Abstract

A system and process for performing orthopedic surgery is provided that uses a patient's existing implant as a registration tool in an orthopedic surgical procedure. The systems and processes may be used with computer assisted systems or navigation systems to aid in the removal of bone, bone cement, or a bone prosthesis, typically a bone prosthesis used in hip replacement surgery, knee replacement surgery, and the like. The removal of the prosthesis may be done by conventional methods, with navigation systems, robotic assistance or articulating hand held systems. The removal of bone and/or bone cement may be performed with navigated systems, robotic systems, articulating hand-held systems and combinations thereof.

Claims (38)

1. A method for revision of a total knee replacement (TKR) for a patient comprising:

performing an imaging scan of a bone with an installed implant to obtain a set of digital imaging data of the bone and the installed implant;

segmenting the installed implant and segmenting the bone outside an implant region in the set of digital imaging data;

obtaining a three dimensional (3D) model based on a manufacturer's specifications of the installed implant;

registering the manufacturer's 3D model to the segmented implant to generate a transformation matrix thereof thus providing the transformation matrix between the manufacturer's 3D model and the digital imaging data; and

revising the TKR with a new implant for the patient based on a pre-operative plan.

2. The method of claim 1 wherein said revising aligns articular surfaces of said new implant with articular surfaces of the installed implant.

3. The method of claim 1 wherein said revising has planar cuts for said new implant that are offset from planar surfaces of said installed implant.

4. The method of claim 1 wherein said revising is performed using a robotic surgical system.

5. The method of claim 1 wherein said revising comprises:

proceeding to remove the installed implant from the patient;

thereafter measuring an amount of bone removed in various planes;

choosing a surgical plan which places new cuts within a remaining bone stock of the bone;

cutting the bone using a robotic surgical system and drilling intramedullary canals;

implanting a femoral component of the new implant to restore an articular surface of the installed implant; and

implanting a tibial component of the new implant to restore the articular surface of the installed implant.

6. A method for revision of a total hip replacement (THR) for a patient comprising:

performing an imaging scan of a bone with an installed implant to obtain a set of digital imaging data of the bone and the installed implant;

segmenting the installed implant and segmenting the bone outside an implant region in the set of digital imaging data;

obtaining a 3D model based on a manufacturer's specifications of the installed implant;

registering the manufacturer's 3D model to the segmented implant to generate a transformation matrix thereof thus providing the transformation matrix between the manufacturer's 3D model and the digital imaging data; and

revising the THR with one of a standard trochanteric osteotomy, a trochanteric slide osteotomy, or an extended trochanteric osteotomy to insert a new implant for the patient based on a pre-operative plan.

7. The method of claim 6 wherein said revising aligns surfaces of said new implant with surfaces of the said installed implant.

8. The method of claim 6 wherein said revising is pre-operatively planned multiple times such that new cut surfaces will be inside the bone for any amount of remaining bone stock.

9. The method of claim 6 wherein said revising is done robotically.

10. The method of claim 6 wherein said revising comprises:

proceeding to remove the installed implant from the patient;

thereafter measuring an amount of bone remaining in various planes;

choosing a surgical plan which places a new cut surface within a remaining bone stock of the bone; and

cutting the bone using a robot.

11. A surgical system comprising:

an input device capable of receiving data from an image of a bone with an installed prosthesis to obtain a set of digital imaging data of the bone and the installed prosthesis;

a database containing three dimensional (3D) models and manufacturing specifications of the installed prosthesis;

a computer processor unit (CPU) capable of:

segmenting the installed prosthesis and segmenting the bone outside a prosthesis region in the set of digital imaging data, registering a manufacturer's 3D model to the segmented implant and generating a transformation matrix thereof thus providing a transformation matrix between the manufacturer's 3D model and the digital imaging data; and

a surgical tool which computes the transformation matrix from the installed prosthesis to the set of digital imaging data of the patient to perform a surgical procedure.

12. The surgical system of claim 11 wherein the surgical tool is a computer-controlled assisted surgical device consisting of one of a navigated system, robotic system, or articulating hand-held system.

13. The surgical system of claim 11 wherein the surgical procedure is pre-operatively planned.
