Abstract

Described are devices, methods, and systems useful in the treatment of fistulae, and in certain embodiments those having openings extending into the alimentary canal, such as anorectal fistulae. Illustratively, an anorectal fistula can be treated by placing a volumetric construct within the primary opening of the fistula. In certain embodiments, the volumetric construct can include a rolled remodelable material processed to form a substantially unitary body. Advantageous such remodelable materials can include collagenous extracellular matrix materials, such as small intestine submucosa.

Claims (26)

1. A method of forming a medical graft useful for treating a fistula, the fistula having a fistula passage extending from a primary opening to a secondary opening, the method comprising:

placing a rolled biocompatible sheet material into a mold; and

drying the rolled biocompatible sheet material within the mold so as to stabilize the rolled biocompatible sheet material in a form comprising a plug body extending from a leading end to a trailing end, said plug body comprising a tapered head and an elongate portion such that a plug diameter at said leading end is smaller than a second plug diameter at said trailing end, wherein the medical graft is configured for receipt in the fistula such that the tapered head is wedged within the primary opening when the elongate portion is pulled through the primary opening towards the secondary opening, wherein the medical graft has an interior filled with contacting layer portions of the rolled biocompatible sheet material, said drying bonds the contacting layer portions of the rolled biocompatible sheet material together, and wherein the tapered head has a maximum diameter larger than a maximum diameter of the elongate portion.

2. The method of claim 1 , comprising compressing the rolled biocompatible sheet material within the mold.

3. The method of claim 1 , wherein drying the rolled biocompatible sheet material comprises drying the rolled biocompatible sheet material under conditions causing sublimation of frozen water.

4. The method of claim 3 , wherein the rolled biocompatible sheet material comprises a remodelable material.

5. The method of claim 4 , wherein the remodelable material comprises a naturally derived material.

6. The method of claim 5 , wherein the remodelable material comprises collagenous extracellular matrix (ECM) material.

7. The method of claim 6 , wherein the collagenous extracellular matrix (ECM) material comprises submucosa, dura mater, pericardium, basement membrane, or dermal collagen.

8. The method of claim 6 , wherein the collagenous extracellular matrix (ECM) material comprises dermal collagen.

9. The method of claim 6 , wherein the collagenous extracellular matrix (ECM) material comprises dermal collagen derived from a pig.

10. The method of claim 1 , wherein the medical graft is from a first end of the rolled biocompatible sheet material to a second end of the rolled biocompatible sheet material.

11. The method of claim 1 , wherein the rolled biocompatible sheet material is stabilized in a form having a conical shape.

12. The method of claim 1 , wherein the rolled biocompatible sheet material comprises dermal collagen derived from a pig.

13. The method of claim 1 , wherein the mold presses layers of the rolled biocompatible sheet material together while the rolled biocompatible sheet material dries.

14. The method of claim 1 , wherein the mold presses the rolled biocompatible sheet material into a shape.

15. The method of claim 1 , wherein the rolled biocompatible sheet material is pressed into a polygonal shape.

16. A method of forming a medical graft useful for treating a fistula, the fistula having a fistula passage extending from a primary opening to a secondary opening, the method comprising:

packing a piece or pieces of an extracellular matrix sheet material within a mold; and thereafter

drying the piece or pieces of the extracellular matrix sheet material, wherein the medical graft has an interior filled with contacting layer portions of the piece or pieces of the extracellular matrix sheet material, and said drying bonds the contacting layer portions of the piece or pieces of the extracellular matrix sheet material together, and wherein the piece or pieces of the extracellular matrix sheet material is stabilized in a form comprising a plug body extending from a leading end to a trailing end, said plug body comprising a tapered head and an elongate portion such that a plug diameter at said leading end is smaller than a second plug diameter at said trailing end, and wherein the tapered head has a maximum diameter larger than a maximum diameter of the elongate portion.

17. The method of claim 16 , wherein the piece or pieces of the extracellular matrix sheet material are dried while contained within the mold.

18. A method of forming a medical graft useful for treating a fistula, the method comprising:

placing a rolled biocompatible sheet material into a mold; and

drying the rolled biocompatible sheet material within the mold so as to stabilize the rolled biocompatible sheet material in a form configured for receipt in the fistula, and wherein the rolled biocompatible sheet material is pressed into a polygonal shape.

19. The method of claim 18 , wherein the rolled biocompatible sheet material comprises collagenous extracellular matrix (ECM) material.

20. The method of claim 19 , wherein the collagenous extracellular matrix (ECM) material comprises dermal collagen derived from a pig.
