Abstract

A method for fusing vertebral bodies comprising the steps of: mounting a first engagement member of a fixation member to a pedicle of a first vertebra and a second engagement member of the spinal fixation member to a pedicle of a second vertebra, the first engagement member having a first extension engaged with an expansion ring, the second engagement member having a second extension engaged with the expansion ring; inserting screws through the first and second engagement members and into the pedicles of the respective first and second vertebrae; rotating the expansion ring in a first direction, thereby distracting the first and second vertebrae; implanting an intervertebral implant between the first and second vertebrae after rotating the expansion ring in the first direction; and rotating the expansion ring in a second direction after implanting the intervertebral implant, thereby compressing the first and second vertebrae.

Claims (31)

1. A method of fusing vertebrae of a mammalian subject comprising:

implanting an intervertebral implant between first and second vertebrae via a posterolateral approach;

mounting a first engagement member to the pedicle of the first vertebra and a second engagement member to the pedicle of the second vertebra, the first and second engagement members being moveably connected via an actuation member disposed entirely between the first and second engagement members; and

moving the first and second engagement members toward each other to compress the intervertebral implant between the first and second vertebrae, wherein moving the first and second engagement members is performed by operating the actuation member from between the first and second engagement members.

2. The method of claim 1 , wherein operating the actuation member includes engaging the actuation member with a driver and rotating the actuation member with the driver.

3. The method of claim 2 , wherein engaging the actuation member with the driver includes intermeshing teeth of the driver with teeth of the actuation member.

4. The method of claim 2 , wherein rotating the actuation member with the driver includes rotating the driver about a first axis perpendicular to a second axis about which the actuation member is rotated.

5. The method of claim 1 , wherein mounting is performed intraoperatively, and the actuation member and first and second engagement members remain mounted postoperatively.

6. The method of claim 1 , wherein the intervertebral implant includes one of autologous bone, allograft bone, DBM or BMP.

7. The method of claim 1 , wherein mounting includes:

driving fasteners through respective openings in the first and second engagement members, and

engaging locking rings coupled to the first and second engagement members with the fasteners to restrict movement thereof.

8. A method of fusing vertebrae comprising:

mounting first and second supplemental fixation devices to the first and second vertebrae;

intraoperatively distracting the first and second vertebrae by actuating first and second actuation members of the first and second supplemental fixation devices, respectively;

inserting an intervertebral implant between the first and second vertebrae and between the first and second supplemental fixation devices; and

intraoperatively compressing the implant with the first and second vertebrae by actuating the actuation members of the first and second supplemental fixation devices, wherein the first and second supplemental fixation devices and actuation members thereof remain mounted to the first and second vertebrae postoperatively.

The method of claim 8 , wherein mounting the first supplemental fixation device includes connecting a first engagement member of the first supplemental fixation device to a pedicle of the first vertebra and connecting a second engagement member of the first supplemental fixation device to a pedicle of the second vertebra.

The method of claim 8 , wherein distracting the first and second vertebrae includes engaging the respective actuation members of the first and second supplemental fixation devices with one or more drivers and rotating the one or more drivers in a first direction, and compressing the implant with the first and second vertebrae includes engaging the respective actuation members of the first and second supplemental fixation devices with the one or more drivers and rotating the one or more drivers in a second direction.

11. The method of claim 10 , wherein engaging the respective actuation members includes intermeshing teeth of the respective actuation members with teeth of the one or more drivers.

12. The method of claim 8 , further comprising associating bone graft material with the intervertebral implant, the bone graft material being one of autologous bone, allograft bone, DBM, or BMP.

13. The method of claim 8 , wherein inserting the intervertebral implant is performed via a posterior approach.

14. A method of treating scoliosis of a mammalian subject comprising:

accessing a first supplemental fixation device having first and second engagement members and an actuation member connected to the first and second engagement members, the actuation member and first and second engagement members having been previously mounted to first and second vertebrae of the mammalian subject in a prior surgical procedure; and

actuating the actuation member to move the first and second engagement members relative to each other and to expand or contract an intervertebral disc space defined by the first and second vertebrae.

15. The method of claim 14 , wherein actuating the actuation member is performed intraoperatively and the actuation member remains mounted postoperatively.

16. The method of claim 14 , wherein actuating the actuation member includes selectively rotating the actuation member in a first or second direction to expand or contract the intervertebral disc space depending on the direction selected.

17. The method of claim 16 , wherein selectively rotating the actuation member is performed with a driver configured to engage the actuation member.

18. The method of claim 14 , further comprising actuating a second supplemental fixation device to expand or contract the intervertebral disc space, the first and second supplemental fixation devices being respectively mounted to first and second sets of pedicles of the first and second vertebrae.

The method of claim 9 , wherein the actuation member of the first supplemental fixation device is connected to the first and second engagement members of the first supplemental fixation device at a location between the first and second engagement members, and actuating the actuation member of the first supplemental fixation system to distract and compress is performed between the first and second engagement members of the first supplemental fixation device.

20. The method of claim 14 , wherein the actuation member of the first supplemental fixation device is connected to the first and second engagement members at a location therebetween, and actuating the actuation member to expand or contract the intervertebral disc space is performed between the first and second engagement members of the first supplemental fixation device.
