic implants in the digestive tract, and the length/width ratio of each of the first and second elongated magnetic implants is provided based on the predetermined diameter.

5. The method of claim 4 , wherein the width of each of the first and second elongated magnetic implants is less than a third of the predetermined diameter of the round anastomosis.

6. The method of claim 2 , wherein the housing of each elongated magnetic implant fully encloses a corresponding magnet therein.

7. The method of claim 2 , wherein the magnet of each elongated magnetic implant comprises a plurality of magnet elements flexibly connected in series to form a flexible magnetic assembly.

8. The method of claim 7 , wherein the flexible magnetic assembly comprises a cable, a ribbon and/or a hitch for connecting the magnet elements together.

9. The method of claim 2 , wherein at least one of the first and second elongated magnetic implants is sized and configured to be insertable into the working channel of a respective one of the first and second endoscopes.

10. The method of claim 2 , wherein the connecting member of each housing protrudes from a longitudinal surface opposed to the elongated flat contact surface.

11. The method of claim 10 , wherein the connecting member comprises a knob and wherein the connector comprises a snare connectable to the knob.

12. The method of claim 11 , wherein the knob is located at a distal extremity of the housing or at a middle location in between opposed extremities of the housing.

13. The method of claim 2 , wherein the connecting member of each housing protrudes from a proximal end thereof.

14. The method of claim 13 , wherein each connecting member is configured to provide, with the connector, a ball-and-jaw connection or a loop-and-grabber connection.

15. The method of claim 2 , further comprising at least one additional elongated magnetic implant superposable or positionable side-by-side relative to the first or second elongated magnetic implant.

16. The method of claim 2 , wherein the elongated flat contact surfaces of the first and second elongated magnetic implants are identical to each other.

17. A method for forming a round anastomosis between two adjacent vessel walls of a digestive tract, the method comprising:

releasably engaging a first elongated magnetic implant and a second elongated magnetic implant with respective endoscopes; and

magnetically coupling a respective flat contact surface of the first elongated magnetic implant and the second elongated magnetic implant to each other through the two adjacent vessel walls of the digestive tract to compress a portion of the two adjacent vessel walls therebetween;

each elongated magnetic implant having a width defined between two opposed sides thereof and a length defined between two opposed ends thereof, wherein the length and the width provide a length/width ratio of between about 4:1 and about 15:1.

18. The method of claim 17 , wherein the length/width ratio of each of the first and second elongated magnetic implants is between about 5:1 and about 8:1.

19. The method of claim 17 , wherein the first and second elongated magnetic implants each comprises a magnet and a housing fully enclosing the magnet.

The method of claim 19 , wherein each housing comprises a connecting member releasably engageable with a connector extending from a lumen of a corresponding delivery catheter, the connecting member and the connector being configured to form a knob-and-snare connection, a ball-and-jaw connection or a loop-and-grabber connection.

21. A method for delivering a first elongated magnetic implant into a first vessel of a digestive tract and a second elongated magnetic implant into a second vessel of the digestive tract, each of the first and second elongated magnetic implant comprising an elongated flat contact surface, the method comprising the steps of:

releasably engaging the first elongated magnetic implant and the second elongated magnetic implant with a distal end of a respective one of a first delivery catheter and second delivery catheter,

loading each of the delivery catheters into a working channel of a respective one of a first delivery endoscope and a second delivery endoscope; and

positioning each of the first and second magnetic implant longitudinally along the longitudinal axis of the respective one of the first and second endoscopes;

each elongated magnetic implant having a width defined between two opposed sides thereof and a length defined between two opposed ends thereof, wherein the length and the width provide a length/width ratio of between about 4:1 and about 15:1.
