Abstract

Embodiments provide swallowable devices, preparations and methods for delivering therapeutic agents (TAs) within the GI tract such as antibodies (AP-antibodies) or other proteins which neutralize PCSK9 molecules. Many embodiments provide a swallowable device e.g., a capsule for delivering TAs into the intestinal wall (IW). Embodiments also provide TA preparations that are configured to be contained within the capsule, advanced from the capsule into the IW and/or peritoneum and degrade to release the TA into the bloodstream to produce a therapeutic effect. The preparation can be operably coupled to delivery means having a first configuration where the preparation is contained in the capsule and a second configuration where the preparation is advanced out of the capsule into the IW. Embodiments are particularly useful for delivery of AP-antibodies and related TA's for the treatment of cholesterol, lipid and related conditions where such TAs are poorly absorbed and/or degraded within the GI tract.

Claims (39)

1. A method for reducing a low-density lipoprotein cholesterol (LDL-C) level in a patient having hypercholesterolemia, the method comprising: providing a solid anti-proprotein convertase subtilsin-kexin type 9 (PCSK9)-antibody (AP-antibody) dosage shaped as a tissue penetrating member, wherein the AP-antibody is selected from the group consisting of Alirocumab, Evolocumab and Bococizumab and wherein a dose of AP-antibody in the solid AP dosage is in a range from about 1 to 15 mg; penetrating the solid dosage AP-antibody into an intestinal wall after oral ingestion by the application of force on the tissue penetrating member such that the tissue penetrating member is retained in the intestinal wall or surrounding tissue; and releasing the AP-antibody into the blood stream from the solid dosage AP-antibody in the intestinal wall or surrounding tissue to reduce the LDL-C level by binding of PCSK9 molecules by the released dosage of PCSK9 antibody.

A method for treating hypercholesterolemia by in a patient by reducing the patient's low-density lipoprotein cholesterol (LDL-C) level, the method comprising: providing a solid anti-proprotein convertase subtilsin-kexin type 9 (PCSK9),-antibody (AP-antibody) dosage shaped as a tissue penetrating member, wherein the AP-antibody is selected from the group consisting of Alirocumab, Evolocumab and Bococizumab and wherein a dose of AP-antibody in the solid AP dosage is in a range from about Ito 15 mg; penetrating the solid dosage AP-antibody into an intestinal wall after oral ingestion by the application of force on the tissue penetrating member such that the tissue penetrating member is retained in the intestinal wall or surrounding tissue; releasing the AP-antibody into the blood stream from the solid dosage AP-antibody in the intestinal wall or surrounding tissue to reduce the LDL-C level by binding of PCSK9 molecules by the released dosage of PCSK9 antibody; monitoring an LDL-C level in the patient; and adjusting a dose of the AP-antibody in response to the monitored LDL-C level in the patient.

A method for treating a cardiovascular condition in a patient caused by hypercholesterolemia by reducing the patient's low-density lipoprotein cholesterol (LDL-C) level, the method comprising: providing a solid anti-PCSK9 antibody (AP-antibody) dosage shaped as a tissue penetrating member, wherein the AP-antibody is selected from the group consisting of Alirocumab, Evolocumab and Bococizumab and wherein a dose of AP-antibody in the solid AP dosage is in a range from about 1 to 15 ma; penetrating the solid dosage AP-antibody into an intestinal wall after oral ingestion by the application of force on the tissue penetrating member such that the tissue penetrating member is retained in the intestinal wall or surrounding tissue; releasing the AP-antibody into the blood stream from the solid dosage AP-antibody in the intestinal wall or surrounding tissue to reduce the LDL-C level by binding of PCSK9 molecules by the released dosage of PCSK9 antibody; monitoring an LDL-C level in the patient; and adjusting a dose of the AP antibody in response to the monitored LDL-C level in the patient.

4. The method of claim 1 , wherein the force is a mechanical force.

5. The method of claim 1 , wherein when inserted into the intestinal wall of a patient on a daily basis, the preparation results in a percent steady state fluctuation in plasma concentration of AP-antibody in the patient in a range of about 0.12 to about 0.39%.

6. The method of claim 1 , wherein the surrounding tissue is a peritoneal wall.

7. The method of claim 1 , further comprising performing genetic testing on the patient to determine a preferred AP-antibody to be delivered to the patient.

8. The method of claim 7 , wherein the preferred AP-antibody is based on effectiveness in reducing LDL-C levels in the patient.

9. The method of claim 7 , wherein the preferred AP-antibody is selected based on that having the fewest or least severe side effects in the patient.

10. The method of claim 7 , wherein the preferred AP-antibody is Alirocumab, Evolocumab and Bococizumab.

11. The method of claim 1 , further comprising: monitoring an LDL-C level in the patient.

12. The method of claim 11 , further comprising: adjusting a dose of the AP antibody in response to the monitored LDL-C level in the patient.

13. The method of claim 12 , wherein the dose of AP-antibody is adjusted to produce a selected level of reduction in an LDL-C level in the patient.

14. The method of claim 13 , wherein the selected level of LDL-C reduction is at least 30%.

15. The method of claim 13 , wherein the selected level of LDL-C reduction is in a range of about 40 to 60 mg/dl.

16. The method of claim 11 , further comprising decreasing a dose of the orally ingested AP antibody when the LDL-C level in the patient falls below a threshold level.

17. The method of claim 16 , wherein the threshold level is <25 mg/dl.

18. The method of claim 1 , wherein the AP-antibody released into the blood stream from the solid dosage AP-antibody reaches a Cmax in a shorter time period than a time period to achieve a Cmax for an extravascularly injected dose AP-antibody.

19. The method of claim 18 , wherein a tmax for the AP-antibody released from the solid dosage AP-antibody is less than 50% of a tmax for the extravascularly injected dose of AP-antibody.

20. The method of claim 18 , wherein a tmax for the AP-antibody released from the solid dosage AP-antibody is less than 10% of a tmax for the extravascularly injected dose of AP-antibody.

21. The method of claim 1 , wherein an immune response in the patient to the AP-antibody is minimal.

22. The method of claim 21 , wherein a white blood cell count in the patient's serum is increased by no more than about 10% in response to release of the AP-antibody into the patient's blood stream.

23. The method of claim 21 , wherein a serum titer of patient antibodies to the AP-antibody is increased by no more than about 10% in response to release of the AP-antibody into the patient's blood stream.

24. The method of claim 21 , wherein a serum titer of interleukin-7 is increased by no more than about 10% in response to release of the AP-antibody into the patient's blood stream.

25. The method of claim 1 , wherein an immune response in the patient to the AP-antibody is reduced as compared to delivery of the AP-antibody by an intramuscular or sub-dermal injection.

26. The method of claim 25 , wherein the immune response in the patient to the AP-antibody is reduced by an amount in the range of about two to thirty times as compared to an intramuscular or sub-dermal injection.

27. The method of claim 2 , wherein the dose of AP-antibody is adjusted to produce a selected level of reduction in LDL-C level in the patient.

28. The method of claim 27 , wherein the selected level of LDL-C reduction is at least 30%.

29. The method of claim 27 , wherein the selected level of LDL-C reduction is in a range of about 40 to 60 mg/dl.

30. The method of claim 2 , further comprising decreasing a dose of the AP antibody when the LDL-C level in the patient falls below a threshold level.

31. The method of claim 30 , wherein the threshold level is <25 mg/dl.

32. The method of claim 2 , wherein an immune response in the patient to the AP-antibody is minimal.

33. The method of claim 32 , wherein a white blood cell count in the patient's serum is increased by no more than about 10% in response to release of the AP-antibody into the patient's blood stream.

34. The method of claim 32 , wherein a serum titer of patient antibodies to the AP-antibody is increased by no more than about 10% in response to release of the AP-antibody into the patient's blood stream.

35. The method of claim 32 , wherein a serum titer of interleukin-7 is increased by no more than about 10% in response to release of the AP-antibody into the patient's blood stream.

36. The method of claim 2 , wherein an immune response in the patient to the AP-antibody is reduced as compared to delivery of the AP-antibody by an intramuscular or sub-dermal injection.

37. The method of claim 36 , wherein the immune response in the patient to the AP-antibody is reduced by an amount in the range of about two to thirty times as compared to an intramuscular or sub-dermal injection.

38. The method of claim 2 , wherein the force is a mechanical force.

39. The method of claim 38 , wherein the force is a mechanical force.
