Abstract

Apparatus and methods are provided for treating a tissue opening, for example a trocar opening used in a minimally invasive surgical procedure. In a tissue closure device, a finger guard or shield can be used, handle or holding area or areas can be used, and positioning indicators can be used. A resiliently flexible target material can also be used for reliably holding a suture during the procedure. Introducers can be used having a suture holder, an alignment indicator and insertion limits.

Claims (17)

1. A tissue closure assembly, comprising:

a longitudinally extending body having a proximal portion for manual disposition and having a distal portion for insertion into an opening in a tissue layer, the distal portion having a first wall extending to a suture holding element supported by the distal portion, and the proximal portion including a shield having a laterally extending surface beyond the first wall,

wherein a first needle-receiving opening is defined at a first axial position on the longitudinally extending body and extends through the laterally extending surface of the shield, and an exit opening is defined at a second axial position on the longitudinally extending body distal of the first axial position and on a periphery of the first wall, and

wherein the longitudinally extending body defines a linear path through the laterally extending surface of the shield and the first wall and from the first needle-receiving opening to the exit opening, and wherein the linear path is aligned with the suture holding element when the suture holding element is in a deployed position.

2. The tissue closure assembly of claim 1 , further comprising a pull rod extending along a length of the longitudinally extending body, wherein a distal end of the pull rod is connected to the suture holding element, and

wherein actuation of the pull rod in a proximal direction, relative to the longitudinally extending body, extends the suture holding element laterally towards the deployed position.

3. The tissue closure assembly of claim 2 , wherein actuation of the pull rod in a distal direction, relative to the longitudinally extending body, retracts the suture holding element laterally away from the deployed position.

4. A method of treating a tissue opening comprising:

inserting the tissue closure assembly of claim 1 into the opening until a tissue surface becomes substantially aligned with an indicator on the longitudinally extending body, but before any laterally extending surface of the body contacts the tissue surface, after the indicator is substantially aligned with the tissue surface, deploying the suture holding element from the longitudinally extending body relative to the tissue.

5. The method of claim 4 , further including manually grasping a surface on the longitudinally extending body proximal of the tissue surface and distal of a shield on the longitudinally extending body.

6. A tissue closure assembly, comprising:

a longitudinally extending body having a proximal portion for manual disposition and having a distal portion for insertion into an opening in a tissue layer, the distal portion having a first wall extending to a suture holding element supported by the distal portion, and the proximal portion including a surface extending laterally outward beyond the first wall,

wherein a first needle-receiving opening is defined at a first axial position on the longitudinally extending body and extends through the surface, and an exit opening is defined at a second axial position on the longitudinally extending body distal of the first axial position and on a periphery of the first wall such that the body defines a linear path from the first needle-receiving opening to the exit opening, and

wherein the suture holding element extends perpendicularly to the longitudinally extending body, in a deployed position, and includes an opening aligned with the first needle-receiving opening.

7. The tissue closure assembly of claim 6 , further comprising a pull rod extending along a length of the longitudinally extending body, wherein a distal end of the pull rod is connected to the suture holding element, and

wherein actuation of the pull rod in a proximal direction, relative to the longitudinally extending body, extends the suture holding element laterally towards the deployed position.

8. The tissue closure assembly of claim 7 , wherein actuation of the pull rod in a distal direction, relative to the longitudinally extending body, retracts the suture holding element laterally away from the deployed position.
