Abstract

The present invention relates to methods and compositions for monitoring, diagnosis, prognosis, and determination of treatment regimens in subjects suffering from or suspected of having a renal injury. In particular, the invention relates to using assays that detect Chitinase-3-like protein 1 as diagnostic and prognostic biomarker assays in renal injuries.

Claims (35)

1. A method for evaluating renal status in a subject not currently receiving dialysis and that meets the definition of RIFLE stage I or F for recovery from acute kidney injury, the method comprising:

performing an assay configured to detect Chitinase-3-like protein 1 in a body fluid sample obtained from the subject to provide an assay result;

correlating the assay result to a likelihood of recovery from acute kidney injury as defined by the subject having a maximum RIFLE stage of R over a recovery period beginning within 72 hours after the sample was obtained by:

comparing the assay result to a threshold value obtained from a population study, wherein the threshold separates the population into a first subpopulation above the threshold and a second subpopulation at or below the threshold, the second subpopulation having an increased likelihood of recovering from acute kidney injury relative to the first subpopulation, and

assigning the subject to the first subpopulation or the second subpopulation; and

treating the subject based on the predetermined subpopulation of individuals to which the subject is assigned, wherein when the assay result assigns the subject to the first subpopulation, the subject is treated for non-recovery from acute kidney injury by initiation of renal replacement therapy.

2. The method of claim 1 , wherein the assay result comprises a measured concentration of Chitinase-3-like protein 1.

3. The method of claim 1 , wherein said correlating step comprises combining a plurality of assay results for different markers, one of which is the Chitinase-3-like protein 1 assay result, using a function that converts the plurality of assay results into a single composite result.

4. The method of claim 1 , wherein the subject meets the definition of RIFLE F when the sample is obtained.

5. The method of claim 1 , wherein the recovery period begins 12 hours after the sample is obtained.

6. The method of claim 1 , wherein the recovery period begins 24 hours after the sample is obtained.

7. The method of claim 1 , wherein the recovery period begins 48 hours after the sample is obtained.

8. The method of claim 1 , wherein the recovery period is at least 24 hours long.

9. The method of claim 7 , wherein the recovery period is at least 24 hours long.

10. A method for evaluating renal status in a subject not currently receiving dialysis and that meets the definition of RIFLE stage I or F for recovery from acute kidney injury, the method comprising:

performing an assay configured to detect Chitinase-3-like protein 1 on a body fluid sample obtained from the subject to provide an assay result;

correlating the assay result to a likelihood of the subject recovering from RIFLE stage I or F acute kidney injury as defined by the subject having a maximum RIFLE stage of R over a recovery period beginning 12 hours after the sample was obtained and lasting at least 24 hours by:

comparing the assay result to a threshold value obtained from a population study, wherein the threshold separates the population into a first subpopulation above the threshold and a second subpopulation at or below the threshold, the second subpopulation being at an increased likelihood of recovering from acute kidney injury relative to the first subpopulation, and

assigning the subject to the first subpopulation or the second subpopulation; and

treating the subject based on the subpopulation of individuals to which the subject is assigned, wherein when the assay result assigns the subject to the first subpopulation, the subject is treated for non-recovery from acute kidney injury by initiation of renal replacement therapy.

11. The method of claim 10 , wherein the recovery period is at least 48 hours long.

12. A method for evaluating renal status in a subject not currently receiving dialysis and that meets the definition of RIFLE stage I or F for persistence of acute kidney injury, the method comprising:

performing an assay configured to detect Chitinase-3-like protein 1 on a body fluid sample obtained from the subject to provide an assay result;

correlating the assay result to a likelihood of the subject having persistent RIFLE stage I or F acute kidney injury as defined by the subject having a minimum RIFLE stage of I over a persistence period beginning within 72 hours after the sample was obtained by:

comparing the assay result to a threshold value obtained from a population study, wherein the threshold separates the population into a first subpopulation above the threshold and a second subpopulation at or below the threshold, the first subpopulation being at an increased likelihood of having persistent acute kidney injury relative to the second subpopulation, and

assigning the subject to the first subpopulation or the second subpopulation; and

treating the subject based on the subpopulation of individuals to which the subject is assigned, wherein when the assay result assigns the subject to the first subpopulation, the subject is treated for persistent acute kidney injury by initiation of renal replacement therapy.

13. The method of claim 12 , wherein the assay result comprises a measured concentration of Chitinase-3-like protein 1.

14. The method of claim 12 , wherein said correlating step comprises combining a plurality of assay results, one of which is the Chitinase-3-like protein 1 assay result, using a function that converts the plurality of assay results into a single composite result.

15. The method of claim 12 , wherein the subject meets the definition of RIFLE F when the sample is obtained.

16. The method of claim 12 , wherein the persistence period begins 24 hours after the sample is obtained.

17. The method of claim 12 , wherein the persistence period begins 48 hours after the sample is obtained.

18. The method of claim 12 , wherein the persistence period begins 72 hours after the sample is obtained.

19. The method of claim 12 , wherein the persistence period is at least 24 hours long.

20. The method of claim 17 , wherein the persistence period is at least 24 hours long.
