Abstract

A bone plate system is provided that includes bone plates and bone anchor assemblies for being inserted into bone plate bores to secure the bone plates to one or more bones. In one aspect, the bone plate system includes a bone anchor assembly having a locking cap and a locking fastener connected to a head portion of a bone anchor. The bone anchor assembly is driven into a bone plate bore before the locking fastener is shifted to expand the locking cap and fix the bone anchor assembly within the bore. In another aspect, the bone plate system includes a bone anchor assembly having a locking cap that deflects radially inward due to contact with the bone plate at large bone anchor assembly insertion angles. In addition, a bone anchor assembly having a preassembled condition with a locking cap resisting back out of a locking fastener.

Claims (28)

1. A method of securing a bone plate to a bone using a preassembled bone anchor assembly, the preassembled bone anchor assembly including a bone anchor having an uppermost end and a lowermost end and a longitudinal axis extending therebetween, the method comprising:

connecting the preassembled bone anchor assembly to a driver, the preassembled bone anchor assembly including a locking cap distinct from the bone anchor that is assembled with the bone anchor and a locking member distinct from the bone anchor that is assembled with the bone anchor, the locking cap extending around the uppermost end of the bone anchor and the locking member, the locking cap including a radially outer upper end portion that extends axially above the uppermost end of the bone anchor and a plurality of projections extending radially inward from the radially outer upper end portion, wherein the locking cap of the preassembled bone anchor assembly is retained on the bone anchor to inhibit relative axial movement of the locking cap along the bone anchor in either an upward axial direction or a downward axial direction;

advancing, using the driver, a lower shank portion of the bone anchor of the preassembled bone anchor assembly through a throughbore of the bone plate and into the bone;

positioning an upper rigid head portion of the bone anchor and the locking cap assembled with the bone anchor in the throughbore so that the locking cap contacts a throughbore surface of the bone plate; and

expanding radially outward the radially outer upper end portion of the locking cap that extends axially above the uppermost end of the bone anchor to secure the preassembled bone anchor assembly relative to the bone plate and secure the bone plate relative to the bone by shifting the locking member of the bone anchor assembly axially along the bone anchor, using a second driver, which causes the locking member to urge the plurality of projections of the locking cap radially outward.

2. The method of claim 1 wherein expanding the radially outer upper end portion of the locking cap includes urging a radially inner lower end portion of the locking cap radially inward into engagement with the bone anchor.

3. The method of claim 2 wherein expanding the radially outer upper end portion of the locking cap includes pivoting a wall of the locking cap that includes the radially outer upper end portion and the radially inner lower end portion.

4. The method of claim 1 wherein expanding the radially outer upper end portion of the locking cap includes expanding gaps between segments of the radially outer upper end portion of the locking cap.

5. The method of claim 4 wherein expanding the radially outer upper end portion of the locking cap includes engaging the segments of the radially outer upper end portion with the throughbore surface of the bone plate.

The method of claim 1 wherein expanding radially outward the radially outer upper end portion of the locking cap by shifting the locking member axially along the bone anchor which causes the locking member to urge the projections radially outward includes engaging cam surfaces of the locking member and the projections of the locking cap axially above the bone anchor uppermost end.

7. The method of claim 1 wherein advancing the lower shank portion of the preassembled bone anchor assembly through the throughbore and into the bone includes advancing the preassembled bone anchor assembly into the throughbore at an angle relative to a central axis of the throughbore; and

positioning the bone anchor upper rigid head portion and the locking cap assembled with the bone anchor in the throughbore includes contacting a radially outer surface of the locking cap against the bone plate and shifting one of the protrusions of the locking cap radially inward toward the locking member.

8. The method of claim 7 wherein expanding the radially outer upper end portion of the locking cap includes cammingly engaging the locking member and the projections of the locking cap.

The method of claim 1 wherein advancing the lower shank portion of the bone anchor through the throughbore and into the bone includes driving the shank portion of the bone anchor into the bone using the driver and shifting the locking member axially along the bone anchor includes driving the locking member with the second driver.

10. A method of securing a bone plate to a bone using a preassembled bone anchor assembly, the method comprising:

connecting the preassembled bone anchor assembly to a driver, the preassembled bone anchor assembly including a bone anchor having an uppermost end and a lowermost end and a longitudinal axis extending therebetween, the preassembled bone anchor assembly further including a locking cap distinct from the bone anchor that is assembled with the bone anchor and a locking member distinct from the bone anchor that is assembled with the bone anchor, the locking cap extending around the uppermost end of the bone anchor and the locking member, the locking cap including a radially outer portion of the locking cap and a plurality of projections extending radially inward from the radially outer portion, wherein the locking cap of the preassembled bone anchor assembly is retained on the bone anchor to inhibit relative axial movement of the locking cap along the bone anchor in either an upward axial direction or a downward axial direction;

advancing, using the driver, a shank of the bone anchor of the preassembled bone anchor assembly through a throughbore of the bone plate and into the bone;

locating a rigid head portion of the bone anchor and the locking cap assembled with the bone anchor in the throughbore of the bone plate so that the locking cap contacts a throughbore surface of the bone plate;

shifting the radially outer portion of the locking cap radially outward against the throughbore surface of the bone plate to secure the preassembled bone anchor assembly relative to the bone plate and secure the bone plate relative to the bone by shifting the locking member, using a second driver, along the bone anchor rigid head portion which causes the locking member to urge the plurality of projections of the locking cap radially outward; and

urging a radially inner portion of the locking cap radially inward against the bone anchor rigid head portion by shifting of the locking member along the bone anchor rigid head portion which causes the locking member to urge the projections radially outward.

11. The method of claim 10 wherein shifting the radially outer portion of the locking cap radially outward against the throughbore surface and urging the radially inner portion of the locking cap radially inward against the bone anchor rigid head portion includes pivoting a wall of the locking cap that includes the radially outer portion and the radially inner portion.

12. The method of claim 10 wherein shifting the radially outer portion of the locking cap radially outward against the surface of the bone plate throughbore includes expanding gaps between upper segments of the locking cap.

13. The method of claim 10 wherein urging the radially inner portion of the locking cap radially inward against the bone anchor rigid head portion includes constricting gaps between lower segments of the locking cap.

The method of claim 10 wherein shifting the radially outer portion of the locking cap radially outward by shifting the locking member along the bone anchor rigid head portion which causes the locking member to urge the projections radially outward includes cammingly engaging the locking member and the projections of the locking cap.

15. The method of claim 10 wherein advancing the shank of the bone anchor through the throughbore and into the bone includes advancing the shank of the bone anchor into the throughbore at an angle relative to a central axis of the throughbore; and

locating the bone anchor rigid head portion and the locking cap in the throughbore of the bone plate includes contacting a radially outer portion of the locking cap against the bone plate and shifting a radially inner portion of the locking cap toward the locking member.

The method of claim 10 wherein advancing the shank of the bone anchor assembly into the throughbore of the bone plate and into the bone includes driving the shank of the bone anchor into the bone using a first driver and shifting the locking member along the bone anchor rigid head portion includes driving the locking member with a second driver.

17. The method of claim 10 wherein urging the radially inner portion of the locking cap radially inward against the bone anchor rigid head portion includes urging the radially inner portion of the locking cap radially inward into a groove of the bone anchor.
