Abstract

A retraction system and method are provided for retracting tissues surrounding a surgical site. In one aspect, a method including engaging slide connections between a guide dilator and a plurality of tissue engaging members and sequentially enlarging an incision using the guide dilator and the plurality of tissue engaging members. In another aspect, a method of inserting a plurality of tissue engaging members into an incision including fixing tip portions of the plurality of tissue engaging members in an insertion configuration, advancing the tip portions into an incision, and restricting movement of the tip portions away from the insertion configuration. A guide dilator system comprising an elongate body, a plurality of tissue engaging members, and slide connections between the elongate body and the tissue engaging members is also provided.

Claims (37)

1. A retractor for enlarging an incision, the retractor comprising:

a frame;

a plurality of tissue engaging members for being inserted into an incision;

a plurality of operating mechanisms for connecting the tissue engaging members to the frame and being operable to allow the tissue engaging members to be moved to retracted positions to enlarge the incision;

a slider of one of the operating mechanisms for being connected to one of the tissue engaging members, the slider being slidably connected to the frame and shiftable between extended and retracted positions, the slider including recesses associated therewith;

an actuator of the one operating mechanism that includes a projection configured to be received in the recesses of the slider, the projection and each of the recesses being configured to permit the slider to be shifted toward a retracted position and resist shifting of the slider toward an extended position, the actuator being operable to withdraw the projection from the recess in which the projection is received to allow the slider to be shifted toward the retracted and the extended positions thereof; and

an actuator lock having a first position that keeps the actuator from being operated to withdraw the projection from the recess in which the projection is received and a second position that allows for operation of the actuator.

2. The retractor of claim 1 wherein the actuator lock abuts the frame in the first position thereof and the actuator lock is spaced from the frame in the second position thereof.

3. The retractor of claim 1 wherein the actuator lock is pivotally connected to the actuator and pivotal between the first and second positions thereof relative to the actuator.

4. The retractor of claim 1 wherein the actuator lock has a third position that causes the actuator to hold the projection out of engagement with the recesses of the slider.

5. The retractor of claim 4 wherein the actuator lock abuts a first portion of the frame in the first position thereof, the actuator lock is spaced from the frame in the second position thereof, and the actuator lock abuts a second portion of the frame in the third position thereof.

6. The retractor of claim 4 wherein the slider is shiftable between the extended and retracted positions thereof along a first axis and the actuator lock is pivotal between the first, second, and third positions about a second axis substantially perpendicular to the first axis.

7. The retractor of claim 1 wherein the actuator includes a detent mechanism configured to hold the actuator lock in either one of the first position and the second position.

8. The retractor of claim 7 wherein the detent mechanism includes first and second detent recesses and a detent member configured to engage the first detent recesses with the actuator lock in the first position and the second detent recess with the actuator lock in the second position.

9. The retractor of claim 8 wherein the detent mechanism includes a biasing member urging the detent member into engagement with either one of the first detent recess and the second detent recess.

10. The retractor of claim 8 wherein the actuator lock has a third position that causes the actuator to hold the projection out of engagement with the recesses of the slider; and

the detent mechanism includes a third recess and the detent member is configured to engage the third detent recess with the actuator lock in the third position thereof.

11. The retractor of claim 1 wherein the one operating mechanism includes a rotatable drive member engaged with the slider and rotatable in a retracting direction to cause the slider to shift toward the retracted position thereof.

12. The retractor of claim 11 wherein the one operating mechanism includes a tool-receiving member coupled to the rotatable drive member and operable to be turned in a first direction to cause the drive member to turn in the retracting direction; and

a clutch mechanism of the one operating mechanism configured to decouple the drive member and the tool-receiving member in response to turning of the tool-receiving member in an opposite, second direction to limit shifting of the slider toward the extended position thereof.

13. The retractor of claim 1 wherein the actuator lock includes a socket adapted to receive a turning tool.

14. A retractor for enlarging an incision, the retractor comprising:

a frame;

a plurality of tissue engaging members for being inserted into an incision;

a plurality of operating mechanisms for connecting the tissue engaging members to the frame and being operable to allow the tissue engaging members to be moved to retracted positions thereof to enlarge the incision;

a rotatable drive member of one of the operating mechanisms rotatable in a retracting direction to cause the tissue engaging member associated with the one operating mechanism to shift toward the retracted position thereof;

a tool-receiving member of the one operating mechanism, the drive member and the tool-receiving member having a drive mode effectuated by turning of the tool-receiving member in a first direction, the drive mode permitting turning of the tool-receiving member in the first direction to cause rotation of the drive member in the retracting direction and shifting of the tissue engaging member toward the retracted position thereof; and

the drive member and the tool-receiving member having a non-drive mode effectuated by turning of the tool-receiving member in a second direction, the non-drive mode inhibiting turning of the tool-receiving member in the second direction from causing rotation of the drive member in a direction opposite the retracting direction.

15. The retractor of claim 14 wherein the one operating mechanism includes a clutch mechanism that decouples the drive member and the tool-receiving member in the non-drive mode.

16. The retractor of claim 14 wherein the drive member and the tool-receiving member include cam surfaces configured to shift the drive member and tool-receiving member apart with turning of the tool-receiving member in the second direction.

17. The retractor of claim 14 wherein the drive member is rotatable about an axis and the drive member and the tool-receiving member are shifted axially apart with turning of the tool-receiving member in the second direction.

18. The retractor of claim 14 wherein the drive member and the tool-receiving member include drive surfaces that abut with turning of the tool-receiving member in the first direction and cause rotation of the drive member in the retracting direction.

19. The retractor of claim 14 wherein the tool-receiving member is carried on the drive member.

20. The retractor of claim 14 wherein the one operating mechanism includes a biasing member adapted to urge the drive member and the tool-receiving member together.

21. The retractor of claim 14 wherein the one operating mechanism includes a slider having a rack associated therewith and the drive member includes a pinion with teeth adapted to engage the rack and drive the slider toward the retracted position with turning of the pinion in the retracting direction.

The retractor of claim 14 wherein the one operating mechanism includes a slider supporting the one tissue engaging member, a plurality of recesses associated with the slider, and an actuator having a projection configured to be received in the recesses, wherein the projection and recesses are configured to permit the slider to be shifted toward a retracted position and resist shifting of the slider toward an extended position and the actuator is operable to withdraw the projection from the recess in which the projection is received to allow the slider to be shifted toward the extended position thereof.

23. The retractor of claim 14 wherein the tool-receiving member includes a rotary drive structure adapted to receive a drive tool.
